Endometrija labdabīgo pārmaiņu ķirurģiskās ārstēšanas iespēju salīdzinošā analīze

Abstract

Endometrija labdabīgās pārmaiņas ir izplatīta patoloģija premenopauzes un postmenopauzes pacientēm, kas tiek ķirurģiski ārstēta. Biežākās endometrija labdabīgās pārmaiņas ir endometrija polipi un endometrija hiperplāzija, šīs pārmaiņas var izpausties asimptomātiski un var radīt simptomus, no kuriem biežākā ir patoloģiska dzemdes asiņošana. Pētījuma mērķis ir atlasīt, analizēt un salīdzināt endometrija hiperplāzijas un endometrija polipu ķirurģiskās ārstēšanas metodes balstoties uz ginekologu pieredzi. Pētījumā atlasīti un analizēti literatūras dati par endometrija polipu un endometrija hiperplāzijas ķirurģiskām ārstēšanas iespējām un intervēti ārsti ginekologi dzemdību speciālisti no Paula Stradiņa Klīniskās universitātes slimnīcas, Rīgas Austrumu klīniskās universitātes slimnīcas un Vidzemes slimnīcas. Lielākā daļa no intervētajiem ārstiem kā labāko endometrija polipu ķirurģiskās ārstēšanas metodi atzīst histerorezektoskopiju šīs metodes vizualizācijas, precizitātes un efektivitātes dēļ, ko arī pierāda citās valstīs veiktie pētījumi. Tomēr kā endometrija hiperplāzijas ārstēšanas metode joprojām tiek pielietota dzemdes dobuma abrāzija, kuru kā ārstēšanas metodi neizmanto trīs ginekologi, veicot šo manipulāciju tikai pacientiem ar endometrija hiperplāziju, kurus nosūta ambulatorais ārsts, bet izmanto uz pierādījumiem balstīto patoģenētisko hormonālo terapiju. Vadlīnijas un pētījumi iesaka endometrija hiperplāzijas ārstēšanā pielietot hormonālu terapiju, bet totālu histerektomiju kā vienīgo ķirurģiskās ārstēšanas metodi atipiskas endometrija hiperplāzijas gadījumā.

Endometrial benign alterations are a common prevalence of premenopausal and postmenopausal patients undergoing surgical treatment. The most common endometrial benign alterations are endometrial polyps and endometrial hyperplasia. These changes may be asymptomatic and can cause symptoms that are most commonly associated with abnormal uterine bleeding. The aim of the study is to select, analyze and compare surgical treatment methods for endometrial hyperplasia and endometrial polyps based on the experience of gynecologists. The study selected and analyzed literature data of endometrial polyps and endometrial hyperplasia surgical treatment options and interviewed doctors gynecologists maternity specialists from the Paula Stradiņa Clinical University Hospital, Riga Eastern Clinical University Hospital and Vidzemes Hospital. Most of interviewed doctors as the best method for surgical treatment of endometrial polyps admit hysteroscopic polypectomy due to the visualization, accuracy and efficacy of this method, which is also evidenced by studies conducted in other countries. However, as an endometrial hyperplasia treatment method, cavity abrasion is still used, which is not used as a method of treatment by three gynecologists, only use this manipulation for patients with endometrial hyperplasia sent by an outpatient doctor, but using evidence-based pathogenetic hormonal therapy. The guidelines and studies recommend the use of hormonal therapy in the treatment of endometrial hyperplasia, but total hysterectomy as the only surgical treatment in the case of atypical endometrial hyperplasia.
