Saikne starp stresu un izdegšanu autiskā spektra traucējumu bērnu vecākiem

Ievads. Autiskā spektra traucējumi (AST) biežāk raksturojas ar agrīnu sākumu. Galvenie raksturlielumi ir traucējumi sociālajā komunikācijā un sociālajā mijiedarbībā, ko papildina ierobežota un atkārtojoša izturēšanās. Attiecīgi šāda milzīga uzmanība, ko bērns pieprasa, varētu sekmēt vecāku izdegšanas pazīmes. Darba mērķis. Ir identificēt izdegšanas faktorus, kas ietekmē autisma spektra bērnu vecāku ikdienas dzīvi. Metodes un materiāli. Šķērsgriezuma pētījumā tika iesaistīti 76 bērnu ar autiska spektra traucējumu vecāki, kuri pilnīgi anonīmi aizpildīja aptaujas. Pielietotās aptaujas bija demogrāfiskā aptauja, Široma-Melameda izdegšanas mēra aptauja, un Stresa skala, kura tika izveidota no Perceived Stress Scale (PSS) skalas. Sociāli-demogrāfiskā aptaujā tika ievākti dati par ģimenes stāvokli, vecāku vecumu, sociālekonomisko statusu, bērnu dzimumu un vecumu. Šajā aptaujā tika ievietoti arī jautājumi par bērna ar AST saistītiem stāvokļiem - par diagnozēm, bērnu funkcionēšanas līmeni, izglītību, kā arī bērnu autisma spektrā pielietotām terapijām. Galā tiek analizēti dažādi faktori un kādā mērā vecākiem ir vai nav izdegšana. Rezultāti. No 76 iesaistītiem vecākiem visas bija sievietes. Lielākā daļa no sievietēm ar izdegšanu bija vecuma kategorijā no 30 līdz 39 g.v (19 sievietes (47,5 %)), kam sekoja sievietes vecuma kategorijā no 20 līdz 29 g.v. (sešas sievietes (66,7 %)). Ar izdegšanu strādājošas sievietes bija 18 (42,9 %) un 15 (53,6 %) sievietes ar izdegšanu nestrādāja (p = 0,26). Izdegšana neatšķīrās sievietēm, kurām bija zēns vai meitene ar autisma sindromu (p = 0,50). Netika atrastas arī statistiski nozīmīgas atšķirības vecāku izdegšanā, skatoties uz bērna funkcionēšanas līmeni: vecāki ar izdegšanu un augsti funkcionējošiem bērniem bija 13 (56,5 %), vecāki ar vidēji funkcionēšanas bērniem bija astoņi (44,4 %) un sievietes ar vāji funkcionējošiem bērniem bija deviņas (40,9 %) (p = 0,55). Kopējais stress būtiski atšķīrās mātēm ar un bez izdegšanas, un zemākā stresa vērtība bija tiem, kuri bija ar izdegšanu (p < 0.01). Daudzdimensiju analīzē kopējais stress nozīmīgi palielināja gandrīz kā uz 50% izdegšanas iespēju (OR = 1.48; 95% ticamības intervāls 1,24; 1,77), un tas ir neatkarīgi no citiem sociāliem demogrāfiskiem faktoriem. Secinājumi. Starp sociāli-demogrāfiskiem mainīgiem netika konstatēta saistība ar vecāku izdegšanu. Kā arī, izvērtējot bērna piedalīšanos dažāda veida terapijās netika atrasta saikne starp vecāku izdegšanu. Vienīgais mainīgais, kam bija tādas atšķirības, izrādījās kopējais stress, kas arī daudzfunkcionālā analīzē palielina izdegšanas iespēju pusotru reizi. Introduction. Autism spectrum disorders are more often characterized by early onset. The main characteristics are disorders of social communication and social interaction, supplemented by limited and repetitive behaviour. Accordingly, such a huge amount of attention that the child demands could contribute to the signs of parental burnout. The aim of the work. It is necessary to identify burnout factors that affect the daily lives of parents of children with autism spectrum. Methods and materials. The cross-sectional study involved parents of 76 children with autism spectrum disorders who completed the questionnaires completely anonymously. The surveys used were the demographic survey, the Shirom-Melamed burnout survey, and the Stress Scale, which was developed from the Perceived Stress Scale (PSS). The socio-demographic survey collected data on marital status, parental age, socioeconomic status, gender, age of children, as well as diagnoses, level of children's functioning, education, as well as therapies used in the autism spectrum of children. In conclusion various factors are analysed and to what extent parents have or do not have burnout. Results. Of the 76 parents involved, all were women. The majority of women with burnout were in the age group of 30 to 39 years (19 women (47,5%)), followed by women in the age group of 20 to 29 years. (six women (66,7%)). There were 18 (42,9%) women working with burnout and 15 (53,6%) women with burnout did not work (p = 0,26). Burnout did not differ between women with a boy or a girl with autism (p = 0,50). No statistically significant differences were found in parental burnout in terms of the level of child functioning: parents with burnout and highly functioning children were 13 (56.5%), parents with moderate functioning children were eight (44.4%) and women with poorly functioning children were nine (40,9%) (p = 0,55). Total stress was significantly different for mothers with and without burnout, and the lowest stress value was for those with burnout (p < 0,01). In a multivariate analysis, total stress was significantly increased to almost 50% chance of burnout (OR = 1,48; 95% confidence interval 1,24; 1,77), independent of other socio-demographic factors. Conclusions. No association with parental burnout was found among the socio-demographic variables. Also, when evaluating the child's participation in different types of therapies, no link was found between parental burnout. The only variable that had such differences turned out to be total stress, which also increases the possibility of burnout by one and a half times in a multifunctional analysis.
