Deguna ventilācijas novērtējums pacientiem ar hronisku obstruktīvu plaušu slimību

Hroniska obstruktīva plaušu slimība (HOPS) ir elpceļu hronisks iekaisums, kas izraisa pārmaiņas ne tikai perifēros elpceļos, bet arī pārējos elpceļos. Pieaug arī pierādījumu skaits saistībā ar HOPS par pārmaiņām deguna gļotādā, kas ietekmē elpošanu caur degunu. Darba mērķis bija salīdzinoši novērtēt deguna pretestības pārmaiņas pacientiem ar HOPS un noskaidrot sakarības starp deguna pretestību un plaušu funkcionālajiem rādītājiem. Pacientiem ar HOPS (n = 26) un atbilstoša vecuma kontroles grupai (n =15) tika veikta rinomanometrija ar četru fāzu rinomanomanometru (Reff, Reffin un Reffex) un spirometrija (FVC, FEV1, FEV1/FVC (%)). Lai salīdzinātu deguna pretestības HOPS un kontroles grupai izmantojām one way analysis of variance (ANOVA). Lai novērtētu abām grupām sakarību starp deguna un plaušu funkcionālajiem rādītājiem izmantojām kovariācijas analīzi (ANCOVA). Neatradām būtisku atšķirību starp HOPS pacientu un kontroles grupas deguna pretestībām. Neskatoties uz atšķirīgām FEV1/FVC (%) vērtībām salīdzināmajās grupās (p < 0,00001), konstatējām sakarības tendenci, ka līdzīgi abu grupu dalībniekiem - jo zemāks ir FEV1/FVC (%), jo lielāka ir deguna pretestība pirms anemizējošu medikamentu lietošanas (Reff p = 0,0591; Reffin p = 0,0527; Reffex p = 0,0582). Šī sakarība izzūd pēc anemizējošu medikamentu lietošanas (Reff p = 0,317; Reffin p = 0,328; Reffex p = 0,295). Līdzīga sakarības tendence veidojas arī starp FEV1 un deguna pretestībām pirms anemizējoša medikamenta lietošanas (Reff p = 0,0749; Reffin p = 0,0733; Reffex p = 0,0737), bet tā izzūd pēc medikamenta lietošanas (Reff p = 0,854; Reffin p = 0,962; Reffex p = 0,799). Pacientiem ar HOPS nav palielināta deguna pretestība salīdzinājumā ar veseliem indivīdiem, bet ir apgriezta sakarība starp deguna pretestību un elpceļu funkcionālā rādītāja (FEV1/FVC (%) lielumiem. Šāda sakarība izzūd pēc anemizējošu medikamentu lietošanas. Background. Chronic obstructive pulmonary disease (COPD) is a chronic inflammation of the airways, which causes changes not only in the peripheral airways but also in other airways. There is also an increase in the number of evidence of changes in nasal breathing in connection with COPD. Purpose. The aim of the work is to compare the changes in nasal resistances (Reff, Reffin and Reffex) in patients with COPD and to determine the correlation between nasal resistances and pulmonary function (FVC, FEV1, FEV1/FVC(%). Material and Methods. Rhinomanometry with 4-Phase-Rhinomanometer and spirometry were performed in COPD patients (n=26) and in the corresponding age control group (n=15). To compare the nasal resistances in COPD and the control group, we used one way analysis of variance (ANOVA). To evaluate the relationship between nasal and lung functional parameters for both groups, we used the analysis of covariance (ANCOVA). Results. We did not find a significant difference between COPD patients and nasal resistances in the control group. In spite of the different values of FEV1/FVC (%) in comparable groups (p <0.00001), we found an affinity that is similar to those in both groups - the higher the nasal resistances, the lower the FEV1/FVC (%) (Reff p = 0.0591; Reffin p = 0.0527; Reffex p = 0.582). This association disappears after the use of sympathomimetic amine medication (Reff p = 0,317; Reffin p = 0,328; Reffex p = 0,295). A similar tendency is also observed between FEV1 and nasal resistances before the use of sympathomimetic amine drug (Reff p = 0.0749; Reffin p = 0.0733; Reffex p = 0.0737) but it disappears after the drug (Reff p = 0.854; Reffin p = 0.962; Reffex p = 0.799). Conclusions. Patients with COPD have no increased nasal resistances compared to healthy subjects, but have an inverse correlation between nasal resistances and respiratory function (FEV1/FVC (%) values, which potentially might be confirmed by further research.
