Artroskopiskajai ķirurģijai paredzētu dažādu koblācijas ierīču un elektrodu iedarbes histoloģiskā audu izvērtēšana

Koblācija savu popularitāti iemantojusi ar vairākām priekšrocībām, salīdzinot to ar konvencionālo elektroķirurģiju, bet literatūra, kas apraksta tās ietekmi uz brūces parametriem ir ļoti limitēta. Diplomdarbs tika izstrādāts ar mērķi noteikt galvenās un nozīmīgākās atšķirības starp koblācijas veidotas brūces parametriem, izmantojot dažādas jaudas, elektrodus un enerģijas ģeneratorus. Brūces tika veidotas trīs atšķirīgos audos – nierē, mēlē un ādā. Pētījuma praktiskā daļa tika veikta sadarbībā ar MedTecResearch centru Vācijā un Rīgas Austrumu klīniskās universitātes slimnīcas Patoloģijas centru. Darba veikšanai tika izmantoti divi vaporizācijas, divi āķveida elektrodi un trīs atšķirīgi enerģijas ģeneratori. Katrs no ģeneratoriem tika izmantots trijos jaudas iestatījumos – “2”, kas atbilst 60W, “5”, kas atbilst 170W un “9”, kas atbilst 390w. MedTecResearch centrā izveidotā speciālā iekārta SIMCUT IIIs, nodrošināja iespēju iestatīt ātrumu, ar kādu elektrods tiek vilkts pār audiem, lai nodrošinātu standartizētus griezienus. Pēc griezienu veikšanas, audu paraugi tika nogādāti Gustrovas Patoloģijas centrā Vācijā, kur tos apstrādāja ar parafīnu un krāsoja ar Hematoksilīnu-eozīnu un ar Periodic acid Schiff. Paraugu mikroskopiskā analīze tika veikta pēc Ruidiaz et al zinātniskā pētījuma parauga. Autors savā darbā izveidojis shēmu pēc kuras izšķir termālā bojājuma četras zonas. Analizējot pētījuma rezultātus tika secināts, ka gan āķveida, gan vaporizācijas elektrodu griezienu kontūras nieres audos un ādā ir atkarīgas no elektroda formas. Nieres un mēles audos ar vaporizācijas un āķveida elektrodiem veidotu brūču dziļums ir atkarīgs no pieliktās jaudas. Ādā tikai ar āķveida elektrodiem veidotas brūces dziļums ir atkarīgs no pieliktās jaudas. Statistiski nozīmīgas atšķirības brūču dziļumos un to malu attālumos starp izmantotajiem viena veida elektrodiem netiek novērotas. Nieres un mēles audos ar visiem elektrodiem un ādā ar vaporizācijas elektrodiem visplašāko nekrozes zonu veidoja Werewolf ģenerators augstas jaudas iestatījumā. Ādā ar abu veidu elektrodiem un nieres audos ar āķveida elektrodiem nekrozes zona pieauga pakāpeniski pieliktajai jaudai. Nieres audos abu veidu elektrodiem laterālā bojājuma zona pieauga atbilstoši pieliktajai jaudai. Mēles audu dažādos slāņos laterālā bojājuma zona veidojās atšķirīgā plašumā. Vislielākā tā ir saistaudos un muskuļos, bet vismazākā daudzkārtainajā plakanajā pārragotajā un parakeratinizētajā epitēlijā. Coblation has many advantages in comparison to conventional electrosurgery, and in recent years, its popularity has only grown. Unfortunately, information about the effects of coblation on wound parameters is very limited. The aim of this work was to determine the main and most important differences in wound parameters using electrodes and generators of different manufacturers, and compare the results using three different power settings. Wounds were made in kidney, tongue, and skin tissue. The research was made in collaboration with MedTecResearch centre in Germany and Pathology Centre of Riga East University Hospital. For this work, two vaporisation and two hook electrodes were used in combination with three generators in three power settings – 60W, 170W and 390W. Special device SIMCUT IIIs was made in MedTecResearch centre to provide standardised cuts. After coblation, tissue specimen were sent to Gustrows Pathology centre, where they were prepared with paraffin and Haematoxylin-eosin and Periodic acid Schiff stain for microscpical work. Microscopical analysis was made after Ruidiaz et al. research sample. In his work, the author developed a special scheme that was used to determine four thermal damage zones. In this research, results were analysed, concluding the following – both hook and vaporisation type electrodes made wound contours depending on the design of the electrode. In kidney and tongue tissue with vaporisation and hook electrodes, wounds depth depends on the generator power applied. In skin, only with hook electrodes this statement was true. There were no statistically significant differences between the same kind vaporisation and hook electrodes regarding the wound depth and width results. In kidney and tongue tissue with all electrodes and in skin with both vaporisation electrodes the largest necrotic zone was made with Werewolf generator in high power settings. In skin with all electrodes and in kidney with hook electrodes, the necrotic zone increased accordingly to added power. In kidney with all electrodes lateral damage zone increased accordingly to added power. Lateral damage did not spread evenly in all of the tongue tissue layers. It was the largest in muscle and connective tissues, and the smallest in epithelium.
