Ievads. Kognitīvie traucējumi var būtiski ietekmēt indivīda darba spējas, sociālo aktivitāti un samazināt dzīves kvalitāti. Darba autore vēlas pievērst cilvēku uzmanību viegliem kognitīviem traucējumiem, jo tie var būt pirmsdemences stāvoklis. Mērķis. Agrīni atklāt iespējamus kognitīvos traucējumus pacientiem pēc 60 gadu vecuma. Materiāli un metodes. Prospektīva tipa pētījums, kas norisinājās ģimenes ārsta praksē (AS “VCA” poliklīnikā “Jugla”). Pētījumā tika iekļauti pacienti vecumā virs 60 gadiem, kuriem iepriekš nebija diagnosticēti kognitīvie traucējumi. Katrs pētījuma dalībnieks aizpildīja protokolu, kas sastāvēja no divām daļām. Pirmā daļa bija anketa, savukārt otrā – MoCA tests. Pētījuma laikā iegūtie dati tika apstrādāti ar statistikas programmu IBM SPSS Statistics 26.0 un Microsoft Excel 2019. Rezultāti. Kopumā pētījumā piedalījās 45 pacienti vecumā no 60 līdz 87 gadiem, no kuriem 80% (n=36) bija sievietes un 20% (n=9) – vīrieši. Respondentu vidējais vecums bija 67,1 gads (SD=7,2 gadi). 62,2% respondentu (n=28) bija vidējā izglītība, 35,6% (n=16) – augstākā un 2,2% (n=1) – nepabeigta pamatizglītība. 48,9% respondentu (n=22) bija krievi, 22,2% (n=10) – latvieši, 6,7% (n=3) – poļi, savukārt pārējie savu tautību nenorādīja. 60% respondentu (n=27) nebija novēroti kognitīvie traucējumi ģimenē, savukārt 6,7% (n=3) – šo informāciju precizēt nevarēja. Lielākai respondentu daļai bija 1 (n=17; 37,8%) vai 2 (n=15; 33,3%) hroniskas slimības. Visbiežāk sastopama hroniska slimība visu respondentu vidū bija arteriālā hipertensija (n=32; 72,7%), otro vietu aizņem hipotireoze (n=6; 13,6%). Pēc MoCA testa rezultātiem, lielākai respondentu daļai tika atklāti kognitīvie traucējumi (n=35; 77,8%). MoCA testa minimālais iegūtais punktu skaits bija 16, savukārt maksimālais – 29 punkti. No visām kognitīvām jomām visbiežāk tika traucēta vārdu atsaukšana atmiņā (n=37; 82,2%), vadības funkcijas (n=34; 75,6%), vizuāli telpiskās spējas (n=29; 64,4%) un valodas raitums (n=28; 62,2%), savukārt visretāk tika traucēta attēlā redzamo dzīvnieku nosaukšana (n=4; 8,9%) un orientācija laikā (n=1; 2,2%), orientācija telpā saglabāta visiem respondentiem. Secinājumi. Kognitīvo traucējumu hipodiagnostika ir aktuāla mūsdienu problēma, to apstiprina pētījumā iegūtie rezultāti – 77,8% respondentu tika atklāti kognitīvie traucējumi, par ko iepriekš nebija zināms, tostarp 33,3% jau agrīni – 60-64 gadu vecumā. Netika iegūta statistiski ticama asociācija starp kognitīviem traucējumiem un dzimumu, tautību, izglītības līmeni, hronisko slimību skaitu, miega traucējumiem, smēķēšanu un alkohola lietošanu (p>0,05). 31,4% respondentu ar normai neatbilstošu MoCA rezultātu (<26 punkti), apstiprinājās pozitīva kognitīvo traucējumu ģimenes anamnēze. 71,4% respondentu ar normai neatbilstošu MoCA rezultātu (<26 punkti), apstiprinājās dažādi kardiovaskulāri riska faktori. Cognitive impairment can significantly affect an individual's working capacity, social activity and reduce the quality of life. The author of the paper wants to draw people's attention to mild cognitive impairment, as they can be a pre-dementia condition. Aim. Early diagnostics of possible cognitive impairment in patients over the age of 60. Materials and methods. A prospective study, that took place in a general practitioner’s practice (“Jugla” medical center). The study included patients over the age of 60 who had not been previously diagnosed with cognitive impairment. Each participant in the study completed a protocol consisting of two parts. The first part was a questionnaire and the second part was a MoCA test. The data obtained during the study was processed by IBM SPSS Statistics 26.0 and Microsoft Excel 2019. Results. A total of 45 patients aged 60-87 years participated in the study, of whom 80% (n=36) were female and 20% (n=9) were male. The average age of respondents was 67.1 years (SD=7.2 years). 62.2% of respondents (n=28) had secondary education, 35.6% (n=16) – higher and 2.2% (n=1) – unfinished primary education. 48.9% of respondents (n=22) were Russians, 22.2% (n=10) – Latvians, 6.7% (n=3) – Poles, while others did not indicate their ethnicity. 60% of respondents (n=27) did not experience cognitive impairment in the family, while 6.7% (n=3) could not specify this information. The majority of respondents had 1 (n=17; 37.8%) or 2 (n=15, 33.3%) chronic diseases. The most common chronic disease among all respondents was arterial hypertension (n=32; 72,7%), the second place was occupied by hypothyreosis (n=6; 13,6%). According to the MoCA test results, the majority of respondents had cognitive impairment (n=35; 77.8%). The minimum score for the MoCA test was 16 and the maximum – 29 points. Of all cognitive domains, delayed recall (n=37; 82,2%), executive functions (n=34; 75,6%), visuoconstructional skills (n=29; 64,4%) and language fluency (n=28; 62,2%) were most commonly impaired, while naming (n=4; 8.9%) and orientation in time (n=1, 2.9%) were the least impaired. Conclusions. Hypodiagnostics of cognitive impairment is a serious problem of our time, as confirmed by the results of the study – 77.8% of respondents discovered cognitive impairment, which was previously unknown, including 33,3% of respondents at the age of 60-64 years. There was no statistically significant association between cognitive impairment and gender, ethnicity, level of education, number of chronic diseases, sleep disturbances, smoking and alcohol consumption (p>0.05). 31.4% of respondents with abnormal MoCA score (<26 points) confirmed a positive family history of cognitive impairment. 71.4% of respondents with abnormal MoCA score (<26 points) confirmed various cardiovascular risk factors.
