Infantilas rīšanas paradumi un citas orofaciālā kompleksa atradnes pirmsskolas vecuma bērniem Latvijā

Ievads: Pētījuma mērķis bija izpētīt saikni starp infantilas rīšanas paradumiem ar citām orofaciālā kompleksa atradnēm pirmsskolas vecuma bērniem Latvijā. Paralēli tam mēs pētījām arī oklūziju. Metodes: Šajā šķērsgriezuma pētījumā piedalījās 141 pirmsskolas vecuma bērnu no 4 līdz 7 gadiem un viņu mātes. Orofaciālā kompleksa izmeklēšanu veica divi ortodonti, kurā tika novērtēta orofaciālā kompleksa miofunkcionālā situācija, oklūzija un bērnu runas defekti. Mātēm tika sniegtas anketas, lai iegūtu informāciju par bērnu veselības stāvokli un citiem identifikācijas datiem. Tika veikta aprakstoša statistika visiem pētījuma mainīgajiem lielumiem un vienvirziena analīze starp infantilo rīšanu un visiem pētījuma mainīgajiem. Visi dati tika analizēti, izmantojot SPSS programmatūru (versija 22) (IBM Corp., 2013). Rezultāti: Vienfaktoru analīzē mēs novērojām statistiski nozīmīgu saistību starp infantilo rīšanu un oklūziju (p = 0,03), elpošanu (p <0,01), runu (p <0,01), starpzobu runu (p <0,01) un vidusauss iekaisumu (p = 0,02). Vienfaktoru analīzē 0,1 nozīmīguma līmenī citus mainīgos lielumus, kas saistīti ar infantilo rīšanu, nekonstatēja. Izmantojot pielāgotu daudzfaktoru regresijas metodi tika novērots, ka mutes elpošana, runas defekti un vidusauss iekaisums statistiski būtiski palielina infantilās rīšanas iespēju. Netika novērota statistiski nozīmīga korelācija starp oklūziju un elpošanu (pinter = 0,27) un starp oklūziju un runu (pinter = 0,19) saistībā ar infantilo rīšanu. Secinājumi: Mutes elpošanas, runas traucējumu un vidusauss iekaisuma nozīme tika pierādīts korelācijā ar infantilo rīšanu, kas nozīmē, ka esošie diagnosticētie orofaciālā kompleksa izmeklējumu dati var būt kaitīgo ieradumu, piemēram, infantilās rīšanas etioloģiskie faktori Atslēgas vārdi: mēles izgrūšana, infantila rīšana, sliktie ieradumi, orofacialā kompleksa izmeklējumi, pirmsskolas bērnu šķērsgriezuma pētījums, ortodontija Introduction: The aim of this study was to investigate the association between tongue thrust habits with other orofacial findings in a sample of preschool children in Latvia. Subsequently we investigated role of occlusion in this association. Methods: In the present cross-sectional study 141 preschool children with an age range from 4 to 7 years and their mothers participated. Physical investigation was performed by two orthodontists where myofunctional situation, occlusion, and speech defects of children were evaluated. Questionnaires were given to mothers to fulfill information about health-related conditions and other identifying patters about their children. Descriptive statistics for all study variables and univariate analysis between tongue thrust habits and all study variables were perfomed. All data was analyzed using SPSS software (version 22) (IBM Corp., 2013). Results: In the univariate analysis we observed statistically significant relationships between tongue thrust habit and occlusion (p = 0.03), breathing (p < 0.01), speech (p < 0.01), lisping (p < 0.01), and otitis media (p = 0.02). No other variables had not been found related to tongue thrust habit in the univariate analysis on the 0.1 significance level. It was observed that mouth breathing, non-normal speech, and otitis media statistically significantly increase the possibility of infantile tongue thrust in fully adjusted multiple logistic regression. Statistically significant interaction between occlusion and breathing (pinter = 0.27), and between occlusion speech (pinter = 0.19) on their association with thrust habits was not observed. Conclusions: The significance of mouth breathing, speech disturbancies and otitis media was found in the association with tongue thrust habit, which means that existing history of diagnosed listed orofacial health- related findings can be precursors for deleterious habits, such as tongue thrust.
