Gūžas un ceļa locītavu endoprotezēšanas operāciju izvērtējums (2009-2018) Traumatoloģijas un Ortopēdijas slimnīcā

Abstract

Mana diplomdarba mērķis ir kvantitatīvi novērtēt gūžas locītavu cementējamā, bezcementa, hibrīda un cervikokapitā tipa endoprotezēšanas operāciju skaitu, ceļa locītavu endoprotezēšanas operāciju, revīzijas operāciju skaitu, kā arī infekciju skarto pacientu skaitu, kas ir kā iemesls revīzijas operācijai un papildus pie katra atbilstošā endoprotēzes tipa kvantitatīvi atspoguļot vidējo pacienta ārstēšanās ilgumu slimnīcā. Pētījuma dati ņemti no TOS arhīva un atspoguļo endoprotezēšanas operāciju skaitu 10 gadu griezumā (2009. – 2018.gadam). Rezultātos kopumā 10 gadu griezumā TOS tika veiktas 18277 gūžas un ceļa locītavu endoprotezēšanas operācijas, ieskaitot revīziju operācijas. Primāro gūžas un ceļa locītavu operāciju skaits no 2009. – 2018.gadam ir palielinājies no 1633 līdz 1889 operācijām 2018.gadā. Šāda veida tendence vērojama arī citviet pasaulē, tajā skaita Zviedrijā. Raugoties uz vidējo ārstēšanās ilgumu stacionārā 10 gadu griezumā vērojama pozitīva tendence - vidējais pacienta uzturēšanās laiks stacionārā ir samazinājies no vidēji 12.55 dienām 2009.gadā, līdz vidēji 8,37 dienām 2018.gadā (skaitot visu tipu primārās artroplastiju operācijas). Populārākais endoprotēžu veids ir cementējāmā tipa endoprotēzes (845 operācijas 2013.gadā, 619 operācijas 2018.gadā), bet velarvien tieši cementējamo implantu operāciju skaits ir 3 – 4 reizes lielāks, nekā bezcementa un hibrīda tipu implantu operāciju skaits. Vidēji gadā tiek veiktas 180 bezcementa tipa implantu operācijas. Visnepopulārākā ir hibrīda tipa gūžas locītavas endoprotēze, vidēji gadā tiek veiktas 188 šāda veida operācijas. Gūžas locītavas cervikokapitālo implantu skaits laika gaitā dinamikā ir svārstīgs, ar tendenci samazināties, vidēji gadā tiek veiktas 107 šāda veida operācijas. Pie bezcementa tipa implantiem pacientu vidējais ārstēšanās ilgums slimnīcā laika gaitā ir samazinājies no vidēji 10,28 dienām 2009.gadā, līdz vidēji 6,79 dienām 2018.gadā. Cementējamā tipa implantiem tās vidēji ir 10,2 dienas. Ārstēšanās ilgums stacionārā pēc hibrīda tipa endoprotezēšanas operācijām ir samazinājies (vidēji tās ir 9,8 dienas), bet ne tik izteikti, kā pie cementa vai bezcementa tipa endoprotezēšanas operācijām. Savukārt cervikokapitālā tipa implantu pacientu vidējais ārstēšanās ilgums stacionārā pagājušajā gadā bija 11,97 dienas. Vidēji gadā nākas veikt 580 ceļa locītavu operācijas un statistiski operāciju skaitam ir tendence palielināties (698 operācijas 2018.gadā). Vidējais pacientu uzturēšanas ilgums stacionārā samazinās (10 gadu griezumā vidēji tās ir 11 dienas). TOS revīzijas operāciju skaits ir pieaudzis ar krasu kāpumu laika posmā no 2014. – 2018.gadam (no 169 – 396 operācijām). To var saistīt ar komplikācijām, no kurām visbiežākā ir infekcija. Kopumā revīzijas infekcijas dēļ sastāda 32% no kopējā endoprotezēšanas operāciju skaita. Vidējais pacientu ārstēšanās ilgums stacionārā ir ilgs (vidēji 18 dienas), bet ir vērojama tendence samazināties.

The aim of the diploma is to quantify hip cementation, cement-free, hybrid and cervicocapital-type endoprosthetic operations, knee endoprosthetic operations, revision operations, the number of patients affected by the infection, as well as to quantify the average patient hospitalization for each type of endoprosthesis. The research data is taken from the TOS archive and illustrates the number of endoprosthetic operations performed over the period of 10 years (2009-2018). Overall, in the last 10 years in TOS, there were 18277 hip and knee replacement prosthetic operations performed, including revision operations. Between 2009 and 2018, the number of primary hip and knee joint operations performed has increased from 1633 to 1889 operations in 2018. If we look at the average duration of treatment in hospital in the 10 year period, we can see a positive trend that shows that the average patient's stay in hospital has decreased from an average of 12.55 days in 2009 to an average of 8.37 days in 2018 (taking into account all types of primary arthroplasty operations). The most popular type of endoprosthesis is clearly a cementable type of endoprosthesis (845 operations performed 2013 vs 619 operations in 2018), but still exactly the number of cementable implant operations is 3-4 times higher than the number of non-cement and hybrid implant operations. On average, there are 180 cementless implant operations per year. As the most uncommon operations, I can name a hybrid type implant, as, on average, there are 188 such operations per year performed. The number of cervicocapital implants of the hip joint has been fluctuating over time, with a tendency to decrease and with an average of 107 such operations per year. In the case of cementless implants, patient duration in the hospital over time has decreased from an average of 10.28 days in 2009 to an average of 6.79 days in 2018. For cementable type implants, patients spend average 10.2 days in hospital. The duration of treatment in hospital after hybrid type endoprosthetic surgery has decreased (to 9.8 days on average), but it is not as pronounced as for cement or cementless endoprotection operations. Meanwhile, the average duration of treatment in a hospital for a cervicocapital type implants was 11.97 days in the last year. On average, there are 580 knee joint operations performed per year, and the number of operations has an increasing tendency. The average number of days in the hospital has decreased (to an average of 11 days). The number of TOS revision operations has sharply increased between 2014 and 2018 (from 169 to 396 operations, reaching 17% of the total number of all endoprosthetic operations). It can be linked to an infection as the most common medical complication. Overall, 32% of the revision surgery is necccesary due to infection. The average duration of patient treatment in a hospital is rather long (18 days on average), but there is a tendency to decrease.
