Tēmas aktualitāte:
Akūtas nieru mazspējas (ANM) incidence ar katru gadu pieaug. Tajā pašā laikā ANM diagnostika ir apgrūtināta un tikai neilgu laiku ir pieejamas klīniskās klasifikācijas, kas ļautu savlaicīgi atklāt akūtu nieru slimību. Ņemot vērā to, ka pieaug gan pacientu morbiditāte, gan uzlabojas diagnostika, pieaug arī akūtas nieru slimības incidence intensīvās terapijas nodaļā. Savukārt, lai uzlabotu pacientu prognozi, savlaicīgi veiktu nepieciešamos izmeklējumus un noteiktu nieru bojājumu pirms neatgriezenisku izmaiņu iestāšanās, ir nepieciešams izprast galvenos riska faktorus; jo tikai savlaicīgi diagnosticējot ir iespējams sniegt pilnvērtīgu palīdzību un uzlabot pacienta prognozi.
Pētījuma mērķis:
Izanalizēt ANM incidenci, biežākos etioloģiskos faktorus un to ietekmi uz akūtas nieru mazspējas klīnisko iznākumu Latvijas slimnīcu intensīvās terapijas nodaļās.
Metodes un materiāli:
Šis ir retrospektīvs pētījums, kurā analizētas pacientu medicīniskās slimības vēstures laika posmā no 2013. gada 1. janvāra līdz 2013. gada 31. decembrim. Datu apstrādei tika izmantota SPSS Statistics 22 un Microsoft Exel 2013th programmatūras. Pētījums saņēmis LU EKMI apstiprinājumu.
Rezultāti:
Kopā tika apskatīti 370 pacienti, no tiem 207 bija vīrieši un 163 sievietes. No visiem apskatītajiem pacientiem 108 tika pārvesti uz citām nodaļām, slimnīcām vai izrakstīti no stacionāra, savukārt 262 miruši ITN. 150 pacientiem hospitalizācijas laikā attīstījās dažādas pakāpes akūts nieru bojājums. 58 pacientiem tika uzstādīta diagnoze ANM, savukārt ANB attīstījās 92 pacientiem. ANS 91% gadījumu bija prerenāls cēlonis, biežākie iemesli bija sepse, sirds, plaušu un aknu slimību dekompensācija.
Secinājumi:
ANM ir slimība ar augstu letalitāti, kas ir atkarīga no nieru bojājuma pakāpes.46,48% pacientu ITN tika diagnosticēts nieru bojājums. Pacientiem ar akūtu nieru bojājumu ir garāks stacionēšanas laiks, sliktāka prognoze un augstāka mirstība, nekā pacientiem bez nieru bojājuma. Background:
Acute renal failure (ARF) incidence is increasing every year. At the same time diagnosis of ARF is difficult and only for short time clinical classifications are available, that would allow early detection of acute kidney injury (AKI). The incidence of AKI in the intensive care unit (ICU) is growing because of improvement of diagnostics and increasing number of morbidity. To improve patient prognosis, it is necessary to investigate and to determine kidney damage before irreversible changes. Therefore it is needed to understand the main risk factors; because only with early diagnosis is possible to provide full assistance and to improve the patient's prognosis.
The aim of the study:
To analyze the incidence of ARF, the most common etiological factors and their impact on clinical outcome in ICU in Latvian clinics.
Materials and Methods:
It is a retrospective study, where I analyzed patients who were hospitalized in ICU in the period from 1 January 2013 to 31 December 2013. Data were analyzed with SPSS Statistics 22 and Microsoft Exel 2013 applications. The study has received the approval of LU EKMI.
Results:
During the study I analyzed 370 clinical cases, of whom 207 were men and 163 women. 108 patients were transferred to other departments, hospitals or discharged from the hospital, while 262 died in the ICU. During hospitalization 150 patients developed different stages of acute kidney disease. 58 patients were diagnosed with AKI, 92 with ARF. Of all AKI cases 91% had prerenal cause. Most frequent causes were sepsis, decompensation of heart, lung and liver diseases.
Conclusions:
ARF is illness with high mortality, that depends on the degree of renal impairment. 46.48% of patients in the ICU were diagnosed with kidney impairment. Patients with AKI had longer hospitalization time, worse prognosis and a higher mortality rate than patients without renal impairment.
