Meticilīna rezistences noteikšana Staphylococcus epidermidis (MRSE) starp veseliem iedzīvotājiem

Ievads. Pēdējos gados ir manāmi pieaugusi Staphylococcus epidermidis un S. aureus izraisītās intrahospitālās infekcijas. Kā parāda ASV statistikas dati, tad laika posmā no 1980. Gadam līdz 1990. Gadam, tad S. epidermidis izraisītās nozokomiālās infekcijas ir pieaugušas četras reizes (Banerjee et al.). Literatūra vēsta arī to, ka S. epidermidis ir atbildīts par lielu daļu nozokomiālo infekciju, kas izraisa bakterēmijā, kas saistītas ar svešķermeņu un dažādu medicīnas iekārtu ievietošanu cilvēka ķermenī (Huebner, 1999). Kā arī koagulāzes negatīvie stafilokoki ir bieži endokardīta izraisītāji. (Caputo, 1987). Pētījuma mērķis. Noskaidrot meticilīna rezistento S. epidermidis (MRSE) baktēriju nēsāšanas īpatsvaru populācijā starp veseliem cilvēkiem un noskaidrot dažādu blakus faktoru ietekmi uz iespēju iegūt MRSE celmu. Pētījuma uzdevumi. Pētījuma ietvaros bija nepieciešams ievākt paraugu no dalībnieka deguna dobuma, lai turpmāk no iegūtajām kultūrām varētu veikt tīrkultūras iegūšanu ar S. epidermidis un pārbaudīt to ar dažādām analīzes metodēm. Rezultāti: 1. Pēc primāra uzsējuma 24 stundu inkubācijas, stafilokoki tika izolēti 100,0% ± 2,9% gadījumu (n=112) dalībniekiem. KONS tika konstatēts 50,9% ± 2,19% (n=57) gadījumu, un S. aureus 17,9% ± 1,73% (n=20), bet KONS un S. aureus kopā 31,3% ± 1,32% (n=35) gadījumos. 2. Kopā no 92 paraugiem, rezistenti pret oksacilīnu bija 11,96% ± 1,0% (n= 11, SD = 3,31). Bet 88,04% ± 2,61% (n=81, SD=23,53) dalībnieku uzsējumu bija jutīgi pret oksacilīnu. 3. 10,87% (n=10) no visiem dalībniekiem (n=92) un 17,24% (n=10) no visiem klīnikā strādājošajiem dalībniekiem (n=58) ir rezistence pret oksacilīnu. 4. Pastāv statistiski ticama asociācija, ka S. epidermidis rezistence pret oksacilīnu novērojama biežāk tiem studentiem, kas strādājuši klīnikā. (p=0,033, OR=6,88). In recent years there have been observed increase in S. epidermidis and S. aureus caused intrahospital infection. As statistic data from USA shows us in timespan from 1980’s to 1990’s, S. epidermidis induced nosocomial infections has increased four times (Banejee er al.). Literature foreshows that S. epidermidis has been responsible for large part of nosocomial infection which brings bacteraemia, which has been constrained to foreign bodies and other medicine equipment placement in human body. (Huebner, 1999). Coagulase-negative staphylococci are also common causes of endocarditis. (Caputo,1987) Objective of the study. To determine the proportion of carriers of resistant S. epidermidis bacteria (MRSE) in the population among healthy people and to investigate the effect of various concomitant factors on the possibility of obtaining a methicillin-resistant S. epidermidis strain. Goals of the study. Within the framework of the study, it was necessary to collect a sample from the participant's nasal cavity in order to obtain a pure culture with S. epidermidis from the obtained cultures and to test it with various analysis methods. Results: 1. After 24 hours of primary inoculation incubation, staphylococci were isolated in 100.0% ± 2.9% of cases (n = 112). CONS was detected in 50.9% ± 2.19% (n = 57) of cases, and S. aureus in 17.9% ± 1.73% (n = 20), but CONS and S. aureus combined in 31.3% ± 1.32% (n = 35) cases. 2. Out of a total of 92 samples, oxacillin resistance was 11.96% ± 1.0% (n = 11, SD = 3.31). But 88.04% ± 2.61% (n = 81, SD = 23.53) of the participants' inoculation were sensitive to oxacillin. 3. 10.87% (n = 10) of all participants (n = 92) and 17.24% (n = 10) of all participants in the clinic (n = 58) was resistant to oxacillin. 4. There is a statistically significant association that S. epidermidis resistance to oxacillin is more common in students who have worked in a clinic. (p = 0.033, OR = 6.88).
