dc.description.abstract | Latvijā pieaug aptaukošanos biežums bērnu un pusaudžu vidū. Pētījuma mērķis bija analizēt, vai hormonālo izmaiņu ietekmi uz svara palielināšanas pusaudžiem.
Metodes: Darbs bija veikts BKUS (novietne Gaiļezerā) ambulatorā nodaļā. Retrospektīvi tika analizēti slimības vēstures 12- 16 gadus veciem pacientiem ar diagnozi „Virssvars” un „Aptaukošanās” no 01.09.2014 līdz 01.04.2015. Pacientiem tika novērtēti sekojoši parametri: vecums, dzimums, svars, augums, dzimšanas svars, TSH (mU/l); fT4 līmeni (pmol/l); prolaktīna līmenis asinīs (ng/ml); kortizola līmenis asinīs (nmol/l). Tika salīdzināti pacienti ar un bez hormonālām izmaiņām.
Rezultāti: Pētījuma ietvaros bija izanalizēti 125 pacientu ieraksti (70 zēni un 55 meitenes). Vidējais pacientu vecums bija 13.6 gadi (SD±1.2). Diagnoze „virssvars” bija 34 pacientiem (27.20%); aptaukošanās bija 91 pacientam (72,80%). No mūsu pacientu kopas 56 cilvēkiem (44.80%) bija konstatētas hormonālas izmaiņas pēc bioķīmijas analīzēm: 32 bija primāra hipotireoze (25.60% no pacientu kopas, 57.10% no visam hormonālam izmaiņām); 11 pacientiem bija hiperprolaktīnēmija (8.80% no pacientu kopas, 19.60% no visiem hormonāliem traucējumiem); 23 pacientiem (18.40% no pacientu kopas, 41.00% no visam hormonālam izmaiņām) - hipokortizolēmija. Papildus, astoņiem cilvēkiem (6.4% no pacientu kopas) bija divu vai visu trīs hormonālu izmaiņu kombinācija. Hipokortizolēmija ticami biežāk sastopama meitenēm – 15 gadījumi no 23 (65.21%; p=0.035). Nav atrasta ticama korelācija starp dzimšanas svaru un svaru pusaudžu vecumā. (r=-0.069).
Secinājumi: Pusaudžiem ar virssvaru un aptaukošanos bieži novēro hormonālas izmaiņas (44.80%), kas nereti kombinējas. Pētītajā pacientu kopā primāra hipotireoze bija novērojama biežāk (25.60% pacientu) kā minēts literatūras datos par hipotireozes izplatību populācijā kopumā, pie tam, tās biežums neatšķīrās starp dzimumiem. Hipokortizolēmija ticami biežāk bija sastopāma meitenēm. | en_US |
dc.description.abstract | In Latvia prevalence of obesity among children and adolescents is rising. The aim was to analyze whether hormonal changes influence weight gain in adolescents.
Methods: The work was carried out in BKUS (Gaiļezers) ambulatory department. Retrospectively we analyzed the medical history of 12-16 year old adolescents with a diagnosis of "overweight" and "obesity" from 01.09.2014 till 01.04.2015. The following parameters were evaluated in patients: age, sex, weight, height, birth weight, TSH (mU/l); FT4 levels (pmol/l); prolactin levels (ng /ml); cortisol levels in the blood (nmol/l). These parameters were compared in patients with and without hormonal changes.
Results: In total records of 125 patients (70 boys and 55 girls) were analyzed. The average age of patients was 13.6 years (SD ± 1.2). Diagnosis “overweight” was found in 34 patients (27.20%); Obesity was found in 91 patients (72.80%). Within our patient group hormonal changes in biochemical analysis were found in 56 people (44.80%): 32 had primary hypothyroidism (25.60% of the patient group, 57.10% of all the hormonal changes); 11 patients had hyperprolactinemia (8.80% of the patient group, 19.60% of all hormonal disorders); 23 patients (18:40% of the patient group, 41.00% of all the hormonal changes) - hypocortisolemia. In addition, eight people (6.4% of patients in group) had two or all three of the hormonal changes. Hypocortisolemia was significantly more common in girls - 15 cases of 23 (65.21%, p = 0.035).
We haven’t found a significant correlation between birth weight and the weight in adolescence. (r = -0.069).
Conclusions: In adolescents with overweight and obesity hormonal changes and its combination are often observed (44.80%). In our patient group primary hypothyroidism was observed more frequently (25.60% of patients) as referred to in the literature data on the prevalence of hypothyroidism in the total population as a whole, wherein, the frequency did not differ between the sexes. Hypocortisolemia occur significantly more common in girls. | en_US |
