Perkutānās traheostomas pacientu aprūpe

Abstract

Bakalaura darba tēma “Perkutānās traheostomas pacientu aprūpe” izvēlēta, ņemot vērā šīs procedūras arvien plašāko pielietojumu intensīvās terapijas nodaļās un nepieciešamību izprast aprūpes specifiku. Tēmas aktualitāti nosaka pieaugošais pacientu skaits ar perkutānām traheostomām, kā arī nepieciešamība nodrošināt kvalitatīvu un drošu aprūpi. Aprūpes kvalitāte šādiem pacientiem ir tieši saistīta ar māsas zināšanām, praktiskajām iemaņām un izpratni par aprūpes principiem. Darba mērķis – izzināt pacientu ar perkutāno traheostomu aprūpes principus intensīvās terapijas nodaļā. Pētījumā tika izvirzīts pētniecības jautājums: Kādi ir perkutānās traheostomas pacientu aprūpes principi intensīvās terapijas pacientiem? Izmantota kvalitatīvā pētniecības metode; pētījuma instruments – daļēji strukturēta intervija. Respondenti – 10 māsas ar darba stāžu līdz trim gadiem ITN. Datu ieguvei veikta interviju transkripcija un kontentanalīze. Rezultāti parāda, ka svarīgākie aprūpes aspekti ietver traheostomas vietas un brūces kopšanu, kaniles tīrīšanu un nomaiņu, sekrēta atsūkšanu, infekcijas profilaksi, pacienta pozicionēšanu un barošanas procesu. Būtiskas problēmas saistītas ar personāla noslodzi, aprūpes algoritmu trūkumu un nepietiekamu apmācību. Māsas pauda nepieciešamību pēc vienotas aprūpes vadlīniju ieviešanas un regulāras profesionālās pilnveides. Pētījums atklāj gan praktiskās aprūpes izaicinājumus, gan emocionālo slodzi māsu darbā ar smagiem pacientiem un akcentē nepieciešamību pēc uzlabojumiem veselības aprūpes sistēmā.

The bachelor’s thesis “Percutaneous Tracheostomy Patient Care” was chosen due to the growing use of this procedure in intensive care units (ICUs) and the need to understand its specific nursing requirements. The topic’s relevance is determined by the increasing number of patients with percutaneous tracheostomies and the need for high-quality, safe care. The quality of patient care is closely linked to nurses’ knowledge, practical skills, and understanding of care principles. The aim of the study was to identify the care principles for patients with percutaneous tracheostomy in ICU settings. The research question was: What are the care principles for patients with percutaneous tracheostomy in intensive care? A qualitative research design was used, with a semi-structured interview as the primary data collection instrument. The study involved 10 nurses with up to three years of experience in ICU. Content analysis of interview transcripts was conducted. Results highlighted key aspects of care: stoma and wound management, tube hygiene and replacement, secretion suctioning, infection prevention, patient positioning, and nutrition. Challenges identified included staff workload, the lack of clear care protocols, and insufficient training. Nurses emphasized the need for unified guidelines and continuous professional development. The study reveals both the practical and emotional challenges faced by nurses in caring for critically ill tracheostomized patients and stresses the importance of systemic improvements in healthcare.
