Antibakteriālo medikamentu rezistences profils pacientiem ar komplicētu urīnceļu infekciju

Strauji augoša baktēriju rezistence pret antibiotikām ir viena no lielākām un visnozīmīgākām problēmām veselības aprūpes sistēmā. Šīs rezistences dēļ, daudzi iepriekš lietotie medikamenti komplicētas urīnceļu infekcijas (cUTI) ārstēšanai kļūst neefektīvi, pasliktinot terapijas iznākumu un pagarinot pacientu hospitalizācijas ilgumu un izmaksas. Tomēr ņemot vērā, ka dažādos reģionos antibakteriālas rezistences dati atšķiras, Eiropas Urologu Asociācijas (EAU) vadlīnijas rekomendē katrā slimnīcā balstīt empīrisku cUTI terapiju uz lokāliem rezistences datiem. Pētījuma mērķis: Izvērtēt Paula Stradiņa Klīniskās Universitātes slimnīcas (PSKUS) Uroloģijas centrā stacionētu pacientu antibakteriālas rezistences datus ar diagnozi komplicēta urīnceļu infekcija. Darba metodes: Retrospektīvs pētījums tika veikts PSKUS Uroloģijas centrā. Tika analizētas pacientu slimības vēstures ar diagnozēm komplicēta urīnceļu infekcija vai akūts komplicēts pielonefrīts. Tika ņemti vērā pacientu demogrāfiskie dati, cUTI attīstību provocējošie riska faktori, urīna uzsējuma izolētu baktēriju spektrs un pielietojamo antibakteriālu medikamentu rezistences dati. Rezultātu raksturojums: Pētījumā tika iekļauti 150 pacienti, 100 ar cUTI un 50 ar akūtu komplicētu pielonefrītu. Visbiežākais cUTI ietekmējošais faktors sievietēm bija nierakmeņu slimība (40,2%), savukārt vīriešiem – labdabīga prostatas hiperplāzija (73,5%). Tika izolēti 15 dažādu baktēriju veidi, trīs biežākie bija E. coli (61,3%), E. faecalis (18,7%) un K. pneumoniae (12,0%). Pētījumā, vislielākā baktēriju rezistence bija pret ampicilīnu (54,4%), trimetoprimu/sulfametoksazolu (33,5%), ciprofloksacīnu (29,5%) un pret norfloksacīnu (29,1%). Pret vadlīnijās rekomendētiem empīriskās terapijas medikamentiem rezistence bija: amikacīns (0,8%), gentamicīns (16,6%), cefotaksīms (12,4%), ceftazidīms (13,3%) un piperacilīns/tazobaktāms (5,5%). Antimicrobial resistance (AMR) of bacteria is one of the most important healthcare problems of 21th century. Due to constant increase of AMR rates, certain drugs, that were historically used for treatment of complicated urinary tract infection (cUTI), now are not as effective as they used to be. This, in turn, worsens clinical outcomes, increases hospital stay and healthcare costs. However, due to variability of AMR rates in different regions, the guidelines of the European Association of Urology recommend to base the choice of empirical antimicrobial agents on local resistance data. Aim of the study: To evaluate the antimicrobial resistance of bacteria in patients with cUTI, who were hospitalised in Urology Centre of Pauls Stradins Clinical University hospital. Methods: The retrospective study was performed in Pauls Stradins Clinical University hospital. Only patients with cUTI and acute complicated pyelonephritis were selected for the study. The following data were acquired: demographic characteristics, the presence of cUTI defining factors, the results of urine culture and AMR of isolated pathogens. Results: One-hundred and fifty patients were selected for the study: 100 with cUTI and 50 with acute complicated pyelonephritis. Renal stones were the most common cUTI defining factor in women (40.2%), while the benign prostatic hyperplasia was the most common factor in men (73.5%). A total of 15 different species of bacterium were isolated form the urine samples. Three most common pathogens were E. coli (61.3%), E. faecalis (18.7%) and K. pneumoniae (12.0%). The highest resistance was observed to ampicillin (54.4%), trimethoprim/sulfamethoxazole (33.5%), ciprofloxacin (29.5%) and to norfloxacin (29.1%). The resistance to the antimicrobial drugs, which are recommended by the guidelines were: amikacin (0.8%), gentamicin (16.6%), cefotaxime (12.4%), ceftazidime (13.3%) and piperacillin/tazobactam (5.5%).
