Nieru scintigrāfija, pareiza radiofarmpreparāta izvēle un kvalitātes nodrošināšana

Abstract

Pētījuma tēma: Nieru scintigrāfija, pareiza radiofarmpraparāta izvēle un kvalitātes nodrošināšana. Radiogrāfers ir sertificēta ārstniecības persona, kura ir tiesīga veikt darbības ar jonizējošā starojuma iekārtam, t.s. nukleārā diagnostikā. Radiogrāfers ir atbildīgs par pacienta saņemtā starojuma devu pēc radioloģiska izmeklējuma. Saņemtā starojumu devu ietekmē pielietotā radiofarmpreparāta doza un izmeklējuma protokols. Pētījuma mērķis: Izvērtēt pacienta saņemtā starojuma efektīvo dozu un iegūtā attēla kvalitāti, atkarībā no pielietotā radiofarmpreparāta. Pētījumā izmantota kvalitatīvā metode. Apkopotie rezultāti liecina, ka radiogrāfers var ietekmēt pacienta saņemto starojuma devu, iegūt maksimāli informatīvus attēlus ar minimālu staru slodzi, izvēloties atbilstošus nieru preparātus, protokolus ar optimālākajiem tehniskajiem parametriem, ņemot vērā klīniskās indikācijas, pacienta fizikālos parametrus. Būtu ieteicami īpaši protokoli pediatriskiem pacientiem un pieaugušajiem. Būtiski veikt scintigrāfijas izmeklējumus, izmantojot atbilstošus nieru kitus, kas nodrošina augstas izšķirtspējas attēla iegūšanu pat pie zemas nieru funkijas, pēc kuriem ārsts var izvērtēt nieru funkcionalitāti. Pētījumā atklājās saistība starp nieru dinamiskā scintigrāfijā izmantoto radiofarmpreparātu, iegūtā attēla kvalitāti, pacienta saņemtā starojuma devas samazinājumu un protokola izvēli. Tādēļ pētījuma izvirzītais mērķis tika sasniegts. Pētījumā apstiprinājās izvirzītā hipotēze - izvēloties RFP atbilstoši diagnozei un vecumam, būtiski iespējams samazināt (līdz pat 60%) pacienta, kā arī personāla saņemto starojuma dozu, un iegūt informatīvu attēlu pat pie ļoti vājas nieru funkcijas. Study theme: Renal scintigraphy, the right choise of radiopharmaceutical and quality assurance. Radiographer is a certified medical practitioner who is entitled to work with ionizing radiation equipment. Radiographer is responsible for the patient radiation dose received by radiological examination. Received radiation doses depends on applied radiopharmaceutical dose and examination protocol. Study objective: To evaluate the patient received radiation effective dose and the image quality, depending on the applied radiopharmaceutical. A qualitative method is used in this research work. The results show that radiographer may affect the patient received radiation dose, to get the most informative images with minimal radiation dosed by selecting the appropriate kidney RFP and protocols, by optimizing the technical parameters depending of the clinical indications and patient's physical parameters. It is advised to use special protocols for pediatric patients and adults. It is essential for dynamic renal scintigraphy examinations to use appropriate kidney kits, which provides high-resolution images even at low renal functions, after which the doctor can evaluate renal functionality. The study revealed a correlation between used renal radiopharmaceutical in dynamic scintigraphy, the image quality, the patient received radiation dose reduction and protocol selection. Therefore, the study's objective was achieved. The study confirmed the hypothesis - choosing the RFP according to diagnosis and age can significantly reduce (up to 60%) patients, and staff received radiation doses, and to obtain an informative picture even in case of very poor kidney function.
