dc.description.abstractIevads. Vadīšanas traucējumi pēc transkatetra aortas vārstuļa implantācijas (TAVI) ir bieži sastopami. Ar izmantotu CoreValve protēzi pastāvīgā elektrokardiostimulatora (EKS) implantācija ir līdz 40%. 3. pakāpes atrioventrikulāra (AV) blokāde pēc TAVI ir viens no sinkopes un pēkšņas kardiālas nāves iemesliem, kuru var novērst savlaicīgi implantējot pastāvīgo EKS. Pētījuma mērķis. Novērtēt pastāvīgā EKS implantācijas biežumu pacientiem pēc TAVI procedūras, tā saistību ar iespējamām izmaiņām elektrokardiogrammā, klīniskajiem un ehokardiogrāfiskajiem faktoriem pirms TAVI. Materiāli un metodes. Viena centra pētījumā retrospektīvi tika analizēti 58 pacientu dati, kuriem laika posmā no 2010.gada līdz 2015.gadam tika veikta transaortāla (DA) vai transfemorāla (TF) TAVI ar 26-31 mm CoreValve bioprotēzēm. No pētījuma tika izslēgti 7 pacienti. Atlikušie 51 pacients tika sadalīti divās grupās - ar EKS un bez EKS implantāciju pēc procedūras. Tika analizēti pastāvīgā EKS implantācijas preprocedurālie prognostiskie faktori. Datu statistiskā apstrāde tika veikta ar SPSS 23.0 programmu. Rezultāti. No pacientu skaita (n=51) pastāvīgā EKS implantācija bija nepieciešama 15,7%. 50,0% pastāvīgo EKS implantēja 26 mm CoreValve protēzēm. EKS implantācijas iemesli bija 3. pakāpes AV blokāde (n=5) un nozīmīga bradikardija (n=3) (p=0,206). Vidējais EKS implantācijas laiks bija 2,3 ± 1,4 diena. Pacientiem ar EKS implantāciju (n=8) salīdzinājumā pret pacientiem bez EKS implantācijas (n=43) preprocedurāla Hisa kūlīša labās kājiņas blokāde bija 12,5% vs. 9,3% (p=0,780), ātriju fibrillācija 37,5% vs. 32,6% (p=0,785), sinusa aritmija 12,5% vs. 0% (p=0,019). QRS komplekss pirms TAVI pacientiem ar pastāvīgā EKS implantāciju bija 104 ± 24 vs. bez EKS implantācijas 93,6 ± 18,9 (p=0,098). Klīnisko un ehokardiogrāfisko faktoru ietekme EKS implantācijai netika atrasta. Hisa kūlīša kreisās kājiņas blokāde (HKKKB) pēc TAVI radās 39,2% pacientu. Pacientiem ar radušos HKKKB pēc TAVI tika novērota pozitīva korelācija ar QRS kompleksa platumu pirms TAVI vs. pacientiem bez HKKKB (r=0,346 un p=0,013). 30 dienu mirstība pēc TAVI pacientiem ar EKS implantāciju bija 3,9%, vs. bez EKS implantācijas 0% (p=0,01). Secinājumi. Pacientiem ar pastāvīgā EKS implantāciju tika novērota tendence uz platāku QRS kompleksu, novēroja sinusa aritmiju pirms TAVI. Klīnisko un ehokardiogrāfisko faktoru ietekme EKS implantācijai nebija statistiski nozīmīga. HKKKB pēc TAVI radās >1/3 daļai pacientu. Pacientiem ar radušos HKKKB pēc TAVI bija vērojams statistiski ticams platāks QRS komplekss pirms procedūras nekā pacientiem bez HKKKB. dc.description.abstractIntroduction. Conduction disturbances after transcatheter aortic valve implantation (TAVI) is a common occurrences. The permanent pacemaker implantation (PPI) is up to 40% with the use of CoreValve prosthesis. Third degree atrioventricular (AV) block after TAVI is one of syncope and sudden cardiac death causes, which can be prevented in a timely manner by implantation of permanent pacemaker. Aim of study. Determine the incidence of PPI frequency after TAVI procedure, compare it with the predictive factors on patients electrocardiogram (ECG), clinical and echocardiographic parametrs before TAVI. Materials and methods. There were 58 patients’ data analyzed in a single centre retrospectively study. During the period of time - 2010 to 2015 patients had either direct aortic (DA) or transfemoral (TF) TAVI with CoreValve bioprothesis 26-31 mm. 7 patients were excluded of the study. The remaining 51 patients were divided in 2 groups - with new PPI and with not new PPI, both after the procedure. The PPI preprocedural prognostic factors were analyzed. Statistical analysis data was retrieved using SPSS 23.0 software. Results. Of patients (n = 51) the PPI was required for 15.7%. 50,0% of PPI was implanted with the 26 mm CoreValve prothesis. PPI causes were 3 degree AV block (n = 5) and a significant bradycardia (n = 3) (p = 0.206). The average PPI time was 2.3 ± 1.4 days. The presence in PPI group (n=8) vs. patients without PPI (n=43) of baseline right bundle branch block was 12.5% vs. 9,3% (p = 0.780), atrial fibrillation 37.5% vs. 32,6% (p = 0.785), sinus arrhythmia 12.5% vs. 0% (p = 0.019). QRS complex before TAVI between patients with PPI was 104 ± 24 vs. without PPI 93.6 ± 18.9 (p = 0.098). Any effect of clinical and echocardographic factors on the PPI was not found. Left bundle branch block (LBBB) after TAVI was 39,2%. Patients with LBBB after TAVI were associated with a possitive correlation with the QRS complex width (before TAVI) vs. without LBBB (r=0,346 and p=0,013). 30 day mortality after TAVI in PPI group was 3,9% vs. without PPI 0% (p=0,01). Conclusions. Patients with permanent pacemaker implantation tend to have a wider QRS complex, observed sinus arrhythmia before TAVI. Statistically significant effect of clinical and echocardiographic factors on the PPI was not found. LBBB after TAVI was >1/3 patients. For the patients with the new LBBB after TAVI, prior the procedure there was wider QRS complex (statistically significantly) than it was for patients without LBBB.
