Uztura paradumu atbilstība Ziemeļvalstu diētai un to saistība ar nogurumu čūlainā kolīta pacientiem

Pētījuma galvenās analizējamās tēmas ir iekaisīgo zarnu slimību (IZS) pacientu hroniskais nogurums, uztura paradumu un specifiski Ziemeļvalstu diētas (ZD) potenciālā labvēlīgā ietekme uz noguruma mazināšanu. Darba mērķi ir novērtēt noguruma esamību un smagumu čūlainā kolīta (ČK) pacientiem, viņu uztura paradumu atbilstību ZD, pārbaudīt saistību starp noguruma līmeni un atbilstības pakāpi ZD, ka arī noteikt saikni starp noguruma smagumu un ČK slimības aktivitāti un ilgumu. Pētījumā tika iesaistīti 43 ČK pacienti un 44 kontroles grupas dalībnieki, kuriem novērtējā noguruma līmeni, un to ietekmi uz ikdienu izmantojot IZS - noguruma pašnovērtējuma aptauju (angl. IBD - Fatigue self-assessment questionary), un viņu diētas atbilstību ZD, izmantojot uztura paradumu anketēšanu un Jauno Ziemeļvalstu diētas (JZD) skalu (angl. New Nordic Diet score). Slimības aktivitātes noteikšanai bija izmantota parciāla Mayo skala (angl. partial Mayo scale). Darba iegūtie dati liecina, ka abu grupu dalībnieku uzturs lielākai daļai maz atbilda ZD, un tikai dažiem respondentiem atbilstība ZD bija augsta. Vairākums no ČK pacientiem izjuta vieglu-vidēji smagu nogurumu, kā arī vairākumam nogurums vidēji ietekmēja ikdienas aktivitātes. Pēc statistiskās analīzes tika konstatēts, ka kontroles grupas dalībnieku nogurums stiprāk ietekmēja viņu ikdienas dzīvi nekā ČK pacientiem. Tika atrasta negatīva korelācija starp atbilstību ZD un noguruma līmeni, noguruma ietekmi kontroles grupas dalībniekiem. Tomēr šī saistība netika konstatēta ČK pacientiem. Pētījuma grupas dalībniekiem netika konstatēta statistiski nozīmīga saistība starp slimības aktivitāti, ilgumu un noguruma līmeni, noguruma ietekmi uz ikdienu. Lai precīzāk izvērtētu ZD iespējamo pozitīvo ietekmi uz noguruma līmeni ČK pacientiem, ir nepieciešams turpmākajos pētījumos iesaistīt lielāku dalībnieku skaitu, ka arī izmantot precīzākus darba metodes, piemēram, fiksēt dalībnieku ēdienreizes uztura dienasgrāmatā noteiktu laiku. The main research topics are chronic fatigue in patients with inflammatory bowel disease (IBD), eating habits and specifically Nordic diet potential positive impact on fatigue level reduction. Aims of our scientific study is to detect the level of fatigue in patients with ulcerative colitis (UC), measure eating habits adherence to Nordic diet, and check potential connection between level of fatigue and Nordic diet compliant dietary habits. Another important aim is to detect connection between level of fatigue and ulcerative colitis disease activity and its duration. In research were enrolled 43 ulcerative colitis patients and a control group of 44 people without this disease. We measured both group participants' level of fatigue and its influence on everyday activities by using IBD - Fatigue self-assessment questionary, and their adherence to Nordic diet by using dietary habits survey and New Nordic Diet (NND) score scale. To measure the level of disease activity we used partial Mayo scoring system. The results of research showed that both group participants’ eating habits had low adherence to Nordic diet, only few of respondents had high NND score. General part of UC patients felt low-average level of fatigue, and majority of them had average influence of fatigue on their lives. After statistical analysis it was detected that control group had higher level of fatigue influence on everyday activities than patients with ulcerative colitis. We have found negative correlation between adherence to Nordic diet and level of fatigue, fatigue impact in participants of control group. This correlation was not found in patients with ulcerative colitis. In research group was not found any association between disease activity, its duration and level of fatigue and its influence on life. To confirm the positive impact of Nordic diet on reducing level of fatigue in patients with ulcerative colitis it is necessary to continue research with bigger number of participants. Additionally, it is possible to use more precise tools to measure adherence to Nordic diet, for instance, to write everyday food diary for a definite period of time.
