Dzīves kvalitāte pacientiem ar iekaisīgo zarnu slimībām Latvijā

Ievads: čūlains kolīts (ČK) un Krona slimība (KS) ir divas galvenās neizārstējamās iekaisīgu zarnu slimības (IZS), kuras bez slimībai specifiskajiem simptomiem būtiski ietekmē arī pacientu dzīves kvalitāti (DzK). Darba mērķi: Novērtēt pacientu ar IZS dzīves kvalitāti, korelāciju starp slimības gaitu un DzK, salīdzināt ČK un KS pacientu dzīves kvalitāti un noskaidrot, vai pastāv korelācija starp DzK un klīniskajiem un demogrāfiskiem rādītājiem. Materiāls un metodes: Tika atlasīti gan stacionēti, gan ambulatori pacienti, kuriem tika lūgts aizpildīt IZS anketu (IZSA), novērtējot sociālos, emocionālos u.c. aspektus, kā arī otru anketu ar vispārīgiem un demogrāfiskiem datiem. Rezultāti: Kopā pētījumā piedalījās 58 pacienti. No 41 dalībnieka ar ČK, 23 bija vīrieši un 18 - sievietes ar vidējo vecumu 39,07 gadi (SD±11,42). Pētījuma dalībnieku ar KS vidējais vecums bija 35,47 gadi (SD±12,855), seši no dalībniekiem bija vīrieši un 11 sievietes. Starp ČK un KS dalībnieku IZSA rezultātiem netika atrastas atšķirības. Starp IZSA rezultātiem un dalībnieku dzimumu, izglītības līmeni, smēķēšanu un smēķēšanas stāžu, dzīvesvietu un slimību endoskopisko ainu netika identificētas statistiskas atšķirības. Rādītāji, kuri ietekmēja IZSA rezultātus, bija pētījuma dalībnieku ārstēšanās stacionārā vai ambulatori, hospitalizētiem pacientiem uzrādot ievērojami (p=0,001) zemāku vidējo IZSA rezultātu. ČK pacientiem tika konstatēta nozīmīga (p=0,01) korelācija starp slimības smagumu un IZSA punktu skaitu, savukārt pacientiem ar KS šāda korelācija netika konstatēta. Secinājumi: Abu dzimumu pacientiem ar IZS ir līdzīga DzK. Dzīves kvalitāte nav saistīta ar pacientu izglītības līmeni. DzK nav atkarīga no pacientu dzīvesvietas pilsētā vai laukos, kā arī no tā, vai pacients ir smēķētājs, un viņa smēķēšanas stāža. ČK un KS gadījumā slimības lokalizācija nekorelēja ar pacientu DzK. ČK pacientiem ar vieglu slimības gaitu bija labāka DzK, salīdzinot ar pacientiem, kuru slimības gaita bija smaga, bet KS gadījumā pacientiem bija līdzīgi vidējie rādītāji neatkarīgi no slimības smaguma pakāpes. Background: Ulcerative colitis (UC) and Crohn’s disease (CD) are two main incurable Inflammatory Bowel Diseases (IBD) which besides the disease-specific symptoms affect quality of life (QOL) of the patients. Objectives: The aim of the study was to evaluate the QOL of patients with IBD, the correlation between the disease activity and QOL, to compare the QOL of UC and CD patients, and to find if there is a correlation between the QOL and clinical and demographic variables. Materials and Methods: Patients were recruited from both – hospitalized and outpatient setting and were asked to fill in IBDQ evaluating the social, emotional, etc. aspects and the second questionnaire with general and demographic data. Results: In total 58 patients participated in the study. From the 41 subjects with UC, 23 were males and 18 were females with mean age 39.07 years (SD±11.42). The mean age of the subjects with CD was 35.47 years (SD±12.855), six males and 11 females. There were no differences found in IBDQ scores between UC and CD subjects. No statistical differences were identified between the two genders, education level, smoking and smoking years, city and countryside inhabitants, and the endoscopic distribution of the diseases and the IBDQ scores. The parameters which affected the IBDQ scores were the hospitalization and non-hospitalization of the participants, with hospitalized subjects having significantly (p=0.001) lower mean IBDQ score. The severity of the disease in case of UC showed that there is a strong relationship (p=0.01) between the disease severity and IBDQ scores, in contrast with the CD subjects were the severity of the disease had no correlation. Conclusion: Both genders with IBD have similar QOL, while their education level is not associated with their health related QOL. The QOL is not affected from city and countryside residency, additionally to the habit and the duration of smoking. The distribution of the disease in case of UC and CD showed no correlation with the patients' QOL. Patients with UC with mild disease activity had a better QOL over the ones with severe disease, but in the case of CD the patients had similar mean scores independently on the severity of the disease.
