Cukura diabēta pacientu terapijas līdzestību ietekmējošie faktori

Pētījuma aktualitāte: cukura diabēts ir hroniska neinfekcijas slimība, kas ar katru gadu skar arvien lielāku pacientu skaitu. Ieteiktos glikēmijas mērķus sasniedz mazāk nekā 50,0% pacientu, kas varētu būt saistīts ar samazinātu terapijas ievērošanu. Nozīmētās terapijas neievērošanas rezultātā var attīstīties hiperglikēmija un ilgtermiņa komplikācijas, kas turpmāk var palielināt saslimstību, priekšlaicīgas nāves risku, kā arī veselības aprūpes pakalpojumu izmaksas. Lai cukura diabēta pacientu slimības kontrole būtu efektīva, būtiski izprast faktorus, kas samazina vai otrādi paaugstina terapijas līdzestību. Pētījuma mērķis: izpētīt cukura diabēta pacientu terapijas līdzestību ietekmējošos faktorus. Pētījuma metodes: darbā ir izmantotas divas anketas: “Pirmā tipa cukura diabēta pacientu terapijas līdzestību ietekmējošie faktori” un “Otrā tipa cukura diabēta pacientu terapijas līdzestību ietekmējošie faktori”. Aptaujas rezultāti ir apstrādāti Microsoft Office Excel 2017 programmā. Rezultātu apstrādē ir izmantotas aprakstošas un analītiskas statistikas metodes. Lai noteiktu atšķirības starp mainīgo kategorijām ir izmantots Pīrsona Hī kvadrāta tests (χ2) ar nozīmīguma līmeni p<0,05 un, lai novērtētu iegūto rezultātu ticamību ir pielietots 95% ticamības intervāls. Darbā ir analizētas terapijas līdzestības kategorijas sakarā ar pirmā un otrā tipa cukura diabēta pacientu saistītiem faktoriem, ar slimību un terapiju saistītiem faktoriem, ar veselības aprūpi un veselības aprūpes speciālistiem saistītiem faktoriem. Nozīmīgākie rezultāti un secinājumi: augsta terapijas līdzestība biežāk bija novērota pirmā tipa cukura diabēta pacientiem (82,4%), salīdzinot ar otrā tipa cukura diabēta pacientiem (64,1%). Kopīgie faktori, kas pirmā un otrā tipa cukura diabēta pacientiem bija statistiski nozīmīgi un sekmēja augstu terapijas līdzestību bija glikētā hemoglobīna līmenis <7,0% vai robežās no 7,0% līdz 8,0%; informācijas sniegšana par terapijas nozīmi un tās neievērošanas sekām; zāļu lietošana tā kā ir nozīmējis ārsts un neizjusta veselības pasliktināšanās mainoties kompensācijas zālēm. Būtiskie faktori, kas veicināja augstu terapijas līdzestību pirmā tipa cukura diabēta pacientiem bija glikēmijas paškontrole; labas zināšanas par izrakstītajām zālēm cukura diabēta ārstēšanai; nodarbošanās ar fiziskajām aktivitātēm. Savukārt otrā tipa cukura diabēta pacientiem augstu terapijas līdzestību sekmēja dzīvesvieta pilsētā; nodarbinātība; slimības ilgums līdz 5 gadiem; perorālo zāļu lietošana; mazāks lietoto zāļu skaits; neizjustas zāļu blakusparādības; diezgan labs veselības stāvoklis; iesaiste slimības izmeklēšanas un ārstēšanas plāna sastādīšanā. Rezultātu praktiskā pielietojamība: rezultātus var izmantot turpmāk – pētot cukura diabēta pacientu terapijas līdzestību uzlabojošos pasākumus. Less than half of patients achieve the recommended glycemic target, which may be due to reduced adherence. Failure to follow the prescribed therapy may lead to hyperglycaemia and long-term complications, which may increase morbidity, the risk of premature death, and the cost of health care. Understanding the factors that reduce or vice versa in treatment is essential for effective control of the disease in diabetic patients. Research purpose: factors influencing diabetes mellitus patient therapy adherence. Research methods: the research: "Factors influencing type 1 diabetes mellitus patient therapy adherence" and "Factors influencing type 2 diabetes mellitus patient therapy adherence" were used in the thesis. The survey results were processed in Microsoft Excel 2017. Descriptive and analytical statistical methods are used for processing the results. The Pearson Chi-square (χ2) with a significance level of p < 0.05, was used to determine the differences between categories of groups. The 95% confidence interval was used to evaluate the reliability of the results. In thesis analyzed adherence categories to diabetes mellitus patient-related factors, factors related to disease and treatment, and factors related to health care and health care professionals. Key Findings and Conclusions: high therapy adherence is more common for patients with type 1 diabetes (82,4%) in comparison to patients with type 2 diabetes (64,1%). Common factors that were statistically significant for patients with 1 and 2 types of diabetes and contributed to high therapy adherence were glycated haemoglobin <7,0% or 7,0% - 8,0%; providing information on the importance of treatment and the consequences of non-adherence; the use of the medicine as prescribed by a doctor and did not feel any deterioration in health due to changes in the compensatory medicaments. Factors that bring the most contribution to the high level of adherence in patients with type 1 diabetes are glycemic self-control, good knowledge of the prescribed drugs for the treatment of diabetes, and physical activity. But for patients with type 2 diabetes high levels of adherence are facilitated by city residence; employment status; duration of illness; use of oral medications; a small number of medications; no adverse drug reactions; relatively good health; self-involvement in the investigation and treatment plan. The practical applicability of the results: the results can be used in the future – to study diabetic patients' therapy adherence to improving equivalence.
