Diplomdarba tēma. Krūts vēža ārstēšana gados vecām pacientēm Latvijā. Ievads. Krūts vēzis ir otrā biežākā onkoloģisko slimību lokalizācijas vieta sieviešu populācijā. Ik gadu saslimšana ar krūts vēzi palielinās par aptuveni 1100 jauniem gadījumiem, mirstības līmenis nemazinās, gada laikā mirst aptuveni 430 saslimušās sievietes. Darba mērķis. Izpētīt un apkopot faktus par pacientu saslimstību ar krūts vēzi Latvijā, tā izplatītākos veidus, kā arī saņemto ķirurģisko un vispārējo ārstēšanas taktiku gados vecām pacientēm. Materiāli un metodes. Pētījumā iekļautas 307 krūts vēža pacientes (diagnozes kods - C50, pēc SSK-10), kuras bija stacionētas un ārstētas Latvijas Onkoloģijas centrā no 2010. gada janvāra līdz 2014. gada decembrim. Pacientes tika sadalītas divās grupās: 1) pacientes vecumā no 26 līdz 55 gadiem, 2) pacientes no 75 gadiem un vecākas. Pirmajā grupā – 243 pacientes (79,20%), otrajā grupā – 64 pacientes (20,80%). Datu statistiskā apstrāde tika veikta, izmantojot datorprogrammas IBM (International Business Machines) SPSS (Statistical Package for the Social Sciences) Statistics 22 un Microsoft Excel 2010. Rezultāti. No kopējā pacientu skaita (N = 307), gandrīz 4/5 daļas jeb 79% bija pacientes jaunākajā vecuma grupā (no 26 līdz 55 gadiem), savukārt 1/5 daļa jeb 21% - vecākās grupas pacientes (no 75 gadiem un vecākas). Abās pacientu grupās dominējošais krūts vēža histoloģiskais variants ir vadu tips, t. i., vecuma grupā no 26 līdz 55 gadiem - 87.2%, bet salīdzināmā grupā (75 gadi un vecākas pacientes) - 78.1%), p = 0.067. Vismazāk sastopamais krūts vēža histoloģiskais variants ir papilārais tips (vecuma grupā no 26 līdz 55 gadiem - 0.8%, bet otrā grupā (75 gadi un vecākas pacientes) - 4.7%), p = 0.030. Operatīvās terapijas veidi jaunākajā grupā (pacientes no 26 līdz 55 gadiem) no 243 pacientēm 59 sievietēm tika veikta mastektomija (M), 41 sievietei tika rezecēts attiecīgais sektors + sargmezgls (sekt + SN) un 38 sievietēm plastisks sektors + sargmezgls (pl. sekt + SN). Vecakajā grupā (pacientes no 75 gadiem un vecākas) no 64 pacientēm 28 sievietēm tika veikta mastektomija (M), 11 sievietēm tika rezecēts attiecīgais sektors + sargmezgls (sekt + SN) un 8 pacientēm krūts dziedzera amputācija + sargmezgls (amp + SN). Ārstēšanas kombinācijas pacientēm vecuma grupā no 26 līdz 55 gadiem bija atšķirīgas. No 243 saslimušajām 101 pacientei tika pielietota kombinācija ĶT+ST+HT (ķīmijterapija + staru terapija + hormonterapija), 43 pacientēm ĶT+ST (ķīmijterapija + staru terapija), 20 pacientēm NEOĶT+ĶT+ST (neoadjuvanta ķīmijterapija + ķīmijterapija + staru terapija) un vēl 20 pacientēm ĶT+HT (ķīmijterapija + hormonterapija), 59 pacientēm – citas kombinācijas. Grupā no 75 gadiem un vecākām pacientēm (N=64) dominē divi ārstēšanas veidi, proti, no 64 saslimušajām, 31 pacientei pielietota HT (hormonterapija) un 20 pacientēm HT+ST (hormonterapija + staru terapija), 13 pacientēm – citas ārstēšanas kombinācijas. Breast cancer is the second most common oncological disease localization place of women in the population. Every year, the incidence of breast cancer increases by about 1,100 new cases, the mortality rate is not reduced within a year die approximately 430 diseased women. Aim of the study. To determine patient morbidity, the most common types of cancer, as well as the surgical and general treatment tactics for elderly patients in Latvia. Materials and methods. The study included 307 breast cancer patients who had been hospitalized and treated at Oncology Centre of Latvia from January 2010 until Decembre 2014. The selection includes patients with diagnosis: breast cancer (SSK-10: C50). Patients were divided into two groups: 1) patients from 26-55 years (including), 2) patients from 75 years (including) and older. The first group – 243 patients (79.20%), the second group – 64 patients (20,80%). The statistical processing of a data was accomplished using softwere form IBM (International Business Machines) and SPSS (Statistical Package for the Social Sciences) Statictics 22 and Microsoft Excel 2010. Results. From the total nomber of patients (N=307), almost 4/5 parts or 79% compile patients in the age group from 26 – 55 years (including), but 1/5 part or 21% of the patients were in the age group from 75 years and older. In both patient groups the dominante breast cancer histological type is wire type (26 – 55 years (including) – 87.2%, 75 years and older – 78.1%), p= 0.067, the least common histological type papillary type (26 – 55 years (including) – 0.8%, 75 years and older - 4,7%), p=0.030. The operative types of therapy in the first group of patients (patients from 26 – 55 years (including)), of total 243 patients, 59 women undergoing masectomy (M), 41 women had resected corresponding sector + sentinel node (sekt+SN) and for 38 women plastic sector + sentinel node (pl. sekt SN). Second group (patients from 75 years and older), a total of 64 patients, 28 women underwent masectomy (M), 11 women was resected corresponding sector + sentinel node (sekt + SN) and 8 patients had amputation of mammary gland + sentinel node (amp + SN). The combinations of treatment for patients in the age group from 26 - 55 years (including), it seems that form 243 patients, 101 patient was used combination ĶT+ST+HT (chemotherapy + radiation therapy + hormontherapy) 43 patients ĶT+ST (chemotherapy + radiation therapy), 20 patients NEOĶT+ĶT+ST (neoadjuvant chemoterapy + chemotherapy + radiation therapy) and other 20 patients ĶT+HT (chemotherapy + hormontherapy). In the group from 75 years and older patients (N=64) there are two dominat treatment types, namely, from 64 patients, 31 patients applied to HT (hormontherapy) and 20 patients HT+ ST (hormontherapy + radiation therapy).
