Miokarda infarkta riska faktori 2. tipa cukura diabēta slimniekiem

NOLŪKS:
Novērtēt majorus riskus faktorus( zem HDL, augsts LDL, hipertonija, korpulence, smēķēšana, alkohols, ģiemenes vēsture), kuri iesaista attīstīties miokardiālu infarktu starp cukura diabēta pacientiem.
PĒTĪJUMA METODE:
Savā pētījuma darbā 60 pacientus intervējuši Paula Stradina Slimnīcā, kardioloģija nodaļā, Rīgā, Latvijā 2008/2009.gadā. Turklāt, pacientu vēstures novērtējuši. Visi izmeklēti pacienti bija vairāk nēka 40 gadu vecuma, no kuriem cukura diabēta pacientus diagnosticējuši ar HbA1c līmeni vairāk nēka 7. Viņu holesterīnu līmeņus izgādājuši no bio-ķīmijas analīzes piesaistītas uz pacientu vēstures.
DARBĪBAS REZULTĀTI:
Visos cukura diabēta pacientos ar miokardiālu infarktu ( zem HDL, augsts LDL un hipertonija) parādījās 90% pozitīvus rezultātus. Starp HDL, Hipertonija, un LDL vērtībām, HDL rezultātus parādījās 97% attīstīties miokardiālu infarktu un hipertonija 95% un LDL 90% attiecīgi. Līdz ar šo viņi ir miokardiāla infarkta majorie riskie faktorie cukura diabēta pacientos. Turklāt, korpulence parādījās 53%, smēķēšana 40%, un alkohols 70% rezultātus attīstīties miokardiālu infarktu cukura diabēta pacientos. Līdz ar šo viņi ir risku faktoru sekundāra grupa. Beidzot, ģimenes vēstures rezultāti bijuši 40%. Korelācijā starp HDL un LDL ar miokardiālu infarktu P vērtību izgādājusi bija mazāk nēka 0,05. Tādēļ rezultāti ir svarīgi.
SECINĀJUMS:
Šis pētījums precizē risku faktoru izplatību piemēram augsts LDL, zem HDL, hipertonija, korpulence, dzimte, smēķēšana, alkohols un ģimenes vērture attīstīties miokardiālu infarktu cukura diabēta pacientos.
Starp visiem riskiem faktoriem- zem HDL, augsts LDL un hipertonija ir majorie un vispārējie riski faktori cukura diabēta pacientos attīstīties miokardiālu infarktu- tādēļ primāri riski faktori.
Diabēta Korpulentiem pacientiem ir paaugstināts risks attīstīties miokardiālu infarktu . Starpība ir liela starp korpulent-infarktu un korpulent-neinfarktu pacientiem – tādēļ parādījās ka korpulenti pacienti kam ir cukura diabēts, viņiem ir divreiz paaugstināts risks attīstīties miokardiālu infarktu.
Cukura diabēta pacientiem kam bija riski faktori piemēram smēķēšana un alkohols attīstījās miokardiālu infarktu – tādēļ sekundāri faktori.
Šajā pētījumā lielākoties pacienti nezināja viņu ģimenes vēstures. Tomēr pacientu piektajai daļai bija miokardiālu infarktu ģimenes vēsture – tādēļ terciārs faktors.
Turklāt pētījumam vajadzīga novērtēt iemeslus iesaista starp riskiem faktoriem un miokardiālu infarktu cukura diabēta pacientos. AIM:
To estimate the major risk factors (low HDL, high HDL, hypertension, obesity, smoking, alcohol, family history) involved in development of myocardial infarction among the type 2 diabetes patients.
SUBJECTS AND METHODS:
In my research work 60 patients were interviewed at Riga Stradina hospital, Cardiology department, Riga, Latvia from the year 2008-2009.Further they were assessed by virtue of patient histories. All patients examined were over the age of 40 years, Type 2 diabetes mellitus patients diagnosed with Hb1Ac greater than 7. Their cholesterol levels were obtained from bio-chemical analysis attached to patient histories.
RESULTS:
In all type 2 diabetes mellitus patients with myocardial infarction low HDL, high LDL and hypertension showed 90% positive results. Among HDL, Hypertension and LDL values, the results of HDL shows 97% in the development of myocardial infarction and hypertension 95% and LDL 90% respectively. So, hereby they are considered as the major or primary risk factors of myocardial infarction in the type 2 diabetes mellitus patients. Further obesity showed 53%, smoking 40% and alcohol 70% results in the development of myocardial infarction in type 2 diabetes mellitus patients. So, they have to be considered as the second group of risk factors. Finally from the family history results were 40%. In correlation between HDL and LDL with myocardial infarction P value obtained was less than 0.05 so the results are considered to be significant.
CONCLUSION:
This study estimates the prevalence of risk factors such as high LDL, low HDL, Hypertension, obesity, gender, smoking, alcohol and family history in development of myocardial infarction in type 2 diabetes mellitus patients.
Among these risk factors low HDL, high LDL and hypertension are the major and most common risk factors among the type 2 diabetes patients in development of CVD especially in myocardial infarction. So, they have to be considered as the primary risk factors.
Obese type 2 diabetic patients have an increased risk of development of myocardial infarction. The ratio between the obese and infracted patients to obese and not infracted patients have a vast difference showing that type 2 diabetes patients who are obese have a two fold increase in the risk of developing myocardial infarction.
Type 2 diabetic patients who were smokers and alcohol abusers mostly developed myocardial infarction than non-smokers and non-alcoholics.so, all these risk factors are considered to be secondary factors that are involved in myocardial infarction development among type 2diabetes patients.
In this study most of the patients were unaware of their family history. But still one fifths of the patients had a family history of myocardial infarction. So, family history is considered as tertiary risk factor.
Further study is needed to describe the etio-pathogenesis involved between the risk factors and myocardial infarction among type 2 diabetes patients.
