S-ICD implantācijas rezultāti Latvijā

Abstract

S-ICD ir jauna alternatīva TV-ICD metodei, kas tiek implantēta pēkšņas kardiālās nāves profilakses nolūkos un atšķirībā no TV-ICD lokalizējas pilnīgi ekstrakardiāli, novēršot vairākas ar transvenozu implantāciju saistītas komplikācijas. Pētījuma mērķis ir salīdzināt S-ICD un TV-ICD implantāciju un tās rezultātus Latvijā, sākot no 2018. gada. S-ICD grupas pacientiem novērots īsāks QRS komplekss, biežāk implantācija veikta sekundārās profilakses nolūkos, izmantojot vispārējo anestēziju, salīdzinot ar TV-ICD grupu. Kā arī, S-ICD grupā nav novērots lielāks komplikāciju un neadekvātu šoku skaits, nav novērota sliktāka dzīvību apdraudošu sirds ritma traucējumu (VT/VF) atpazīšanas funkcija, salīdzinot ar TV-ICD grupu, un tāpēc ir secināts, ka S-ICD ir līdzīgi TV-ICD efektīva un droša metode pacientiem ar ICD implantācijas indikāciju.

S-ICD is a new alternative to the TV-ICD method, which is implanted to prevent sudden cardiac death and unlike TV-ICD is implanted completely extracardiacally, reducing the number of complications associated with transvenous implantation. The aim of the study is to compare S-ICD and TV-ICD implantation and implantation results in Latvia, starting from 2018. Patients in the S-ICD group had a shorter QRS complex, more frequent implantations for secondary prophylaxis and all patients underwent implantation under general anesthesia compared to TV-ICD group. There was no increased rate of complications and inappropriate shocks, and no poorer detection of life-threatening cardiac arrhythmias (VT/VF) compared to TV-ICD group - S-ICD is a similarly efficacious and safe method to TV-ICD in patients with an indication for ICD implantation.
