Azatioprīna terapijas riska izvērtēšana pacientiem ar iekaisīgām zarnu slimībām, pielietojot TPMT enzīma ekspresijas noteikšanas metodi

Abstract

Iekaisīgo zarnu slimību pacientu skaits arvien pieaug ne tikai pasaulē, bet arī Latvijā. Lai novērtētu medikamentu efektivitāti, ir iespējams analizēt enzīmu aktivitāti organismā. Viens no tiem ir TPMT enzīms, ko var novērtēt, lietojot tiopurīnu zāļu terapiju. Pētījuma mērķis bija veikt salīdzinošu TPMT enzīma aktivitātes novērtēšanas analīzi pacientiem ar kādu no iekaisīgām zarnu slimībām (čūlainais kolīts, Krona slimība), kuri jau lieto azatioprīna zāļu terapiju, ar pacientiem, kuri vēl nav sākuši lietot, lai izvērtētu iespējamo zāļu vielas blakņu risku. Lai noteiktu TPMT enzīma ekspresiju, tika izmantota ELISA metode. Pētījuma rezultāti liecina, ka tiem pacientiem, kuri šobrīd lieto azatioprīnu, nomērītais TPMT enzīma aktivitātes līmenis bija normas robežās, kas norāda, ka nav papildus risks, lietojot šo terapiju. Kopumā pētījuma rezultāti liecina, ka TPMT enzīma aktivitātes noteikšana būtu nepieciešama pirms azatioprīna zāļu terapijas, lai novērstu nevēlamās blaknes un izvērtētu terapijas risku, kā arī būtu nepieciešams ieviest Latvijā kā standartmetodi.

Inflammatory bowel disease patients number are growing in the world and also in Latvia. To assess the efficiency of the medication, it is possible to analyze enzyme activities in the body. One of them is TPMT enzyme, which can be assessed using a thiopurine drug therapy. The aim was to make a comparative assessment of the TPMT enzyme activity analysis of patients with an inflammatory bowel disease (ulcerative colitis, Crohn's disease), who are already taking azathioprine drug therapy, with patients who have not yet begun to be use. To determine expression of TPMT enzym using ELISA method. The study results show that patients who currently use azathioprine, the measured TPMT enzyme activity levels were within the normal range, this indicates there is no additional risk associated with this therapy. Overall, the study results suggest that the TPMT enzyme activity would be needed before azathioprine drug therapy to prevent adverse reactions and evaluating treatment risks. It would be necessary to introduce in Latvia as the standard method.
