Pēcoperācijas perioda ilgums un komplikāciju biežums atkarībā no deguna dobuma tamponādes veida rinoķirurģiskajiem pacientiem

Attīstoties jaunām rinoķirurģijas metodēm un jaunajiem hemostātiskajiem materiāliem otorinolaringoloģijā, ir sevišķi svarīgi izvērtēt to efektivitāti un kvalitāti ambulatorajos apstākļos. Darba mērķis: Izanalizēt rinķorurģisko operāciju komplikāciju un pēcoperācijas perioda ilgumu atkarībā no operācijas veida un pielietota tamponādes materiāla privātklīnikā “Headline” laika periodā no 2017. gada oktobra līdz 2019. gada marta mēnesim. Materiāli un metodes. Šī retrospektīvā pētījuma laikā tika analizēti 504 pacienti (pacienti tika apskatīti visi pēc kārtas, neveidojot izlases). No slimības vēsturēm tika savākta tāda informācija kā pacienta dzimums, vecums, operācijas veids, deguna dobuma tamponādes materiāls, pēcoperācijas vizīšu skaits, dienu skaits līdz pirmajai pēcoperācijas vizītei, asiņošana un citas komplikācijas, antibakteriālo līdzekļu pielietošana pēc operācijas. Iegūtie dati tika apstrādāti, izmantojot Microsoft IBM SPSS V.22 programmu. Ticamības pakāpi novērtējā ar tādiem statistiskiem testiem kā Pirsona Hī kvadrāttests (ꭓ2), vienfaktora dispersijas analīze (One-Way ANOVA), Spīrmena korelācijas koeficients. Secinājumi. 1.Rinķirurģiskās operācijas veids un pacientu vecums pats par sevi neietekmē komplikāciju incidenci, asiņošanas incidenci un postoperatīvo vizīšu skaitu; 2.Deguna tamponāde, izmantojot “Gelita” hemostātisko sūkli ir saistāma ar paaugstināto asiņošanas risku un lielāku pēcoperācijas vizīšu skaitu, bet tamponāde ar formētiem deguna tamponiem, izmantojot “Merocel” PVA-NT tamponu, ir saistāma ar lielāku komplikāciju incidenci, savukārt, silikona plātnīšu pielietošana nav saistāma ar lielāko komplikāciju incidenci, asiņošanas incidenci un postoperatīvo vizīšu skaitu; 3.Profilaktiskās pēcoperācijas antibakteriālās terapijas nozīmēšanas incidence ir atkarīga no konkrēta gadījuma, tomēr visbiežāk tā tiek pielietota funkcionālās endoskopiskās sinusu operācijās, tās nozīmēšanā notiek pēc ķirurga pieredzes un ir atkarīga no operācijas apjoma un pacientu atsaucības. Visbiežāk pielietojamais līdzeklis – Amoxicilīns / klavulanskābe. 4.Dienu skaits līdz pirmajai pēcoperācijas vizītei ir atkarīgs no kopējā vizīšu skaita, jo pacienti ar lielāko postoperatīvo vizīšu skaitu meklē palīdzību ātrāk. As the new rhinosurgery methods and new absorbable hemostatic materials in otorhinolaringology are still being developed nowadays, it is important to evaluate their quality and effectiveness in outpatient settings. Aim: to analyze the postoperative complications and the duration of the postoperative period depending on the type of surgery and used tamponade material in the “Headline” private clinic between October 2017 and 2019 March. Materials and methods. During this retrospective study, 504 patients were analyzed (patients were examined in turn without random sampling). The history of the disease included information such as the patient's sex, age, type of surgery, nasal cavity, postoperative visits, number of days to the first postoperative visit, bleeding and other complications, use of antimicrobials after surgery. The resulting data was processed using Microsoft IBM SPSS V.22. The degree of confidence in the estimation by statistical tests such as Pearson Chi square test (ꭓ2), single-factor dispersion analysis (One-Way ANOVA), Spearman correlation coefficient. Conclusions. 1. The type of surgical operation and the age of the patients do not affect the incidence of complications, bleeding and the number of postoperative visits; 2. Nasal packing using Gelita hemostatic sponge is associated with increased risk of bleeding and higher number of postoperative visits, while nasal packing using Merocel PVA-NT tampon is associated with a higher incidence of complications. Silicone splints are not related to the incidence of complications, the incidence of bleeding and the number of postoperative visits; 3. The incidence of prophylactic post-operative antimicrobial therapy depends on the case, but most often it is used for functional endoscopic sinus surgery, according to the surgeon's experience, type of surgery and patient cooperation. The most commonly used agent is Amoxicillin /Clavulanic Acid. 4. The amount of days before the first postoperative visit depends on the total number of visits, as patients with the highest number of postoperative visits seek help faster.
