Alkohola atkarības farmakoloģiskās ārstēšanas rezultātu analīze ambulatorajā praksē

Diplomdarba tēma: Alkohola atkarības farmakoloģiskās ārstēšanas rezultātu analīze ambulatorajā praksē. Darba mērķis. Analizēt ambulatorajā praksē pielietotās alkohola atkarības farmakoloģiskās ārstēšanas metodes un izvērtēt ārstēšanas efektivitāti. Darba hipotēze. Ambulatorajā praksē pielietotās farmakoloģiskās ārstēšanas metodes atbilst vadlīnijām un ir efektīvas alkohola atkarības pacientu ārstēšanas procesā. Materiāli un metodes. Diplomdarba teorētiskajā daļā apkopota zinātniskā literatūra un pētījumi par alkohola atkarības ārstēšanas procesā pielietotiem medikamentiem un ārstēšanas vadlīnijām. Diplomdarba pētījumā gaitā tika pētītas ambulatorās medicīniskās kartes pacientiem ar alkohola atkarības diagnozi. Pētot ambulatorās kartes, tika analizēti pacientu dati: dzimums, vecums, narkologa apmeklējumu regularitāte, narkotisku vai psihoaktīvu vielu lietošana anamnēzē, piedalīšanās Minesotas ārstēšanās programmā, nozīmētā terapija un ārstēšanas procesa iznākumi, iegūtās remisijas ilgums dažādās pacientu grupās. Rezultāti. Pētījumā iekļautas 102 pacientu ambulatoro medicīnisko karšu dati. Pētījuma rezultāti liecina, ka lielāka daļa no alkohola atkarības pacientiem ir vīrieši (73% no kopēja pacientu skaita), 37% no pētāmas grupas ir pacienti vecumā no 36 līdz 45 gadiem, 57% no pacientu skaita apmeklēja narkologu atkārtoti. Veicot iegūto datu analīzi, noskaidrots, ka alkohola atkarības ārstēšanas procesā, narkologi visbiežāk pielieto komplementāro metodi (pielietota 44% no pacientu skaita), kas ietver sevī atbalsta psihoterapiju, suģestīvo terapiju un sensibilizējošus medikamentus. Kā otrā visbiežāk izmantotā alkohola atkarības ārstēšanas metode ir medikamenta disulfirāma lietošana (pielietots 32% no pacientu skaita). Komplementārās metodes un disulfirāma kombinācija ir trešā visbiežāk izmantotā alkohola atkarības ārstēšanas metode (pielietota 14% no pacientu skaita). Veicot iegūto datu analīzi, noskaidrots, ka lielākai pacientu daļai, 31% no pacientu skaita, novērots remisijas ilgums no viena gada līdz diviem gadiem. Remisiju no sešiem mēnešiem līdz vienam gadam konstatēja 23% no pacientu skaita. Remisiju no diviem līdz trijiem gadiem konstatēja 22% no visiem pacientiem. Remisija līdz sešiem mēnešiem bija novērota 12% no pacientiem un remisijas periodu trīs gadi un vairāk novēroja 12% no pacientu kopēja skaita. Analizējot datus, tika secināts, ka regulāri uz pieņemšanu nozīmētājā laikā ieradās tikai 47% no pacientu skaita. Secinājumi. Iegūtie rezultāti liecina, ka ambulatorajā praksē, alkohola atkarības ārstēšanai, visbiežāk pielieto komplementārās terapijas metodi, nozīmē disulfirāmu un disulfirāmu kombinācijā ar komplementārās terapijas metodi. Naltreksonu, nalmefēnu un akamprosātu uzturošajā terapijā nozīmē reti. Pēc pielietotās farmakoloģiskās terapijas lielāka daļa no pacientiem sasniedz remisiju vidēji no viena līdz diviem gadiem, neliela pacientu daļa pēc ārstēšanas sasniedz remisiju ilgāku par trīs gadiem. Title: An analysis of the pharmacological treatment of alcohol addiction in outpatient practice. The aim of the study. To analyze the methods of pharmacological therapy used in outpatient practice and evaluate the effectiveness of the treatment of alcohol addiction. Hypothesis. Pharmacological therapy used in outpatient practice corresponds to the guidelines for the treatment of alcohol addiction and is effective. Material and Methods. The theoretical part of the thesis summarizes the scientific literature and research on the medicines and methods used to treat alcohol addiction. Outpatient unit medical records of patients with a diagnosis of alcohol addiction were studied in accordance with a research aim. Examining the records on outpatient unit patients medical records, the following were analyzed: gender of patients, age, regularity of narcologist’s (addiction psychiatrist) visiting, drug and psychotropic substances use in the patient’s history, participation in the Minnesota treatment programme, prescribed pharmacotherapy, duration of the remission period in different age groups. Results. The study included 102 outpatient unit medical records. The results of the study show that the majority of patients with alcohol addiction are men (73% of the total number of patients), 37% of the studied group of patients were in the age category from 36 to 45 years old, 57% of patients visited the narcologist again. Analyzing the data obtained, it was found that the most often used treatment method is the complementary treatment, which includes supportive therapy, suggestive therapy and sensitizing medications (prescribed to 44% of patients). The second most often used treatment method is the use of disulfiram (prescribed to 32% of patients). The combination of disulfiram and the complementary method was used to treat 14% of patients. After analyzing the obtained data, it was found that after the treatment most of the patients (31%) were able to achieve one year to two years long remission. 23% of patients managed to achieve six months to one year long remission. Two to three years long remission was observed in 22% of patients. Remission up to six months was observed in 12% of patients, and remission longer than three years was achieved in 12% of cases. Analyzing the data, it was concluded that only 47% of the number of patients regularly visit a narcologist. Conclusions. The findings of the study indicate that the most often used methods for the treatment of alcohol addiction are the complementary method, disulfiram prescription, and a combination of the complementary method with the use of disulfiram. Naltrexone, Nalmefene, Acamprosat are rarely used in the main therapy. After pharmacological treatment, patients achieve remission on average from one year to two years. After treatment, only a small proportion of patients reaches remission of three years or longer.
