Staphyloccocus epidermidis virulences faktoru un antibakteriālās rezistences noteikšana

Darba aktualitāte: Neraugoties uz to, ka diagnosticēšanas un ārstniecības iespējas strauji attīstās, vēl joprojām aktuāla ir tēma par baktēriju rezistenci. S. epidermidis tiek uzskatīts par vienu no biežākajiem nazokomiālo infekciju ierosinātājiem. Šīs baktērijas svarīgākais virulences faktors ir biofilmas veidošanās. Ņemot vērā baktēriju īpašības, S. epidermidis izraisītās infekcijas kļūst grūti ārstējamas. Darba mērķis: Noteikt medicīnas studentu ādas mikroflorā (mikrobiota) esošos stafilokokus un identificēt S. epidermidis baktērijas, noteikt to virulences, antibakteriālo rezistenci un salīdzināt ar to stafilokoku celmu īpašībām, kas iegūti no klīniskā materiāla. Metodes un materiāli: Diplomdarba pētnieciskajā daļā tika apkopota informācija par S. epidermidis virulences faktoriem un antibakteriālo rezistenci divām pētījuma grupām (veseli cilvēki un pacienti). Iegūtais materiāls tika kultivēts, mikroorganismi no barotnēm tika izolēti, izmantojot BBL™ Crystal™ identifikācijas sistēmu. Izmantojot diska difūzijas testu, tika noteikta antibakteriālā jutība. Lai noteiktu biofilmas veidošanos tika izmantota mikrotitru plates metode. Rezultāti un secinājumi: 1.Hemolītisko aktivitāti uzrādija (n=9), kas iegūti no veselu cilvēku (studentu) identificētajām baktērijām, un (n=18) no pacientu klīniskajiem materiāliem. 2.Biofilmas klātesamība raksturīga (n=13) no analizētajiem materiāliem, kas iegūti no pacientu klīniskajiem izolātiem. Veselu cilvēku grupai biofilmas klātesamība novērojama (n=6). 3.Visi celmi (n=48) ir jutīgi pret vankomicīnu un rezistenti pret ampicilīnu. 4.Rezistence pret cefoksitīnu raksturīga izolātiem, kas iegūti no pacientu klīniskā materiāla. Tie veido (n=23) un (n=9) rezistenci uzrāda veselu cilvēku izolāti. 5.Makrolīdu grupas antibakteriālais līdzeklis eritromicīns uzrāda līdzīgus rezultātus otrās paaudzes cefalosporīnam – cefoksitīnam. 6.Pastāv statistiski ticama asociācija, ka S. epidermidis rezistence pret cefoksitīnu un eritromicīnu raksturīga studentiem, kas strādājuši ITN (p=0,03). The importance of the work: Despite the fact that diagnosis and treatment options are rapidly evolving, the topic of resistant bacteria is still relevant. S. epidermidis is considered to be one of the most frequent agents of nosocomial infections. The most dangerous virulence factor for this bacteria is biofilm formation. Biofilm is a community of microorganisms that attach to implanted surfaces and based of its qualities, S. epidermidis becomes very difficult to treat. The purpose of the research: To determine the microflora composition of the skin of medical students and identify S. epidermidis bacteria, determine their virulence factors, antibacterial resistance and compare with the properties of their staphylococcal strains derived from clinical material. Methods and materials: The research part collected information on virulence factors and antibacterial resistance of S. epidermidis in two study groups (healthy people and patients). The resulting material was identified and the microorganisms were isolated from the medium using the BBL ™ Crystal ™ identification system. Anti-bacterial sensitivity is determined using the disk diffusion test. I used the microtiter plate method to determine biofilm formation. Results and conclusions: 1. Haemolytic activity was demonstrated in (n = 9) of bacteria identified in healthy subjects (students) and in (n = 18) of patients clinical material. 2. The presence of biofilm is more characteristic for the analysed material obtained from clinical isolates, which is (n = 13) than for the control group, which is (n = 6). 3. All strains (n = 48) are vancomycin sensitive and ampicillin resistant. 4. In (n = 23) cefoxitin resistance is specific to isolates obtained from patient clinical material and in (n = 9) healthy human isolates are showing resistance. 5. The macrolide antibacterial agent erythromycin shows similar results to second generation cephalosporin - cefoxitin. 6. There is ar statistically significant association than S epidermidis rezistance to cefoxitin and erythromycin is characteristic of ITN students (p=0,03).
