Saskarsmes problēmas onkoloģijā un paliatīvajā aprūpē

Pēdējos gados ziņu avotos bieži tiek minētas nesaprašanās starp ārstu un pacientu. Šajos rakstos ir minēta ārstu nespēja vai nevēlēšanās pacientam pietiekami izskaidrot ārstēšanu un procedūras, veltīt laiku pacientam. Pētījuma mērķis ir noskaidrot pacientu apmierinātību ar saskarsmi un komunikāciju onkoloģijā un paliatīvajā aprūpē kā arī atklāt kļūdas, kas radušās komunikācijā starp ārstu un pacientu. Tika izveidota anketa, kas aptver dažādus saskarsmes elementus. Dati tika analizēti un tika veidotas sakarības starp komunikācijas aspektiem un pacientu apmierinātību ar saskarsmi onkoloģijā un paliatīvajā aprūpē. Datu apstrādes rezultātā atklājās, ka pacienti Latvijā ir gandrīz pilnībā apmierināti ar komunikāciju paliatīvajā aprūpē. Turpretim onkoloģijas aptaujas rezultāti atklāja, ka viena ceturtā daļa no pacientiem nav apmierināti ar saskarsmi šajā jomā. Tālākās datu analīzes rezultātā atklājās četras galvenās komunikācijas daļas onkoloģijā kurās pierādījās kļūdas: •Komunikācijā pirmās vizītes laikā. •Pacienta iesaistīšanā dialogā. •Onkologa tālākajā rīcības plāna skaidrojums. •Pacienta iesaistīšana tālākajā rīcības plāna izstrādē. Šīs problēmas ir iespējams risināt, izmantojot Lielbritānijā pielietotos speciālistu apmācības modeļus. Mūsdienās dialogs ir centrēts uz pacientu. Šāda veida dialogs uzlabo ārstēšanas rezultātu, samazina hospitalizācijas ilgumu un veicina uzticības veidošanos starp pacientu un speciālistu. There have been a lot of mentions about the lack of communication between doctors and patients. Patients comment the lack of a doctor’s capability or wish to explain the treatment and procedures that occurre during treatment. The aim of this research is to gather data that reflect the patient satisfaction with communication in oncology and palliative care and find key points in communication that reflect the errors that occurred during interaction between the doctor and the patient. A forma was made that reflects several parts of communication between the doctor and the patient. The data was analyzed with purpose of finding connections between the overall satisfaction in communication in oncology and palliative care and other aspects of communication. The analyzed data showed that patients in Latvia are almost completely satisfied with the level of communication in palliative care, but the results of the oncology part show the one quarter of patients are not satisfied with the communication. Further analyzing of the data revealed four parts of communication in which errors were found: •Communication during the first visit. •Involving patient in the dialogue. •Explanation of further strategy of treatment. •Involving the patient in the further planning. These problems can be dealt with by specialist attending instructional classes about communication. This type of approach is used in Great Britain. In present time dialogue is patient centered. This type of communication is crucial in improving the outcome of treatment, shortening the patients time spent in hospital and forming a stable connection between the patient and doctor.
