Subjektīvā kontroles lokusa un emocionālās inteliģences ietekme uz izdegšanas sindromu Latvijas medicīnas studentiem un rezidentiem

Izdegšanas sindromu veicina hronisks stress. Augsts emocionālais intelekts (EI) pasargā no izdegšanas. Internāls kontroles lokuss (KL), uzskats, ka dzīves notikumi ir atkarīgi no savas rīcības, prognozē zemāku izdegšanu. Mērķis: Izpētīt EI un KL ietekmi uz izdegšanas sindromu medicīnas studentiem un rezidentiem Latvijā. Uzdevumi: Noteikt izdegšanu, EI un KL Latvijas ārstniecības studentiem un rezidentiem un kā tie ietekmē izdegšanu. Materiāli un metodes: Medicīnas studenti un rezidenti šķērsgriezuma pētījumā brīvprātīgi, anonīmi aptaujāti ar sociāldemogrāfisko rādītāju aptauju, EI skalu (Schutte et al., 1998, Latvijā adaptējuši Reņģe un Ņižņika,2012), Internālā–Eksternālā KL skalu (Rotter, 1996, adaptējusi Puķīna, 2000) un Maslačas izdegšanas aptauju–Vispārīgo aptauju (Maslach, Jackson, Leiter, 1996, adaptējusi Caune, 2004). Statistiskā analīze veikta programmā SPSS Statistics 22. Pētījuma būtiskuma līmenis: p<0,05. Rezultāti: 280 respondentiem izdegšana: augsta līmeņa izsīkums ir 58% studentiem, 60% rezidentiem, cinisms – 45% un 56%, augsta profesionālā efektivitāte – 30%. Vidējā EI (119,52 balles) neatšķiras studentiem un rezidentiem. KL: Rezidentiem iekšējs KL ir 52%, studentiem–29%. Spēcīgāka EI korelācija ar izdegšanu ir studentiem, rezidentiem nav korelācijas starp EI un izsīkumu. Rezidentiem ir spēcīgāka KL korelācija ar izdegšanu bet nav korelācijas starp KL un efektivitāti. Pieaugot vecumam, pieaug cinisms, vairāk darbavietu prognozē augstākus cinisma rādītājus. Sievietēm augsts izsīkums ir 2,4 reizes biežāk nekā vīriešiem; nestrādājošiem indivīdiem augsts izdegšanas līmenis atrasts 1,9 reizes biežāk nekā strādājošiem. Secinājumi: Augsts izsīkuma līmenis ir līdz 60 %, cinisms 45 – 56 % studentiem un rezidentiem, augsta profesionālā efektivitāte – tuvu 30 %. Studentiem raksturigs eksternāls KL, rezidentiem – internāls KL. Augstāka EI un internālāks KL izlasē prognozē zemākus izdegšanas rādītājus; pieaugot vecumam un darbavietu skaitam, pieaug cinisma līmenis. Chronic stress contributes to burnout. High emotional intelligence (EI) protects from burnout. Internal locus of control (LOC) predicts lower burnout. Aim: To assess the effect of EI and LOC on Latvian medical students’ and resident doctors’ burnout. Objectives: To determine burnout rates, EI and LOC in medical students and resident doctors and how burnout is affected. Materials and methods: In the cross-sectional study participants were voluntarily, anonymously surveyed using the sociodemographic survey, Maslach Burnout Inventory–General Survey (Maslach, Jackson, Leiter, 1996, adapted in Latvia by Caune, 2004), The Self-Report Emotional Intelligence Test (Shutte et al., 1998, adapted by Reņģe, Ņižņika, 2012), Internal-External LOC Scale (J. B. Rotter, 1966, adapted by Puķīna, 2000). SPSS Statistics 22 was used for the statistical analysis; significance level: p<0.05. Results: 280 participants’ burnout: high exhaustion rates in 58% of students and 60% of the residents, cynicism –45% and 56%, high professional efficacy–about 30%. Mean EI score (119,52) did not differ between students and residents. LOC: 52% and 29% of the residents and students had internal LOC. Stronger correlation between EI and burnout was found in students; no correlation between EI and exhaustion in residents. Stronger correlation between LOC and burnout was found in residents; no correlation between LOC and efficacy. Cynicism increased with age and employment positions. High exhaustion was seen in women 2,4 times more often than in men; unemployed individuals had 1,9 times higher high exhaustion levels compared to working individuals. Conclusions: High exhaustion levels were in up to 60%, cynicism – 45 – 56% of the students and residents, high professional efficacy – almost 30%. Students had more external LOC, residents – internal LOC. Higher EI and more internal LOC predicts lower burnout levels in the given sample; cynicism increased with age and employment positions.
