Bakalaura darba tēma ir „Centrālo venozo katetru potenciālās aprūpes problēmas un risinājumi intensīvās terapijas nodaļā”. Tēmas aktualitāti nosaka tas, ka centrālā venozā katetra izmantošana nav viennozīmīgi droša procedūra, tā saistās ar vairākiem potenciāliem riskiem un komplikācijām. Darba mērķis ir noskaidrot un aprakstīt centrālo venozo katetru potenciālās aprūpes problēmas un risinājumus intensīvās terapijas nodaļā. Tika izvirzīti divi pētījuma jautājumi: „Kādas ir centrālo venozo katetru potenciālās aprūpes problēmas?” un „Kādi ir centrālo venozo katetru potenciālo aprūpes problēmu risinājumi?”. Bakalaura darbā tiek pielietota kvalitatīvā pētniecības metode. Pētījuma instrumenti- novērošanas protokols un intervija. Novērošanas protokola izstrāde balstīta uz 2011.gada CDC vadlīnijām ar intravaskulāro katetru saistīto infekcijas profilaksi. Pētījuma uzdevumi bija zinātniskās literatūras un Beatrises Keilišas nepilnīgi veiktas pacientu aprūpes modeļa analīze; izstrādāt novērošanas protokolu un veikt centrālā venozā katetra aprūpes procedūras novērtējumu; izstrādāt intervijas jautājumus; apkopot un analizēt pētījuma rezultātus; izdarīt secinājumus un izstrādāt rekomendācijas. Pētījuma rezultātā tiek sasniegts darba mērķis un sniegtas atbildes uz pētījuma jautājumiem. Centrālo venozo katetru potenciālās aprūpes problēmas var attīstīties katetra ievades, izmantošanas un aprūpes laikā. Biežākās potenciālās aprūpes problēmas ir katetra oklūzija un tromboze, infekcijas risks, hemorāģija un hematoma. Potenciālo aprūpes problēmu risinājumi ir roku higiēna, aseptikas ievērošana, sterilu barjeru lietošana, punkcijas vietas kopšana, katetra skalošana, piekļuves ostu dezinfekcija, savienotājsistēmu maiņa, katetra vizuālais novērtējums un dokumentācija. The theme of the bachelor thesis is “Central venous catheters potential care problems and solutions in intensive care unit”. The topicality of the theme is determined by the fact that the use of central venous catheter is not clearly safe procedure and involves a number of potential risks and complications. The aim of the bachelor thesis is identify and describe potential care problems and solutions for central venous catheters in intensive care unit. Two questions were raised: “What are the potential care problems for central venous catheters?” and “What are potential care problems solutions for central venous catheters?” A qualitative research method is used in the bachelor`s thesis. Research tools are- observation protocol and interview. Observation protocol were based on 2011 CDC guidelines for the prevention of intravascular catheter- related infections. The research tasks were analysis of the scientific literature and Beatrice Kalisch missed nursing care model; develop an observation protocol and evaluate the central venous catheter care procedures; develop interview issues; aggregate and analysis results of the study; draw conclusions and make recommendations. The study results in achieving the aim of the bachelor thesis and providing answers to the questions of the study. Potential care problems for central venous catheters may develop during catheter insertion, use and care. The most common potential care problems are catheter occlusion and thrombosis, risk of infection, hemorrhage and hematoma. Solutions for potential care problems include hand hygiene, aseptic observe, use of sterile barriers, site care, catheter flushing, device disinfection, administration sets change, catheter visual assessment and documentation.
