Pasaulē ir aktuāli pētījumi par pepsinogēna I, pepsinogēna II, gastrīna- 17 lomu Helicobacter pylori eradikācijas izvērtēšanā. Īpaši tiek izcelta pepsinogēna II loma.
Darba mērķis bija novērtēt bioķīmisko marķieru, PGI/PGII attiecības un IgG antivielu dinamiku četras nedēļas pēc eradikācijas un izvērtēt to izmantošanas iespēju eradikācijas kontrolei.
Pētījumā piedalījās 42 pacientu grupa ar aktīvu H. pylori infekciju. Pacientu grupai pirms un pēc eradikācijas terapijas tika paņemts analīžu paraugs, lai izvērtētu bioķīmisko marķieru, IgG antivielu un PGI/PGII attiecības izmaiņas. Tika izvērtēts cik pacientiem eradikācija bijusi veiksmīga un cik neveiksmīga kā arī salīdzināti bioķīmisko marķieru rādītāji abās grupās.
Izvērtējot bioķīmisko marķieru dinamiku salīdzinot pirms un pēc eradikācijas rezultātus var secināt, ka eradikācijai praktiski izmantojams biomarķieris ir pepsinogēns II, tomēr šo rādītāju nepieciešams izvērtēt ilgākā laika posmā. IgG antivielu noteikšana nav praktiski pielietojama eradikācijas kontrolei. Paaugstinātas IgG antivielas saglabājas ilglaicīgi. Eradikācijas un PSI terapijas rezultātā ir mazinājies iekaisums un H. pylori blīvums neatkarīgi vai eradikācija bijusi veiksmīga vai neveiksmīga, tapēc abās grupās ir vērojams bioķīmisko marķieru koncentrācijas vērtības samazinājums. Various studies on the role of Pepsinogen I, Pepsinogen II and Gastrin 17 in eradicating Helicobacter Pylori have gained global importance and popularity. Especial attention is drawn to the role of Pepsinogen II.
The scope of the thesis was to examine the biochemical markers, PGI/PGII ratio and IgG antibody dynamics four weeks after eradication and inspect application possibilities for eradication control.
The study involved a sample of 42 patients with active H.pylori infection. Analysis samples were taken before and after the eradication therapy in order to examine changes affecting the biochemical markers, IgG antibodies and PGI/PGII ratio. Further, the quantitative data of patients with successful and unsuccessful eradication were evaluated and the indicators of those groups compared.
Having examined the dynamics of the biochemical markers, pre- and post eradication results, it may be concluded that the biological marker that can be practically used for eradication is Pepsinogen II, however the indicators should be subject to lengthier testing. Determination of IgG antibodies cannot be practically applied for eradication control. High levels of IgG antibodies remain for long. Eradication and PPI therapy have reduced the inflammation and H. pylori tensity, irrespective of whether the eradication was successful or not, and therefore reduction of biochemical markers is observed in both groups.
