Staphylococcus aureus, it īpaši tā meticilīnrezistentie celmi ir vieni no izturīgākajiem mikroorganismiem visā pasaulē, kas pateicoties tiem piemītošajai ģenētiskajai daudzveidībai spēj mainīt sev piederošo virulences faktoru ekspresiju. Tā rezultātā kļūstot par strukturāli un ķīmiski daudzveidīgiem mikroorganismiem, kam ir būtiska loma ar veselības aprūpi saistīto infekciju izplatībā. Mūsdienās, neskatoties uz pēdējos gados vērojamo MRSA celmu izplatības samazināšanos lielākajā daļā ES lielvalstu, arvien biežāk tiek ziņots par pieaugošu Panton-Valentine leikocidīna pozitīvu S. aureus celmu izplatību. Pateicoties, kam tiek sekmēta gan antibakteriālās rezistences attīstība, gan organisma imūnās sistēmas atbildes reakcijas samazināšanās. Tādējādi veicinot SSTIs, bakteriēmijas un pneimonijas infekciju komplikāciju attīstību. Darba mērķis: Noteikt Staphylococcus aureus klīniskajos izolātos meticilīnrezistences gēna mecA un toksīna PVL sintēzi noteicošā gēna luk klātesamību. Darba uzdevumi: 1. Noteikt S. aureus izolātu antibakteriālo jutību, izmantojot BBL™ disku difūzijas testu. 2. Veikt S. aureus DNS izdalīšanu. 3. Noteikt mecA gēna klātbūtni S. aureus izolātos, izmantojot GenoType ® MRSA un FilmArray ® Multiplex PCR testus. 4. Veikt toksīna PVL sintēzi noteicošā gēna luk molekulārbioloģisko noteikšanu, izmantojot GenoType ® MRSA testu. 5. Veikt iegūto rezultātu statistisko apstrādi. Rezultāti un secinājumi: Lietojot fenotipiskās metodes un sekojoši analizējot 261 S. aureus kultūru, tika identificēta 100% visu analīzes paraugu jutība pret glikopeptīdu grupas medikamentu – vankomicīnu. Līdzīgus rezultātus uzrādīja arī antibakteriālās jutības testi pret citu grupu antibiotikām, kas tādējādi var tikt pielietotas gan MSSA, gan MRSA izraisītu infekciju ārstēšanā. BBL™ disku difūzijas testa laikā tika atklāti kopumā 8 jeb 3,06% pret cefoksitīnu rezistenti gadījumi, no kuriem vienam paraugam pēc attiecīgi GenoType ® MRSA un FilmArray ® Multiplex PCR testu veikšanas apstiprinājās PVL toksīna klātesamība. Tā rezultātā liecinot, par joprojām pastāvošo, pret antibakteriālajiem līdzekļiem rezistento S. aureus celmu eksistenci. Atslēgvārdi: MRSA, Staphylococcus aureus, PVL, mecA, antibakteriālā rezistence Staphylococcus aureus, in particular its methicillin-resistant strains, are one of the most resistant microorganisms in the world, which, due to their inherent genetic diversity, are able to alter the expression of their virulence factors. As a result, becoming structurally and chemically diverse microorganisms that play a key role in the spread of healthcare associated infections. Knowdays, despite the decline of the prevalence of MRSA strains in most EU countries, inversely an increasing currency of Panton-Valentine leukocidin positive S. aureus strains are being reported. Due to which the development of antibacterial resistance and the reduction of the body's immune response are promoted. Thereby initiating the development of complications of SSTIs, bacteremia and pneumonia infections. Aim of the work: Is to determine the presence of methicillin-resistance gene mecA and toxin PVL synthesis gene luk in Staphylococcus aureus clinical isolates. Tasks: 1. Determine the antibacterial susceptibility of S. aureus isolates using the BBL ™ disc diffusion test. 2. Perform S. aureus DNA extraction. 3. Detect the presence of the mecA gene in S. aureus isolates using GenoType ® MRSA and FilmArray ® Multiplex PCR tests. 4. Perform molecular biological identification of the gene determining the toxin PVL synthesis using the GenoType ® MRSA test. 5. Perform statistical processing of the results obtained in the study. Results and conclusion: During phenotypic methods, in which 261 cultures of S. aureus where analysed, 100% of test samples showed sensitivity to the glycopeptide drug group medicament- vancomycin. A similar result was shown also after antibacterial susceptibility testing against other classes of antibiotics, in which case they can be used to treat both MSSA and MRSA infections. Durning the BBL ™ disk diffusion test, a total of 8 or 3,06% cases of cefoxitin-resistant isolates where detected, of which one sample confirmed the presence of PVL toxin, after GenoType ® MRSA and FilmArray ® Multiplex PCR tests. In this case showing evidence of the continued existence of S. aureus strains resistant to antimicrobials. Keywords: MRSA, Staphylococcus aureus, PVL, mecA, antibacterial resistance
