Ultrasonogrāfijas pozitīvais pienesums mammogrāfijai pacientēm ar pirmreizēji histoloģiski pierādītu krūts vēzi

Pasaulē krūts vēzis ir visizplatītākais ļaundabīgais audzējs sievietēm. Krūšu patoloģiju izmeklēšanā pirmās izvēles diagnostistiskā metode ir mammogrāfija, bet tās sensivitāte ir atkarīga un pacientu krūšu blīvuma, tādēļ nereti nākas pielietot ultrasonogrāfiju. Darba mērķis ir noskaidrot, vai ultrasonogrāfija sniedz pozitīvu pienesumu mammogrāfijai visu pētāmo vecumu grupu pacientēm krūts vēža diagnostikā. Darbā izvirzīti arī vairāki uzdevumi – noskaidrot vai pastāv korelācija starp pacientes vecumu un BI – RADS stadiju nosakot ar MG; noskaidrot vai pastāv korelācija starp pacientes vecumu un BI – RADS stadiju nosakot ar USG; noskaidrot vai pastāv korelācija starp pacientes vecumu un USG pienesumu MG; noskaidrot vai histoloģiskajiem subtipiem pastāv korelācija ar ultrasonogrāfijas pienesumu mamogrāfijai. Krūts vēža klasifikācijai tika izmantota TNM sistēma. Audzēja lokalizācija noteikta izmantojot Starptautisko slimību klasifikatoru (SSK – 10). Pētījumā izmantota BI – RADS sistēma. Sākotnēji darba analīzei tika atlasītas 400 pacientes, kurām histoloģiski pierādīts krūts vēzis laika posmā no 2011. līdz 2015.gadam, pētījumā iekļautas 79 pacientes. Pētījumā iekļautas tikai tās pacientes, kurām veikta gan krūšu USG, gan MG vēža diagnostikai. Pacienšu vidējais vecums 66,7 gadi. Visizplatītākā lokalizācija ir krūts augšējais ārējais kvadrants – 57%. Pētāmajā populācijā biežāk sastopamās stadijas bija IA – 35 % un II A stadija – 25%. BI-RADS V mammogrāfiski diagnostificēts 40%, ultrasonogrāfiski 56%. Ir statistiski ticama saistība starp vecumu un BI – RADS stadiju nosakot ar MG. Pieaugot pacientu vecumam, pieaug BI – RADS atrade, Chi2 tests, p= 0,002. USG nav statistiski ticama saistība starp vecumu un BI – RADS stadiju. Pieaugot pacientu vecumam, BI – RADS atrade nepieaug, Chi2 tests, p= 0,260. Pacientes tika sadalītas divās vecuma grupās, virs un zem 70 gadu vecuma. Ir statistiski ticama korelācija starp vecumu un ultrasonogrāfijas pozitīvo pienesumu mammogrāfijai, Chi2 tests, p= 0,02. Ir statistiski ticama korelācija starp histoloģijas subtipu un USG pozitīvo pienesumu MG, Chi2 tests, p= 0,04. Duktāla karcinoma 32% dod pozitīvu pienesumu, 5% negatīvu. Ultrasonogrāfija sniedz pozitīvu pienesumu mammogrāfijai visās vecuma grupās. Ir statistiski ticama korelācija starp vecumu un BI – RADS stadiju nosakot ar MG. Pieaugot pacientu vecumam, pieaug BI – RADS atrade. USG nav statistiski ticama saistība starp vecumu un BI – RADS stadiju. Pieaugot pacientu vecumam, BI – RADS atrade nepieaug. Duktālas karcinomas gadījumā ultrasonogrāfiju sniedz pozitīvu pienesumu mammogrāfijai. Breast cancer is the most widespread malignant tumor of women in the world. First choice diagnostic in breast pathology investigation is mammography but its sensitivity depends on the density of patients' breasts therefore often ultrasonography is employed. The aim is to discover whether complementing MG with USG improves quality of investigations in breast cancer diagnostic for patients of all age groups. Several tasks are set in the paper: to test whether a correlation exists between patient’s age and BI – RADS stage discovered by MG, to test whether a correlation exists between patient’s age and BI – RADS stage discovered by USG, to test whether a correlation exists between patient’s age and the contribution of USG to MG, to test whether histological subtypes are correlated with the contribution of USG to MG. For the classification of breast cancer the TNM system was used. The localization of the tumor is determined by using the international classification of diseases (ICD – 10). BI – RADS system is employed in the research. Initially, 400 patients were selected for the research with histologically confirmed breast cancer in the time span from 2011 to 2015, 79 patients remained in the final stage of research. Only patients on which both breast USG and MG cancer diagnostic were performed were qualified for the research. The average age of the patients was 66.7 years. Most frequent localization was the top external quadrant of the breast - in 57% of cases. Most frequent stages in the sample IA 35% and II A 25%. BI-RADS V was diagnosed by MG in 40%, by USG in 56% of cases. Statistically significant correlation between age and BI – RADS stage diagnosed by MG was found. With increased age of patients, BI – RADS discovery increased, Chi2 test, p= 0.002. No statistically significant correlation was found between age and BI – RADS stage diagnosed by USG, Chi2 test, p= 0.260. Patients were divided into two age groups, above and below 70 years. Correlation between age and the contribution of USG to MG is statistically significant, Chi2 test, p= 0.02. Correlations between the subtype of histology and the positive contribution of USG to MG is statistically significant, Chi2 test, p= 0.04. USG in ductal carcinoma in 32% of cases contributes positively, in 5% negatively. USG contributes positively to MG in all age groups. Correlation between age and BI – RADS stage diagnosed by MG is statistically significant. The discovery by BI – RADS increases with increased patient’s age. No statistically significant correlation between age and BI – RADS stage diagnosed by USG was found. In the case of ductal carcinoma USG contributes positively to MG.
