Funkcionālo un radioloģisko rezultātu salīdzinājums pacientiem pēc pleca locītavas endoprotezēšanas ar Global Icon™ Stemless un Global Unite® anatomic sistēmām

Abstract

Darba aktualitāte: Pleca locītavas endoprotezēšana ir viena no ķirurģiskās ārstēšanas metodēm osteoartrīta slimībai. Mūsdienu tehnoloģiskās iespējas nodrošina endoprotēžu attīstību un daudzveidību. Aizstājot pacienta dabīgo locītavu ar mākslīgo implantu, svarīgi ir izvēlēties viņa stāvoklim, ikdienas aktivitātēm un prasībām atbilstošāko protēzes veidu, lai sasniegtu labāko pēcoperācijas rezultātu. Darba mērķis: Salīdzināt Global Icon anatomiskās bezkājiņas implanta efektivitāti ar Global Unite implanta ar kājiņu efektivitāti, izvērtējot klīniskos un radioloģiskos rezultātus gadu pēc operācijas. Darba uzdevumi: Veikt CMS rezultātu analīzi pirms operācijas un gadu pēc endoprotezēšanas operācijas pacientiem abās grupu populācijās. Salīdzināt CMS rezultātus pacientiem ar Global Icon un Global Unite implantiem gadu pēc operācijas.Veikt rentgenoloģisko izmeklējumu analīzi, izvērtējot pleca locītavas protēzes stabilitāti. Metodes un materiāli: Pētījumā tika iekļauti 25 pacienti, kam tika veikta pleca locītavas endoprotezēšana ar Global Icon stemless implantu un Global Unite stemmed implantu. Iekļauti tika pacienti ar diagnozi- pleca locītavas osteoartrīts. Pētījumā tika vērtēti pirms operācijas Constant-Murely score rezultāti, un to izmaiņas gadu pēc operācijas. Dati tika apkopoti un apstrādāti IBM Statistics 22 programmā. Rezultāti: 12 pacientiem pleca locītava tika aizstāta ar Global Icon implantu, bet 13 pacientiem ar Global Unite. Global Icon grupas vidējais pacientu vecums bija 52,8 gadi; SD=2.57, bet Global Unite grupā 61.1 gads; SD=1.64 CMS skalas vidējais kopvērtējums pirms operācijas Global Icon pacientu grupai bija 25.08; SD=2.15, savukārt Global Unite grupā- 24.07; SD=1.61. Gadu pēc operācijas Global Icon pacientu vidējais CMS kopsummas punktu skaits pieauga līdz 74.18 punkti; SD= 1.97, bet Global Unite grupā- 58.6; SD=3.26. Secinājumi: Global Icon stemless pleca locītavas protēze ir ar efektīvākiem klīniskajiem rezultātiem īstermiņā, nekā Global Unite stemmed pleca protēze, neatkarīgi no pacienta vecuma. Radiogrāfisko attēlu analizēšanas gaitā, kaulu reabsorbcijas pazīmes tika novērotas pa vienam gadījumam katrā pētījuma grupā, bez klīniskām izpausmēm.

Introduction: Shoulder arthroplasy is one of the surgical treatments for degenerative osteoarthritis. Modern technologies provide constant development and diversity of endoprostheses. When replacing patient’s joint with an artifical implant it is important to choose the type of prosthesis that suits best to the condition, fulfills daily activities, needs and most importantly achieves the best postoperative results. Aim: Compare the effectiveness of Global Icon Stemless implant and Global Unite Stemmed implant by evaluating the clinical and radiological results a year after the surgery. Methods: The study included 25 patients who underwent shoulder arthroplasty with Global Icon Stemless implant and Global Unite Stemmed implant. Only patients with pre-operative diagnosis osteoarthritis were included. In the study, patients were evaluated by Constant-Murely score before the surgery and a year after surgery to assess clinical improvements. Data were gathered and evaluated using SPSS IBM Statistics 22 software. Results: 12 patients’ shoulder joint was replaced by Global Icon implant and 13 patients’ shoulder joint was replaced with Global Unite implant. The average age of patients in Global Icon group was 52.8 years; SD=2.57, and 61.1 years ; SD=1.64 in Global Unite group. The average pre-operative total CMS for Global Icon patient group was 25.08; SD=2.15, while the average total score for Global Unite was 24.07; SD=1.61. A year after the surgery, the average CMS for Global Icon patients was 74.18 points; SD=1.97, while Global Unite average total score was 58.6; SD=3.26. Conclusions: Regardless of patient’s age Global Icon Stemless shoulder prosthesis has more effective short term clinical results than Global Unite stemmed shoulder prosthesis. As a result of radiographic analysis, signs of bone reabsorbtion without clinical manifestations were observed in one case in each study group.
