Piecu H1-antihistamīnu medikamentu darbības efektivitātes salīdzinoša izpēte

Dažādu alerģiju izplatība pieaug gan visā pasaulē, gan arī Latvijā. Viens no svarīgākajiem mediatoriem, kas izdalās alerģisko reakciju laikā, ir histamīns. Nozīmīgi alerģisko reakciju ārstēšanas medikamenti ir H1-antihistamīna medikamenti. Darba mērķis – izmantojot dažādas metodes, salīdzinoši noskaidrot piecu H1-antihistamīna medikamentu – 50 mg kvifenadīna, 50 mg sekvifenadīna, 5 mg levocetirizīna, 5 mg desloratadīna un 1 mg klemastīna efektivitāti.
Ir samērā maz paņēmienu, ar kuriem pētīt histamīna izraisītās reakcijas un salīdzināt H1-antihistamīnu efektivitāti. Tā kā histamīnam ir liela nozīme ādas papulas un eritēmas reakcijas veidošanās procesā, kas savukārt ir ādas alerģisko reakciju pamatā, tad klasiski ārstnieciskā efekta salīdzināšanai izmanto papulas un eritēmas diametra mērīšanu milimetros, kā arī papulas un eritēmas laukuma aprēķināšanu. Savukārt ņemot vērā histamīna spēju izteikti palielināt asins perfūziju ādā pastiprinot vazodilatāciju un palielinot mikrovaskulāro caurlaidību, par vienu no H1-antihistamīnu terapeitiskās efektivitātes kritēriju var izmantot to spēju samazināt histamīna izraisīto perfūzijas pieaugumu, ko var noteikt ar lāzerdoplerogrāfijas metodes palīdzību.
H1-antihistamīnu sedatīvā efekta novērtēšanai tika izmantoti testi no dažādām psihomotorās aktivitātes testēšanas grupām – tiešās un izvēles reakcijas laika noteikšanas tests; uzmanības koncentrēšanas tests; ciparu – simbolu aizstāšanas (kodēšanas) uzdevums; miegainības vizuālās analoģijas skala (VAS) un Stenfordas miegainības skala.
Pētījumā iegūtie rezultāti rāda, ka papulas un eritēmas lielumu no pētījumā izmantotajiem medikamentiem vislabāk samazināja levocetirizīns, tad desloratadīns, sekvifenadīns, kvifenadīns un klemastīns. Levocetirizīns arī visefektīvāk samazināja perfūzijas pieaugumu procentuāli, kā arī laukumu zem līknes, efektivitātes ziņā tam sekoja desloratadīns, kvifenadīns, klemastīns un sekvifenadīns.
Attiecībā uz H1-antihistamīnu izraisīto sedāciju, pētījumā neizdevās iegūt vienotu un pārliecinošu rezultātu, kurš no pētījumā izmantotajiem H1-antihistamīniem izraisa visizteiktāko un kurš – visminimālāko sedāciju. Ticama sedācijas efekta izvērtēšana pētījumos ir apgrūtināta, jo nav standartizēta veida, kā novērtēt tieši sedatīvo efektu. Pašlaik šāda tipa pētījumos biežāk lietotā metode – vispārēja psihomotorās aktivitātes izvērtēšana – nav specifiska sedācijai, tā ir subjektīva un ar vāju korelāciju starp reālo sedāciju un testu rezultātiem.

One of the main mediators taking part in the pathogenesis of allergic reaction is histamine. Substantial medications that are used to treat allergic reactions are H1-antihistamine medications. The aim of the project is to ascertain the single dose efficacy of five H1-antihistamine medications – 50 mg quifenadine, 50 mg sequifenadine, 5 mg levocetirizine, 5 mg desloratadine and 1 mg clemastine by using various methods.
There are relatively few means by which the reactions generated by histamine can be evaluated and the effectiveness of H1-antihistamine medications can be compared. As histamine plays an important role in the generation of skin wheal and flare reaction that is the base of the dermal allergic reactions, classically, for the comparison of the curative effect the measuring of the diameter and area of the wheal and flare is being used. But for the direct effect that histamine has on the local vasculature by increasing the vasodilatation, microvascular permeability and thus enhancing the blood perfusion in skin, as a criterion for the evaluation of the therapeutical efficiency of the H1-antihistamine medications measuring of the inhibitory effect they have on the increase of the blood perfusion can be used. This is being measured by the means of laser Doppler method.
For the evaluation of the sedative effect of H1-antihistamines various tests can be used, covering different groups of tests evaluating the psychomotor activity – simple and choice reaction time, attention concentration test, digit – symbol coding task, visual analogue scale and Stanford sleepiness scale.
The data obtained in the project indicate that the wheal and flare is best reduced by levocetirizine, the other medications in order of decreased effectiveness – desloratadine, sequifenadine, quifenadine and clemastine. Levocetirizine also showed the greatest potency in decreasing the perfusion both in percents and by measuring the area under the curve, followed by desloratadine, quifenadine, clemastine and sequifenadine.
Regarding the sedation induced by H1-antihistamines, no unified and convincing data concerning the sedative effect of the used medications were obtained in the project. Reliable assessment of the sedation can not be made feasible credibly because there are no standardized means of the evaluation of the sedative effect. The group of methods being used most frequently – assessment of the psychomotor activity – is not specific to sedation and has a poor correlation between the real sedation and the results of the tests.
