dc.description.abstract | Pētījuma galvenās analizējamās tēmas ir akūtu laparotomiju pacientu brūču slēgšanas metodes un tehnikas un to ietekme uz brūces dzīšanas procesu, agrīnu komplikāciju attīstību. Darba mērķis ir retrospektīvi salīdzināt dažādas brūču slēgšanas metodes pie neatliekamām laparotomijām. Pētījumā tika analizēti 100 pacientu operāciju protokoli un slimības vēstures pēc neatliekamām laparotomijām, operāciju protokoli tiek pielietoti sākot ar 2021.gadu. Tika analizētas brūču slēgšanas metodes, tehnikas un izmantotie materiāli. Tika apkopoti dati par pēc operāciju agrīnām komplikācijām. Darbā iegūtie dati liecina ,ka lielākā daļa ķirurgu izmanto vispārpieņemtas metodes un izmanto rekomendējamos materiālus brūču slēgšanai pēc neatliekamām laparotomijām. Tika konstatēts ,ka sastop salīdzinošu mazu skaitu pēc operāciju komplikācijas. Pēc statistikās analīzes tika konstatēts ,ka visbiežākās patoloģijas kad tiek veikta neatliekama laparotomija ir ileuss, kuņģa čulas perforācija un apendicīts. Ti-ka konstatēts, ka izmantojot šauro dūriena tehniku ≤ 10mm un >10 – 20mm aponei-rozes šūšanai un komplikāciju attīstībai , netika iegūta statistiski nozīmīga saistība (p = 0,576). Bet novēro tendenci ,ka komplikācijas var attīstīties platās šuves gadī-jumā. Apskatot antibakteriāla diega izmantošanas saistību ar komplikācijām, netika atrasta statistiski nozīmīga saistība (p>0,999). Visbiežāk izmantotais diegs ir PDS. Aponeirozes šuvē iekļautie slāņi uzrādīja statistiski nozīmīgu saistību ar komplikā-ciju esamību (p = 0,031), vairākos slāņos komplikācijas ir biežāk nekā tad, ja apo-neirozē iekļauti tikai aponeirozes slāņi, t. i., 9.3% pret 0,0%. Lai precīzāk izvērtētu visefektīvāko brūces slēgšanas metodi un vislabākos materiālus, nākotnē ir jāveic prospektīvs pētījums ar stingru kontroli pēc nealieka-mām laparotomiskām operācijām. Turpmākajos pētījumos ir jāiesaista lielāks dalīb-nieku skaits. Precīzāk jāizvērtē pēc operāciju komplikāciju attīstība. | |
dc.description.abstract | The main topics of the study are the methods and techniques of wound clo-sure in patients with acute laparotomy and their impact on the wound healing pro-cess, the development of early complications. The aim of the study is to retrospectively compare different wound closure methods in emergency laparotomies. The study analyzed the operating protocols and medical histories of 100 pa-tients after emergency laparotomies, operating protocols are applied from 2021 onwards. Wound closure methods, techniques and materials used were analyzed. Da-ta on early postoperative complications were collected. The data obtained in the work show that most surgeons use generally accep-ted methods and use the recommended materials, which are closed after urgent lapa-rotomies. A relatively small number has been shown to be present as a result of sur-gery. According to statistics, the pathologies of the analysis when performing an emergency laparotomy are ileus, perforation of the abdominal ulcer and appendici-tis. No statistically significant correlation was found using the narrow stitch te-chnique ≤ 10mm and> 10 - 20mm for suture and complication of the aponeus (p = 0.576). But there is a tendency for development to develop in the case of a wide seam. When looking at the association of antibacterial thread use with compli-cations, no statistically significant association was found (p> 0.999). The most commonly used thread is PDS. The layers included in the aponeurosis suture showed a statistically significant association with the presence of complications (p = 0.031), in several layers it is more common than if only the aponeurosis layers are included in the apo-neurosis. i.e. 9.3% versus 0.0%. In order to more accurately evaluate the most effective method of wound clo-sure and the best materials, a prospective study with strict control after non-essential laparotomy operations should be performed in the future. Further research is needed to increase the number of participants. The development of complications after sur-gery needs to be assessed more closely. | |
