Opioīdu biežāk izraisītās blakusparādības onkoloģiskiem pacientiem paliatīvās aprūpes etapā

Ievads. Sāpes ir ļoti izplatītas onkoloģisko pacientu populācijā. Onkoloģiskie pacienti var ciest sāpes jebkurā stadijā un ārstēšanas posmā. Šīs sāpes var būt dažāda rakstura un intensitātes. Ir vairāki fizioloģiski mehānismi, ar kuru palīdzību vēzis izraisa sāpes. Statistikie dati liecina, ka sāpes izjūt no 30 līdz 50% pacientu, kas saņem aktīvu pretvēža ārstēšanu, un no 75 līdz 90% pacientu, kuriem ir progresējoša, metastātiska vai termināla slimība. Izteiktas līdz neciešamas sāpes biežāk izjūt pacienti III-IV vēža stadijās. Šajos gadījumos, pēc PVO noteiktām vadlīnijām, terapijā ir jāpievieno opioīdi. Diemžēl, lietojot opioīdus, pacientiem diezgan bieži ir novērojami nevēlami blakusefekti. Tas var sagādāt pacientam diskomfortu un pat ciešanas, un neskatoties uz labu atsāpinošo efektu, pieņemama dzīves kvalitāte nebūs sasniegta. Tas ir ļoti svarīgi, īpaši pacientiem, kas saņem paliatīvo aprūpi. Materiāli un metodes. Prospektīvs pētījums, kas tika veikts stacionārā “Biķernieki”, paliatīvās aprūpes nodaļā. Pētījumā tika iekļauti 52 pacienti, vecumā no 36 līdz 82 gadiem, kuri saņēma ārstēšanu stacionārā “Biķernieki”, paliatīvās aprūpes nodaļā, laika posmā no 2020.gada janvāra līdz 2020.gada martam. Datu iegūšanai tika izmantota anketēšanas metode. Anketā tika iekļauti jautājumi par respondenta dzimumu, vecumu, diagnozi, blakusslimībām, saņemto terapiju un blakusefektu esamību. Rezultāti. Saņēmot opioīdu terāpiju 69% respondentu novēroja vienu vai vairākus blakusefektus. 31% respondentu blakusefektus nenovēroja. Tramadola grupā visbiežāk novērota blakne ir vājums – 61% (n=11), aizcietējumi 44% (n=8), miegainība 33% (n=6), slikta dūša 27% (n=5), paaugstināta svīšana 16% (n=3), urīna aizture un miega traucējumi 11% (n=1). Fentanīla grupā vājums 37% (n=11), miegainība 29% (n=8), aizcietējumi un slikta dūša 11% (n=3), paaugstināta svīšana 7% (n=2), miega traucējumi 3% (n=1). Nieze nebija novērota nevienam respondentam, kas saņēma fentanīlu. Morfīna grupā vājums 71% (n=5), miegainība 57% (n=4), slikta dūša 42% (n=3), nieze 28% (n=2), miega traucējumi, aizcietējumi un urīna aizture 14% (n=1). Paaugstināta svīšana nebija novērota nevienam respondentam. Salīdzinot šos medikamentus un izraisītus blakusefektus, statistiski nozīmīga atšķirība ir aizcietējumu grupā (p=0,029) un niezes grupā (p=0,015). Pārējās grupās nav atrasta statistiski nozīmīga atšķirība (p>0,05). Secinājumi. Opioīdu izraisītus blakusefektus novēroja 69% respondentu, kas nozīmē, ka problēma joprojām ir aktuāla.Visbiežākie opioīdu terapijas blakusefekti ir vājums, miegainība, aizcietējumi un slikta dūša. Retāk novērotie blakusefekti ir paaugstināta svīšana, miega traucējumi, nieze un urīna aizture.Salīdzinot konkrētus medikamentus, transdermālām fentanīlam ir laba panesamība un zems blakusefektu veidošanas risks. Savukārt perorālam tramadolam ir lielāks aizcietējumu riks, bet intravenozajam morfīnam lielāks niezes risks. Pain is very common among oncological patient population. Oncological patients may experience pain at any stage of illness and any stage of treatment. Experienced pain can be of different nature and intensity. There are a number of physiological mechanisms by which cancer causes pain. Statistical data show that between 30 and 50% of patients receiving active anticancer treatment and between 75 and 90% of patients with advanced, metastatic or terminal disease are experiencing pain. Severe to intolerable pain is felt more frequently in patients at the stages of III-IV cancer. In these cases, opioids should be added to treatment according to WHO guidelines. Unfortunately, opioids tend to have side effects in patients quite frequently. This can cause patient discomfort and even suffering, and despite a good pain relieving effect, acceptable quality of life will not be achieved. Materials and methods. A prospective study conducted in the “Biķernieki” hospital’s palliative care ward. The study included 52 patients, within age variations from 36 to 82, who received treatment in the “Biķernieki” hospital’s palliative care ward, between January 2020 and March 2020. To obtain the data questionnaire method was used. The questionnaire included questions concerning the respondent's sex, age, diagnosis, compositional conditions, received treatment and the existence of side effects. Results. 69% of respondents experienced one or more side effects when administering opioid therapy. 31% of respondents did not experience any side effects. In the group of patients using tramadol, the most common side effects are weakness – 61% (n = 11), constipation 44% (n = 8), somnolence 33% (n = 6), nausea 27% (n = 5), increased sweating 16% (n = 3), urinary retention and sleep disorders 11% (n = 1). In the group of patients using fentanil, weakness 37% (n = 11), somnolence 29% (n = 8), constipation and nausea 11% (n = 3), sweating increased 7% (n = 2), sleep disorder 3% (n = 1). Itching was not observed in any of the respondents who received fentanyl. In the group of patients using morphine, weakness 71% (n = 5), somnolence 57% (n = 4), nausea 42% (n = 3), itching 28% (n = 2), sleep disorder, constipation and urinary retention 14% (n = 1). No respondents experienced increased sweating. When comparing these medicines and associated side effects, a statistically significant difference is between the constipation group (p = 0.029) and the itching group (p = 0.015). No statistically significant difference was found in other groups (p > 0,05). Conclusions. Opioid-related side effects were observed in 69% of respondents, which means that the problem is still relevant. The most common side effects of opioid treatment are weakness, drowsiness, constipation and nausea. Uncommon side effects include increased sweating, sleep disturbances, itching and urinary retention. Compared to certain medicines, transdermal transdermal fentanyl is well tolerated and has a low risk of side effect
