Vairogdziedzera papillāro vēžu sastopamība "P.Stradiņa KUS" un klīnisko gadījumu analīze no 2010.-2015. gadam

Pēc onkoloģijas reģistra datiem, pierādījām, ka Latvijā ir papillāra vēža incidences pieaugums un analizējām kā tas ietekmē operācijas apjomu P. Stradiņa KUS atlasītajiem 250 pacientiem no 2010.–2015. gadam. Mēs noteicām faktorus, kas varētu ietekmēt operācijas apjomu. Statistiski ticami operācijas apjomu ietekmē audzēja izmērs (p=0.031) un FNA papillāram un mikropapillāram vēzim, bet tikai gadījumos, kad veikta totāla tiroīdekotmija (p=0.02). Pierādījām, ka 50% audzēja izmērs ir 1.1–4 cm un totāla tiroīdektomija dominē visās audzēja izmēra grupās. Mikropapillāram vēzim totāla tiroīdektomija visbiežāk tika veikta šūnu atipijas dēļ, bet papillāram - malignas citoloģijas atbildes dēļ. Vecums >45 gadiem nosaka audzēja vēlīnāku stadiju, vīrieša dzimumam ir biežāks recidīva risks, bet TSH līmenis tiks ieviests kā jauns audzēja specifiskais riska izvērtēšanas kritērijs, bet pētījumā nekonstatējām statistisku korelāciju starp šiem riska faktoriem un operācijas apjomu. FNA atbilde ietekmē taktiku, jo pēc tās izvērtē malignitātes risku, bet mēs atklājām statistiski nozīmīgu korelāciju starp FNA un papillāra vairogdziedzera audzēja izmēru (p=0.026), pierādot, ka benignu jeb labdabīgu citoloģijas atbildi visbiežāk diagnosticē audzēja izmērā līdz 0.5 cm, bet malignu PTC izmērā >1.1cm. Pēdējos gados pieaug retrospektīvu pētījumu skaits, kas atzīst parciālu tiroīdekotmiju par pietiekamu, bet P. Stradiņa KUS to veic tikai 12% mikropapillāra vēža gadījumu, turklāt pacientiem, kuriem veikta totāla tiroīdektomija sešas reizes biežāk attīstījās hipokalciēmija (p=0.027). After collecting data from oncology registry, we proved that in Latvia there is increase of incidence papillary cancer and we analysed how it affects the operation amount for a group of 250 patients from 2010 to 2015 in Clinical University Hospital P. Stradins. We identified factors that could affect the operation volume. There was a statistical significant impact on the operation amount and tumor size (p=0.031) and FNA for micropapillary, papillary cancer, but only where performed in totall thyroidectomy (p=0.02). We proved that 50% of the tumors are in size from 1.1-4 cm and totall thyroidectomy dominated in all tumor size groups. For micropapillary cancer totall thyroidectomy mostly was made of FNA diagnostic answer cell atypia, but for papillary malignant cytology answers. Age of over 45 years is the most associated with advanced tumor stages, the male sex have a higher risk of recurrence, but thyrotropin level is going to be a new tumor-specific risk assessment criterion, however this study did not find any statistical correlation between these risk factors and the volume of operations. After FNA diagnostic answers, we could make an assessment for malignancy risk and choose the best trematment option, but we found a statistically significant correlation between FNA and papillary thyroid tumor size (p = 0.026), demonstrating that benign cytology answer is the most commonly diagnosed for tumor size of up to 0.5 cm, and malignant for PTC size >1.1cm. In recent years there have been an increased number of studies, which recognise hemythyroidectomy as sufficient, however in Clinical University Hospital P. Stradins hemythyroidectomy was done only for 12% of micropapillary cancers, patients who have undergone totall thyroidectomy had six times higher incidence of hypocalcemia (p = 0.027).
