"Forsētās oscilometrijas iespējas plaušu funkcijas monitoringā COVID-19 pneimonijas pacientiem akūtajā un rekonvalescences periodā" Laura Grima. Koronarvīrusa slimība 2019 (COVID-19) ir ļoti aktuāla problēma visā pasaulē, kas pārslogojusi veselības aprūpes sistēmu. COVID-19 pneimonijas gaita ir grūti prognozējama, tādēļ būtiski izvērtēt tās dinamiku, izmantojot objektīvas izmeklēšanas metodes. Mērķis noskaidrot vai forsētā oscilometrija var kalpot kā metode, lai novērtētu COVID-19 pneimonijas gaitu akūtajā un rekonvalescences periodā un vai šie rādītāji asociē ar citiem slimības smagumu raksturojošiem rādītājiem. Šajā prospektīvā pētījumā pacientiem ar akūtu COVID-19 pneimoniju (n=30) tika veikta forsētā oscilometrija (FOT) visu hospitalizācijas laiku (n=96), ko salīdzināja ar datortomogrāfijas (CT) rezultātiem (n=30). Atkārtotus FOT mērījumus (n=17) veica trīs mēnešus pēc izrakstīšanas, reizē ar spirogrāfijas (n=26), pārejas faktora un visa ķermeņa pletizmogrāfijas izmeklējumiem(n=22). Rezultāti: hospitaizācijas laikā AX un RF ir paaugstināti, bet par pārsteigumu starp FOT un CT-indeksu nepastāv būtiska sakarība, pastāv būtiskas sakarības: AX un RF atkarībā no PaO2/FiO2 un dienu skaita pirms izrakstīšanas (p<0,05). Atkārtoti FOT mērījumi ar būtisku atšķirību R5-20, AX, RF (p<0.05). AX atkarībā no FEV1 (p<0.05), RF atkarībā no RV/TLC un MMEF (p<0,05). Secinājumi. Šī metode ļauj sekot slimības dinamikai akūtajā periodā. Sensitīvāk slimības smaguma dinamiku atspoguļo reaktances rādītāji AX un RF, bet rādītājs R5-20 liecina par perifēro elpceļu obstrukcijas heterogenitāti plaušās. Ar šiem FOT rādītājiem asociē atrašanās laiks slimnīcā un PaO2/FiO2, bet netika novērota asociācija ar CT indeksu. Trīs mēnešus pēc izrakstīšanas, reaktances rādītāji ir būtiski uzlabojušies, bet vēl nesasniedz individuālās normas līmeni. R5-20 rādītājs, kas liecina par elpceļu obstrukcijas heterogenitāti, pēc 3 mēnešiem ir atgriezies tuvu normas robežai. Rādītāji RF asociē ar RV/TLC un MMEF, kas ir perifēro elpceļu obstrukcijas rādītāji un AX asociē ar FEV1, kas ir kopīgais obstrukcijas rādītājs elpceļos. X5 asociē ar RV/TLC, kas arī ir rādītājs, kas atspoguļo obstrukciju perifērajos elpceļos. Coronavirus disease 2019 (COVID-19) is an ongoing challenge around the world and has overloaded the health care system. COVID-19 pneumonia dynamics is hard to prognose thus it is important to evaluate the dynamics of COVID-19 pneumonia using objective diagnostic methods. Aim: can forced oscillometry (FOT) be used as a method to evaluate COVID-19 pneumonia dynamics during acute and reconvalescence period and do FOT indices associate with other data that characterize disease severity. Methods: we prospectively studied patients with acute COVID-19 pneumonia (n=30) and performed FOT (n=96) during hospital and compared this data to CT results (n=30). Repeated FOT measurements (n=17) were performed three months after discharge in conjunction with spirography (n=26), diffusion capacity and full body plethysmography (n=22). Results: during hospitalisation AX and RF were above normal values but to my surprise there were no significant association between FOT indices and CT score. Significant associations were between AX, RF and PaO2/FiO2 and days before discharge (p<0,05). Repeated FOT measurements showed significant difference for R5-20, AX, RF (p<0.05). There is significant association between AX and FEV1 (p<0.05), RF and RV/TLC, MMEF (p<0,05). Conclusions. FOT allows to follow disease dynamics during acute period. The most sensitive indices for disease severity dynamics are AX and RF, but R5-20 shows heterogeneity of peripheral obstruction in lungs. These FOT indices associate with time spent in hospital and PaO2/FiO2, but not with CT score. Three months after discharge reactance indices has significantly improved, but do not reach normal values yet. R5-20 shows heterogeneity of airway obstruction and after three months it has returned close to normal value. RF associate with RV/TLC and MMEF that indicate peripheral airway obstruction and AX associate with FEV1 that indicates obstruction in airways overall. X5 associate with RV/TLC that represents peripheral airway obstruction.
