| dc.description.abstract | Vislielākais ieguvums no farmaceitiskās aprūpes ir pacientiem ar hroniskām slimībām, kas tiek ārstēti ilgu laika periodu, paralēli lieto vairākas zāles un kuriem ir potenciāli augsts ar zāļu lietošanu saistītu problēmu attīstības risks. Maģistra darba mērķis ir novērtēt farmaceita un tā sniegtās farmaceitiskās aprūpes lomu hronisku pacientu primārajā aprūpē.
Pētījumam tika atlasīti 10 pacienti ar hroniskām slimībām, kam, izvērtējot primārās novērtēšanas procesa intervijas un ikmēneša individuālo aprūpes plānu rezultātus un atbildes, tika analizēta farmaceita sniegtās farmaceitiskās aprūpes nozīme.
Pētījuma laikā pacientiem tika sniegti 125 farmaceita ieteikumi par lietoto zāļu terapiju un dzīvesveida izmaiņām, no kuriem ievēroti 86 norādījumi.

| dc.description.abstract | The greatest benefit of pharmaceutical care is for patients with chronic diseases who are treated for a long time period, concurrently taking a number of medications and have potentially high risk to develop the medication-related problems. The aim of Master’s Thesis is to evaluate the role of pharmacist and it’s provided pharmaceutical care for chronic patients in primary care.
There were 10 patients with chronic diseases selected for study, which estimating results and answers from interviews of primary assessment process and monthly individual patient care plan, the role of pharmacist provided pharmaceutical care was analyzed.
During the study patients were given 125 pharmacists recommendations about used drug therapy and lifestyle changes of which 86 were realized.
