Arteriālu stentu izvērtējums pacientiem pēc perifēro asinsvadu stentēšanas

PAMATOJUMS
Perkutāna Translumināla Angioplastija ar sekojošu stenta implantāciju ir relatīvi jauna, plaši akceptēta ārstēšanas tehnika, ko lieto perifērisku maģistralo artēriju stenozes vai oklūzijas gadījumos. Kā jebkurai ārstēšanas metodei arī stentu implantācijai ir iespējamas komplikācijas. Viena no tādām ir stenta lūzums, kam var sekot stentētā segmenta restenozi vai oklūziju.
MĒRĶIS
Mēs meklējām: stentu lūzumu biežumu perifēriskājās artērijās; stenozes un/vai oklūzijas biežumu, restenozes pakāpi vai oklūziju saistībā ar stentu lūzumu; stenta modeļa ietekmi uz lūzumu biežumu; saistību ar stenta implantācijas anatomisko lokalizāciju un stentētā segmenta garuma ietekmi uz stenta lūzumiem.
METODES
Mēs meklējām publicētus zinātniskos rakstus PubMed datu bāzē līdz 2008 gada februārim.
Mēs veicām retrospektīvu kohortu pētījumu apsekojot P.Stradiņa KUS Asinsnsvadu ķirurģijas centra pacientus, kas ārstējušies laika periodā no 1998 lidz 2007. Tika apsekotas trīsdesmit trīs stentu implantācijas 26 pacientiem.
REZULTĀTI
No 33 stentu implantācijām tika konstatēti deviņi stentu lūzumi. Lūzumu sastopamības biežums svārstījās no 0.1 līdz 20/1000 personu-mēnešiem P.Stradiņa KUS Asinsnsvadu ķirurģijas centra pacientiem. Pēc literatūras datiem tas svārstās no 0.6 līdz 60/1000 personu-mēnešiem. P.Stradiņa KUS Asinsnsvadu ķirurģijas centra pacientiem tika atrastas astoņas restenozes vai oklūzijas sastādot 5.5/1000 personas-mēnešus.
SECINĀJUMI
Stentu lūzumu biežums P.Stradiņa KUS Asinsnsvadu ķirurģijas centra pacientiem ir augsts (sastopamības biežums 27%), kā arī restenožu un oklūziju sastopamība ir ievērojami liela (sastopamības biežums 24%). Jo garāks stents, jo biežāk novērojami stentu lūzumi un restenozes vai oklūzijas gadījumi. Sentu modeļu daudzveidība relatīvi nelielajā apsekotajā populācijā neļauj noteikt stentu lūzuma biezumu atkarībā no tā modeļa. Netika novērota sakarība starp lūzumu un restenozes un/vai oklūzijas attīstību. Ir nepieciešami turpmāki pētījumi ar lielāku apsekojamo populāciju un garāku novērojuma laiku jaunu tipu stentiem. BACKGROUND
Percutaneous transluminal balloon angioplasty with stent placement is a widely accepted and clinically useful technique for managing peripheral arterial occlusive disease. Stents have complications postoperatively. One possible complication is stent fracture. Stent fracture may cause restenosis and/or occlusion.
OBJECTIVES
We sought to; determine the rate of stent fracture in the peripheral arteries, identify the rate of stenosis and/or occlusion after stenting and its relation to stent fracture, find out if the brand of the stents used has any influence on the rate of the fracture, and to clarify if there are any relations between the location and length of the peripheral stent and its fracture rate post-operatively.
METHODS
At first we searched the published literatures in PubMed up to February 2008. Then; in “Paul Stradins clinical university hospital” (hereinafter PSCUH), department of vascular surgery, 33 peripheral stenting procedures in 26 patients (21 males and 5 females) were followed up.
RESULTS
Nine fractures out of 33 stenting procedures we reported in PSCUH. The stent fracture’s incidence rate (IR) ranged from 0.1 to 20/1.000 person-months (p-m) in PSCUH vs. 0.6 to 60/1000 (p-m) in literature. Eight re-stenoses and/or occlusions were found in PSCUH with an IR of 5.5/1000 p-m. Nitinol and Wallstent did not fracture.
CONCLUSION
The rate of stent fracture in peripheral arteries in PSCUH is slightly high (Cumulative Incidence 27%). The rate of re-stenoses and/or occlusions is moderate (Cumulative Incidence 24%). The longer the stent is the higher will be its fracture and complication rates. Due to the diversity of stent brands used in this small population, a clear-cut conclusion concerning stent brands and their implication on postoperative outcomes can not be drawn. Furthermore, there was no relation between fracture and Re-stenosis and/or occlusion.
Further studies with larger study populations, which will involve new types of stents and will evenly distribute the stenting procedures to the different vascular beds for a longer follow up period, are warranted.
