Dotā maģistra darba tēma ir pašvaldību loma veselības nozarē. Tēma ir aktuāla, jo ārstniecības nozare vienmēr ir bijusi problemātiska, gan finansējuma dēļ, gan cilvēkresursu trūkuma dēļ. Ar šīm problēmām saskarās gan Latvijā, gan pasaule kopumā. Veicot dažādas reformas ne vienmēr tiek sasniegti vēlamie mērķi. Iespējams mērķus sasniegšanai pozitīva ietekme ir tā, ka ārstniecības iestādes pieder ne tikai valstij, bet to kapitāldaļu turētāji ir arī pašvaldības, kuru uzdevumi un pienākumi ir atrunāti pašvaldību likumdošanā. 2019. gada nogalē pasauli pārņēma Covid-19 vīruss, kas 2020. gada martā tika konstatēts arī Latvijā. Vīrusa izraisītā pandēmija ieviesa savas korekcijas pasaules un Latvijas veselības nozarē. Valsts operatīvās medicīniskās komisijas izsludināja ārkārtas situācija medicīnas nozarē 2020. gada nogalē. Izsludinot ārkārtas situāciju nozarē ārstniecības iestādēm tikai piešķirti papildus finanšu resursi, lai palielinātu gultas vietu skaitu un pārprofilētu un pielāgotu esošās slimnīcas, ar Covid – 19 inficētu pacientu vajadzībām. Šādas nodaļas tika izveidotas arī slimnīcās, kuru kapitāldaļu turētāji ir pašvaldības. Pašvaldību loma veselības nozarē ir atrunāta Latvijas Republikas likumā “Par pašvaldībām, ” kur tiek minēts, ka pašvaldībām ir jānodrošina savas pašvaldības iedzīvotājiem pieejama veselības aprūpe. Finansēšana veselības aprūpes attīstībā un pakalpojumu sniegšanas kvalitātes uzlabošanā ir katras pašvaldības iniciatīva. Vairāki bijušie veselības ministri, tai skaitā Ilze Viņķele uzsvēra pašvaldību nozīmi problēmu risināšanā un spējā adekvāti reaģēt krīzes situācijā. Īpaši pandēmijas un ārkārtas situācijas laikā, uzsverot, ka pašvaldības ir ļoti nozīmīgs partneris dažādu nozaru ministrijām. Līdz ar to pandēmijas laiku var izmantot, lai, kopīgi strādājot, nepilnības novērstu, un iedzīvotājiem būtu pieejama un kvalitatīva veselības aprūpe. Ārkārtas situācija veselības nozarē un pandēmija valstī aktualizē jautājumu par pašvaldību lomu veselības nozarē un nepieciešamību turpmāk saglabāt ārstniecības iestādes, reģionos un centrālajās pašvaldībās, ar nemainīgu gultas vietu skaitu. Veicot pētījumā iegūto rezultātu analīzi tika noskaidrots, ka pašvaldībā ir jāsaglabā veselības aprūpes iestādes, kuras sniedz primāro un sekundāro veselības aprūpi, lai nodrošinātu savas pašvaldības robežas, tās iedzīvotājiem pieejamu veselības aprūpi. Darba ir iekļauti astoņi attēli un viens pielikums. Pētījuma mērķis ir noskaidrot un izvērtēt pašvaldību lomu primārajā un sekundārajā veselības aprūpē. The topic of this master's thesis is the role of local governments in the health sector. The issue is relevant because the medical industry has always been problematic, both due to funding and lack of human resources. These problems are faced both in Latvia and in the world as a whole. Various reforms do not always achieve the desired goals. The possible positive effect of achieving the goals is that the medical institutions are not only owned by the state, but also by the local governments, whose tasks and responsibilities are stipulated in the local government legislation. At the end of 2019, the world was taken over by the Covid-19 virus, which in March 2020 was also detected in Latvia. The pandemic caused by the virus introduced its adjustments in the global and Latvian health sector. The State Emergency Medical Commissions declared a medical emergency in late 2020. By declaring a state of emergency in the sector, medical institutions have only been given additional financial resources to increase the number of beds and to re-profile and adapt existing hospitals to the needs of 19 Covid-infected patients. Such departments were also established in hospitals owned by municipalities. The role of local governments in the health sector is stipulated in the Law of the Republic of Latvia “On Local Governments”, where it is mentioned that local governments must ensure accessible health care for the residents of their local government. Funding for the development of health care and the improvement of the quality-of-service provision is an initiative of each municipality. Several former health ministers, including Ilze Viņķele, emphasized the importance of local governments in solving problems and their ability to respond adequately to a crisis situation. Especially during a pandemic and emergency, emphasizing that local governments are a very important partner for ministries in various sectors. As a result, pandemic time can be used to work together to close gaps and ensure that citizens have access to high-quality healthcare. The emergency situation in the health sector and the pandemic in the country raise the issue of the role of local governments in the health sector and the need to maintain medical institutions, regions and central municipalities, with a constant number of beds. The analysis of the results obtained in the study revealed that in the municipality it is necessary to maintain health care institutions that provide primary and secondary health care in order to ensure the borders of their municipality, health care available to its inhabitants. The work includes eight figures and two appendices. The aim of the research is to find out and evaluate the role of local governments in primary and secondary health care.
