Kopsavilkums
Urīnceļu infekcijas ir viena no biežākajām nozokomiāli iegūtajām infekcijām. Visbiežāk šī infekcija skar vecākus cilvēkus (>50 gadiem), kaut gan vēl joprojām pastāv dažādas domstarpības zinātniskajā literatūrā attiecībā uz šo apgalvojumu. Nozokomiālās urīnceļu infekcijas ir nopietns saslimstības un mirstības iemesls hospitalizētiem pacientiem.
Mērķis un uzdevumi
Šī pētījuma mērķis ir nozokomiālās urīnceļu infekcijas etioloģisko faktoru analīze traumatoloģijas slimnīcā.
Uzdevumi:
1.Izpētīt urīna analīžu rezultātus Traumatoloģijas un Ortopēdijas slimnīcā, kas ietvēra 1 gada periodu
2.Analizēt urīnceļu infekciju etioloģisko faktoru spektru
3.Analizēt pacientu, kuriem diagnosticēta urīnceļu infekcija, dzimumu un vecumu
4.Analizēt izolēto uropatogēnu antibakteriālo jutību
Materiāli un metodes
Pētījumā tika analizēti pacientu dati no 1.-5. nodaļai (ieskaitot traumas centru, mugurkaula ķirurģijas nodaļu, intensīvās terapijas nodaļu ), kuriem Rīgas Traumatoloģijas un Ortopēdijas slimnīcā tika diagnosticēta urīnceļu infekcija. Pētījumā tika iekļauts laika posms no 2008. gada oktobrim līdz 2009. gada decembrim.
Laika posmā no 2008. gada oktobrim līdz 2009. gada decembrim no 168 pacientiem tika savāktas urīna analīzes un 50 no tiem bija pozitīva kultūra uz uropatogēniem.
Rezultāti
No 168 urīna analīžu paraugiem 50 gadījumos (30%)bija pozitīva kultūra uz uropatogēniem, bet 118 gadījumos (70%) baktērijas netika diagnosticētas.
Dzimumu attiecība starp pacientiem, kuriem bija pozitīva baktēriju kultūra urīnā, attiecīgi bija 78% (sievietes) un 22% (vīrieši). 53% gadījumu urīnceļu infekcijas etimoloģiskais faktors bija E. Coli, otrais visbiežāk izdalītais mikroorganisms bija Enterococcus faecalis (17%).
Intensīvās terapijas nodaļā visbiežāk urīnceļu infekciju izraisīja E. coli (27%) un Enterococcus faecalis (27%).
Trešajā nodaļā, kurā kopējais izolēto urīnceļu patogēnu skaits bija vislielākais, visbiežāk urīnceļu infekciju izraisīja E. coli (57%) un Enterococcus faecalis (17%).
Visvairak polirezistento celmu bija P. aeruginosa, E. coli, E. faecalis, Citrobacter spp. kultūrās.
Secinājumi
1.Laika posmā no 2008. gada oktobrim līdz 2009. gada decembrim no 168 pacientiem tika savāktas urīna analīzes, 50 no tiem bija pozitīvas, bet 118 negatīvas kultūra uz uropatogēniem.
2.Urīnceļu infekciju galvenokārt izraisīja gram negatīvās nūjiņas no dzimtas Enterobacteriaceae un nefermentējošām baktērijām - Pseudomonas aeruginosa un Acinetobacter baumannii. Visbiežāk izdalītais microorganisms bija E. coli. No gram pozitīvajām baktērijām visbiežāk izdalīja Enterococcus faecalis. No viena urīna parauga tika izolēta Candida albicans.
3.Lielākā daļa pacientu ar urīnceļu infekciju bija sievietes (78%), vidējais vecums šajā grupā bija 66, 94 gadi. Vidējais vecums vīriešiem bija 59,91 gadi. Visos 7 gadījumos, kad pacientiem bija ievietots urīnkatetrs, tika izolētas baktērijas urīnā.
4. Pseudomonas aeruginosa vidū tika reģistrēta multirezistence pret antibiotikām (5 kultūrās no 7), arī E. coli, Enterococcus faecalis, Citrobacter spp. vidū tika atzīmēti multirezistences gadījumi prêt antibiotikām. Background
Urinary tract infections (UTIs) are the most common type of nosocomial infection found in long-term care facilities (LTCFs). Urinary tract infections (UTIs) are common in the elderly, yet there is much disagreement in the literature regarding many aspects of this condition. Nosocomial urinary tract infection is an important cause of increased morbidity and mortality in hospitalised patients.
Aim and tasks
The aim of the study to analyze aetiology of nosocomial urinary tract infections (UTI) in a surgical hospital.
Tasks.
1. To study the results of urine analyses in the Hospital of Traumatology and Orthopaedics during 1 year period.
2. To analyze aetiological spectrum of UTI.
3. To examine the gender and age of patients with UTI.
4. To analyze the antimicrobial susceptibility of isolated uropathogens.
Material and Methods
The study enclosed collection of urine specimens data in Riga Orthopedics and traumatology hospital from unit 1-5 which includes trauma center, spinal surgery unit, and intensive care unit , dating between October 2008 till December 2009.
During the period between October 2008 and December 2009 data of urine analysis was collected from 168 patients, 50 of them had positive tests for uropathogens and 118 had negative tests.
Results
From all urine samples (168) that were collected, in 50 cases (30%) uropathogen was revealed but in 118 cases (70%) there were no bacteria in patient urine.
The gender proportion in patients who had positive uropathogen test in urine samples. 78% were female and only 22% were male. In 53% of cases the etiologycal factor of urinary tract infections was E. coli the second larger percentage was occupied by 17% Enterococcus faecalis.
Isolated strains from patient urine samples that were in Intensive Care Unit are shown, and the most frequently it was E. coli (27%) and Enterococcus faecalis (27%).
The most frequently isolated uropathogens in 3rd Unit of Orthopedics and traumatology hospital were E. coli (57%) and Enterococcus faecalis (17%).The surgical unit 3 is mostly affected by uropathogens.
Polyresistant strains were registered among E.coli, P.aeruginosa, E. faecalis, Citrobacter spp. A.o.
Conclusions
1. During the period between October 2008 and December 2009 data of urine analysis were collected from 168 patients, 50 of them had positive tests for uropathogens and 118 had negative tests.
2. UTI were caused mainly by gramnegative rods – representatives of the family Enterobacteriaceae and non-fermenting bacteria Pseudomonas aeruginosa and Acinetobacter baumannii.E. coli was the most frequent among all isolated strains in hospitalized patients. From grampositive microorganisms Enterococcus faecalis was a leading agent. Candida was isolated from 1 urine sample.
3. The biggest part of patients was female gender (78%) and the average age of this group was 66, 94 years. The average age of mans was 59, 91 years .In all cases when patients were ongoing long term urological catheter (7 patients), they were positive for uropathogens.
4. The highest proportion of polyresistance was registered among Pseudomonas aeruginosa strains ( 5 cultures from 7), polyresistance was found also in E. coli strains, Enterococcus faecalis, Citrobacter spp.
