Rezistentas hipertensijas sastopamība obstruktīvas miega apnojas pacientu vidū

Obstruktīva miega apnoja (OMA) ir hroniska slimība un viens no visbiežāk sastopamākajiem miega traucējumiem, kas raksturojas ar epizodisku, daļēju (hipopnoja) vai pilnīgu (apnoja), elpceļu aizvēršanos miegā, rezultējoties ar mikropamošanās epizodēm. OMA izšķir kā neatkarīgu riska faktoru kardiovaskulāru notikumu attīstībai, tā tiek uzskatīta par vienu no visbiežāk sastopamajiem rezistentas hipertensijas (RH) cēloņiem. RH asociē ar augstāku kardiovaskulāru un renālu notikumu risku. Darba mērķis - noskaidrot RH sastopamību pacientiem ar pierādītu OMA. Retrospektīvi analizēta informācija no pacientu ambulatorajām kartēm dr. Andas Nodievas ārsta praksē pneimonoloģijā. Iekļauti 114 pacienti ar pierādītu OMA. Par katru pacientu apkopota informācija: dzimums, vecums, OMA pakāpe, apnojas/hipopnojas indekss (AHI) simptomātika, ķermeņa masas indekss (ĶMI), kakla apkārmērs, arteriālā hipertensija (AH) anamnēzē, sistoliskais asinsspiediens (SAS), lietotie hipotensīvie medikamenti, blakusslimības. Izmantotas Eiropas Kardiologu biedrības (ESC) izstrādātās 2013.gada AH vadlīnijas. Pētījumā iekļauti 114 pacienti, 61 vīrietis, 53 sievietes. Vīriešu vidējais vecums - 57, bet sieviešu - 62 gadi. No visiem pacientiem, 87 (76,3%) anamnēzē ir AH. Pacienti ar AH ir par 10 gadiem vecāki, 61,8 un 51,9 gadi. ĶMI, AHI un kakla apkārtmērs vīriešiem un sievietēm ar AH ir lielāki nekā pacientiem bez AH. Normotensiju sasnieguši 22 pacienti, 2 no tiem atbilst RH definīcijai. AH pakāpe un hipotensīvās terapijas anamnēze zināma 68 pacientiem, 25 pacienti (36,8%) atbilst RH definīcijai. RH novēro 13 (52%) sievietēm un 12 (48%) vīriešiem. RH pacienti vidēji saņem 4 antihipertensīvo medikamentu grupas. RH pacientiem novēro vidēji spēcīgu korelāciju ar statistisku ticamību starp kakla apkārtmēru un AHI (rs=0,540; p=0,012). Secinājumi - AH tiek novērota 76% OMA pacientu. Pacienti ar AH, ir vidēji par 10 gadiem vecāki. Pacientiem ar AH tiek novērots lielāks ĶMI, kakla apkārtmērs un AHI. RH ir bieži sastopama OMA pacientu vidū. To vienlīdz bieži novēro sievietēm un vīriešiem. RH pacienti vidēji saņem 4 hipotensīvo medikamentu grupas. RH pacientiem novērots lielāks ĶMI nekā pacientiem ar kontrolētu hipertensiju, kā arī vīriešiem ar RH novērots lielāks kakla apkārtmērs. Obstructive sleep apnea (OSA) is a chronic condition and common form of sleep – disordered breathing. OSA is characterized by frequent episodes of partial (hypopnea) or complete (apnea) upper airway collapse during sleep resulting in reccurent arousals. OSA is an independent risk factor for a variety of adverse cardiovascular outcomes and it is considered as one of the most common causes of resistant hypertension (RH). RH is associated with worse cardiovascular and renal outcomes. The aim is to determine the prevalence of RH in patients with OMA. In dr. Anda Nodieva’s medical practice information of patients ambulatory case record was analyzed retrospectively. 114 patients with OSA were included. The following information was collected for each patient: gender, age, grading of OSA and apnea-hypopnea index (AHI), OSA symptoms, body-mass index (BMI), neck circumference, a presence of arterial hypertension (AH), used antihypertensive drugs, co-morbidities. European Society of Cardiology (ESC) guidelines for the management of Arterial Hypertension (2013) were used. In total, 114 patients were included, 61 men, 53 women. Average age of men – 57, of women – 62 years. Of all patients, 87 (76,3%) had a history of AH. Patients with AH are about 10 years older than those without hypertension - 61,8 and 51,9 years. BMI, AHI and neck circumference is higher in patients with AH. 22 patients have reached normotension of which 2 patients conformed to definition of RH. The history of arterial hypertension grade and used antihypertensive therapy is known for 68 patients, 25 patients conformed to definition of RH. 13 women and 12 men have RH. On average, patients with RH received 4 different antihypertensive drugs. Study demonstrates moderate correlation between neck circumference and AHI in patients with RH (rs=0,540; p=0,012). Conclusions - more than 76% of patients with OSA have AH. Patients with AH are average 10 years older than those without AH. BMI, neck circumference and AHI appear to be larger in patients with AH. More than 1/3 of patients with OSA corresponded to definition of RH. RH is common in patients with OSA. RH is equally common between men and women. On average, patients with RH received 4 different antihypertensive drugs. Patients with RH seemed to have higher BMI, in men with RH larger neck circumference has been observed.
