Interferona ietekme uz vairogdziedzera funkcionālo stāvokli vīrusa hepatīta C pacientiem

Pacientu, kas slimo ar hronisku vīrushepatīta C infekciju ārstēšanā lieto pegilēto interferonu alfa kombinācijā ar ribavirīnu. Neskatoties uz rezultātu uzlabošanos infekcijas ārstēšanā, interferona terapijai novērotas vairogdziedzera komplikācijas, kas ir galvenais terapijas pārtraukšanas iemesls. Ir novērojumi, ka interferons alfa veicina tireoidīta attīstību, izmainot humorālo un šūnu imūno atbildi un iedarbojoties tireotoksiski. Interferona inducēts tireoidīts izpaužas kā autoimūns vai arī kā destruktīvs tireoidīts.
Pētījuma mērķis
Izvērtēt vairogdziedzera funkcionālo stāvokli pacientiem, kas slimo ar hronisku vīrushepatīta C infekciju un kuru ārstēšanā lieto pegilēto interferonu alfa.
Darba materiāli un metodes
Pētījumā tika iekļauti pacienti ar diagnozi „hronisks vīrushepatīts C”. Pacienti tika iedalīti divās grupās. Pirmajā grupā tika iekļauti pacienti, kuri tika ārstēti ar pegilēto interferonu alfa-2a (Pegasys) kombinācijā ar ribavirīnu, bet otra pacientu grupa tika ārstēti ar pegilēto interferonu alfa-2b (Pegintron) kombinācijā ar ribavirīnu. Abu grupu pacienti tika iedalīti divās apakšgrupās: pirmajā tika iekļauti pacienti vecumā no 21 līdz 54 gadiem, otrajā no 55 līdz 65 gadiem.
Iegūto datu statistiskā analīze veikta izmantojot programmu Microsoft Excel 2002, nosakot % un konfidences intervālu.
Rezultāti
1. Pegilēto interferonu lietošanas laikā 23.8% pacientu novēro vairogdziedzera darbības traucējumus. 18.4% pacientu, kuru ārstēšanā lieto pegilēto interferonu alfa-2a kombinācijā ar ribavirīnu, novērojama tireoīdstimulējošā hormona paaugstināšanās un/vai samazināšanās.
⅓ daļai pacientu (33.3%), kuru ārstēšanā lieto pegilēto interferonu alfa-2b kombinācijā ar ribavirīnu, novēro tireoīdstimulējošā hormona paaugstināšanos un/vai samazināšanos. Tireoīdstimulējošā hormona izmaiņas sievietēm (47.6%) šajā grupā novēro vairāk kā divas reizes biežāk nekā vīriešiem (22.2%).
2.Pacientiem, kuru ārstēšanā lieto pegilēto interferonu alfa-2b (Pegintron) kombinācijā ar ribavirīnu, biežāk novēro tādas tireoīdstimulējošā hormona izmaiņas, kas raksturīgas destruktīvam tireoidītam (klīniskās gaitas 3 fāzes, kas atspoguļojas tireoīdstimulējošā hormona izmaiņās).
Tireoīdstimulējošā hormona izmaiņas pacientiem, kuru ārstēšanā lietoja pegilēto interferonu alfa-2a (Pegasys) kombinācijā ar ribavirīnu, ir līdzīgas izmaiņām ko novēro pie autoimūna tireoidīta (tireoīdstimulējošā hormona supresija).
Secinājumi
Pacientu, kas slimo ar hronisku vīrushepatīta C infekciju, ārstēšana ar pegilēto interferonu alfa rada būtiskas tireoīdstimulējošā hormona izmaiņas, kas sevišķi spilgti izpaužas ārstēšanā lietojot Pegintron. Ārstējot pacientus ar interferonu alfa, nepieciešama ne vien tireoīdstimulējošā hormona, bet arī vairogdziedzera autoantivielu (tireoglobulīna, tireoperoksidāzes, tireoīdstimulējošā hormona receptoru antivielas) kontrole dinamikā. Interferon alpha in combination with ribavirin has been widely used in treatment of patients with chronic hepatitis C virus infection. Despite major improvements in the treatment of hepatitis C, interferon is associated with serious thyroid complications and may result in discontinuation of interferon therapy. Interferon alpha precipitates thyroiditis by both immune modulatory mechanisms and direct thyroid toxic effects. Interferon induced thyroiditis can manifest both as autoimmune thyroiditis and as destructive thyroiditis.
Aim
To evaluate effects of interferon alpha on thyroid function in patients with chronic hepatitis C virus infection.
Materials and methods
Patients with diagnosis hronic hepatitis C were eligible for the study. All patients were divided in two major groups. In group one were included patients who received pegilated interferon alpha-2a in combination with ribavirin, group two received pegilated interferon alpha-2b in combination with ribavirin. Both groups were further divided in two age based subgroups. In first subgroup were included patients in age range from 21 to 54 years, in secon subgroup were included patients in age range from 55 to 65 years.
Obtained data were analysed using Microsoft Excel 2002 (%, Confidence).
Results
1.Thyroid disorder developed in 25% of patients who received pegilated interferon. Increased and/or decreased levels or thyroid stimulating hormone were observed in 18.4% of patients who recieved pegilated interfero alpha-2a in combination with ribavirin, and in 33.3% or in ⅓ of patients who were treated with pegilated interferon alpha-2b. Changes in thyroid stimulating hormone levels were more than two times more common between women (47.6%) in interferon alpha-2b group than in men (22.2%).
2.Patients, who were treated with pegilated interferon alpha-2b (Pegintron) in combination with ribavirin, developed thyroid stimulating hormone changes characteristic to destructive thyroiditis (3 clinical stages that correspond to thyroid stimulating hormone changes).
Changes in thyroid stimulating hormone in patients who were treated with interferon alpha-2a (Pegasys) in combination with ribavirin, are similar to changes observed in autoimmune thyroiditis (total suppression of thyroid stimulating hormone).
Conclusions
Treatment of patients with chronic hepatitis C infection with pegilated interferon alpha is associated with substantial changes in thyroid stimulating hormone levels that are particuraly prominent when patients are treated with Pegintron. To reduce development of thyroid autoimmunity it is essential that physicians treating patients with interferon alpha not only monitore thyroid stimulating hormone levels but also thyroid antibodies (thyroid peroxidase antibodies, thyroglobulin antibodies, thyroid stimulating hormone receptor antibodies).
