Priekškambaru fibrilācijas ārstēšanas ar amiodaronu efektivitāte un joda inducētās hipertireozes risks

Latvijā nav veikti plaši pētījumi saistībā ar amiodarona blakusefektiem un vairogdziedzera patoloģiju.
Konstatēju, ka no 179 pacientiem, kuriem bija noteikti vairogdziedzera hormoni ar amiodarona inducēto tireotoksikozi saslima tikai 2,2% (N=4). Apkopojot datus, nav statistiski ticama saistība starp amiodarona lietošanu un amiodarona inducēto hipertireozi (p=0,195)
Cēloniska sakarība starp amiodarona lietošanu un tireotoksikozes attīstību netika konstatēta. Iespējams, ka amiodarona inducētās tireotoksikozes sastopamība ir biežāka nekā to parādīja veiktā slimības vēsturu datu retrospektīvā analīze, jo nepieciešama vairogdziedzera hormonu līmeņa (fT4, fT3) un TSH noteikšana visiem pacientiem ar priekškambaru fibrilāciju gan pirms, gan pēc amiodarona terapijas uzsākšanas. In Latvia there are no extensive research about side effects of amiodarone and it affects on thyroid gland.
Was found that from 179 patient, with determined hormone levels, only 2,2% (N=4) had thyrotoxicosis. There is no statistically significant relationship between the use of amiodarone and amiodarone-induced hyperthyroidism (p = 0.195)
Conclusions: A causal relationship between amiodarone use and the development of thyrotoxicosis was not detected. It is possible that amiodarone induced thyrotoxicosis incidence is higher than showed medical records of retrospective analysis of data, the need for thyroid hormone levels (FT4, FT3) and TSH determination is required for all patients with atrial fibrillation before and after of amiodarone therapy start.
