Gūžas locītavas osteoartrīta radioloģisko pazīmju izmaiņas sešu mēnešu periodā pēc mononukleāro šūnu injekcijas

Darba aktualitāte: Osteoartrīts (OA) ir hroniska deģeneratīva locītavu slimība, kura izplatība pieaug līdz ar vecumu. Gūžas locītavas OA samazina cilvēka dzīves kvalitāti. Klasiski OA ārstēšana ir simptomātiska. Tā kā OA procesā galvenokārt tiek iesaistīts locītavas skrimslis, kura pašatjaunošanās notiek ļoti vāji, ir nepieciešama terapija, kas atjaunotu skrimšļa un, iespējams, arī citu locītavas veidojošo audu struktūru. Viena iespēja ir terapija ar mezenhimālajām cilmes šūnām, kas iegūtas no mononukleāro šūnu frakcijas, kas tiek iegūta no paša pacienta kaulu smadzenēm. Mērķis: Noteikt struktūru izmaiņas pēc mononukleāro šūnu injekcijas gūžas locītavās ar OA, izmantojot magnētiskās rezonanses (MR) attēlus. Materiāli un metodes: Tika izvērtēti 17 pacientu MR attēli, kuriem intraartikulāri ir injicētas mononukleārās šūnas. Morfoloģiskās pārmaiņas gūžas locītavā tika izvērtētas pirms mononukleāro šūnu injekcijas un sešus mēnešus pēc šo šūnu injekcijas ar semikvantitatīvu metodi, izmantojot SHOMRI (ang. Scoring Hip Osteoarthritis with MRI) skalu. Rezultāti: Pēc cilmes šūnu injekcijas gūžas locītavā, ciskas kaula galviņas skrimšļa zudums samazinājās par 7%, bet šī skrimšļa atjaunošanās nav statistiski ticama (p=0.28). Kaulu smadzeņu tūska samazinājās par 1%, bet citas gūžas locītavas struktūras pēc terapijas nav uzlabojušās vai ir minimāli pasliktinājušās. Secinājumi: Pēc cilmes šūnu terapijas samazinās skrimšļa zudums. Minimāli samazinās arī kaulu smadzeņu tūska. Citas gūžas locītavu struktūras pēc cilmes šūnu terapijas nav uzlabojušās. Typicality of the thesis: Osteoarthritis (OA) is a chronic degenerative joint disease, which prevalence increases with the age. The hip joint OA reduces a person's quality of life. Typically OA treatment is symptomatic. Since the OA process mainly involves the cartilage of a joint, which regeneration occurs very poorly, the therapy, which would regenerate the cartilage's forming tissues - and possibly other forming tissues of the joint - is needed. An option is a therapy with mesenchymal stem cells, which are being obtained from the mononuclear cell fraction, harvested from the patient's own bone marrow. The aim: To determine the structural changes after mononuclear cell injection in the hip joint with OA by analysing magnetic resonance (MR) images. Materials and Methods: There were evaluated MR images of 17 patients, who had received intra articular injections of mononuclear cells. The morphological changes in the hip joint were evaluated before injection of mononuclear cells and six months' after the injection with semi quantitative method by using SHOMRI (Scoring Hip Osteoarthritis with MRI) scale. The results: After injection of the mononuclear cells in the hip joint, the cartilage loss of femoral head decreased by 7%, but regeneration of the cartilage is not statistically significant (p=0.28). The bone marrow edema decreased by 1%, but other structures of hip joint after the therapy have not improved or minimally worsened. The conclusion: After infection of mononuclear cells the loss of cartilage is decreased. Minimally decreased the bone marrow edema as well. Other structures of the hip joint after mononuclear cell therapy have not increased.
