Pepsinogēnu kā biomarķieru loma kuņģa vēža diagnostikā

Pasaulē un Latvijā novēro augstus mirstības rādītājus kuņģa vēža gadījumā sakarā ar pārsvarā asimptomātisko slimības norisi un vēlīno diagnostiku. Invazīvas skrīninga metodes, kā endoskopija ar biopsiju veikšanu, ir efektīvas tikai augsta riska reģionos. Līdz ar to nepieciešams izstrādāt jaunas, efektīvas neinvazīvas skrīninga metodes. Visplašāk tiek pētīti pepsinogēnu testi, kas identificē pacientus ar kuņģa gļotādas atrofiju. Darba mērķis bija noteikt pepsinogēnu līmeņus kuņģa vēža pacientiem, tā izvērtējot šo biomarķieru efektivitāti kuņģa vēža pacientu identifikācijai. Pētījumā tika iekļauti 295 pacienti ar morfoloģiski apstiprinātu kuņģa vēža diagnozi. Pepsinogēnu līmeņi tika noteikti, izmantojot ELISA metodi. Rezultāti liecina, ka, izmantojot pepsinogēnu testus, divas trešdaļas kuņģa vēža pacientu netiktu identificētas. Mortality rates of gastric cancer are high in most parts of the world and in Latvia. This is because gastric cancer occurs mostly asymptomatically and the diagnosis is delayed. Invasive screening like upper endoscopy and biopsy sampling is effective in high incidence regions. Therefore there is a need for effective non-invasive screening methods. Pepsinogen testing is the most extensively studied method to screen for gastric atrophy. The aim was to determine pepsinogen levels and to evaluate the efficacy of these tests to identify gastric cancer patients. 295 patients with morphologically confirmed gastric adenocarcinoma enrolled in this study. The levels of pepsinogens were detected using an ELISA method. This research revealed that two thirds from all the gastric cancer cases would be undetected if the pepsinogen tests were used.
