Pacientu apmierinātība ar anestēziju un analgēziju

Ievads: Pacientu apmierinātība ir svarīgs rādītājs, ko izmanto veselības aprūpes kvalitātes mērīšanai. Pacientu apmierinātība tiek raksturota kā subjektīvs iegūtā pakalpojuma novērtējums, kas tiek definēts kā cerību un saņemtās aprūpes salīdzinājuma rezultāts. Darba mērķis: Izvērtēt pacientu apmierinātību ar Traumatoloģijas un Ortopēdijas slimnīcā sniegtajiem anestēzijas un analgēzijas pakalpojumiem un identificēt neapmierinātību izraisošos faktorus. Materiāli un metodes: Observācijas prospektīvs pētījums tika veikts Traumatoloģijas un Ortopēdijas slimnīcā no 2022.gada septembra līdz decembrim. Pētījuma instruments bija aptaujas anketa. Tika ievākti dati par pacientu apmierinātību ar tiem sniegto aprūpi un atsāpināšanu perioperatīvajā periodā, sāpēm un blakusparādībām pēcoperācijas periodā. Apmierinātība tika izvērtēta no 0 līdz 10, kur 0 apzīmē pilnībā neapmierināts un 10 pilnībā apmierināts. Pacienti tika lūgti izvērtēt sāpju intensitāti 10 ballēs, izmantojot vizuālu analoga skalu. Pīrsona Hī kvadrāta statistiskais tests izmantots, lai identificētu asociāciju starp pacientu apmierinātību ar anestēziju un analgēziju un to ietekmējošiem faktoriem. Pētījuma būtiskuma līmenis izvēlēts p<0.05. Datu apstrāde veikta ar IBM SPSS Statistics 27. Rezultāti: Pētījumā piedalījās 102 pacienti (71 sieviete un 31 vīrietis), kas tika sadalīti četrās vecuma grupās – 18 līdz 39 (n=6), 40 līdz 59 (n=18), 60 līdz 79 (n=70) un no 80 līdz 90 gadiem (n=8). Pilnībā apmierināti ar tiem sniegto anestēziju un analgēziju bija 70.6% pacientu ar vidēju apmierinātības rādītāju 9.42 balles. 76.5% pacientu bija pilnībā apmierināti ar pēcoperācijas atsāpināšanu, neskatoties uz mērenas intensitātes sāpēm. Apmierinātība ar anestēziju bija vienādi augsta starp sievietēm un vīriešiem, kaut gan sievietes atzīmēja stiprāku sāpju intensitāti un izteiktākas blakusparādības. Augstākā apmierinātība ar anestēziju, kā arī ar pēcoperācijas atsāpināšanu bija 80 līdz 90 gadu vecuma grupā. Izmantojot Pīrsona Hī kvadrāta testu, tika novērota statistiski ticama asociācija starp apmierinātību ar anestēzijas veidu un pirmsoperācijas etapa (pirmsoperācijas apskates un tajā sniegto informāciju (p<0.001)), operācijas etapa (veltīto uzmanību (p<0.001), informāciju (p=0.001), attieksmi (p=0.006), privātumu (p=0.007) un darba ātrumu (p<0.001)) un pēcoperācijas etapa (sāpju intensitāti kustību laikā (p<0.05), niezi (p=0.012) un diskomfortu (p<0.001)) faktoriem. Secinājumi: 1. 70.6% pacientu bija pilnībā apmierināti ar sniegto anestēziju un analgēziju ar vidējo apmierinātības rādītāju virs 8,5. 2. Pirmsoperācijas faktori, kā pirmsoperācijas apskate un sniegtā informācija, ietekmē pacientu apmierinātību. 3. Operācijas etapā veltītā uzmanība, informācija, attieksme, privātums un anestēzijas komandas darba ātrums ietekmē pacientu apmierinātību. 4. Pēcoperācijas etapā gan sāpju intensitāte kustību laikā, gan blakusparādības (nieze un diskomforts) ietekmē pacientu apmierinātību. Introduction: Patient satisfaction is an important measure used to evaluate the quality of health care. Patient satisfaction is characterized as a subjective evaluation of the service received, which is defined as the result of a comparison between expectations and the care received. Objective: Evaluate patient satisfaction with the anesthesia and analgesia services provided at the Hospital of Traumatology and Orthopaedics and identify the factors causing dissatisfaction. Methods and materials: From September to December of 2022 an observational prospective study was conducted at the Hospital of Traumatology and Orthopedics. The research instrument was a questionnaire. Data was collected on patient satisfaction with care and analgesia in the perioperative period, pain, and side effects in the postoperative period. Satisfaction was categorized on a scale from 0 to 10, 0 being completely unsatisfied and 10 being completely satisfied. Patients were asked to evaluate pain intensity in 10 point system using Visual Analog Scale. Pearson’s chi-squared statistical test was used to identify the association between patient satisfaction with anesthesia and analgesia and its influencing factors. Level of statistical significance is set at p<0.05. Data analysis was performed with IBM SPSS Statistics 27. Results: 102 patients (71 women and 31 men) participated in the study, who were divided into four age groups - 18 to 39 (n = 6), 40 to 59 (n=18), 60 to 79 (n=70) and 80 to 90 years (n=8). 70.6% of patients were completely satisfied with the anesthesia and analgesia provided to them, with a mean satisfaction score of 9.42 points. 76.5% of patients were completely satisfied with postoperative analgesia despite moderate intensity pain. Satisfaction with the received anesthesia was equally high between men and women, although women reported greater pain intensity and more severe side effects. The highest satisfaction with anesthesia, as well as with postoperative analgesia, was in the 80-90 age group. Pearson’s chi-squared test determined statistically significant association between satisfaction with anesthesia and the preoperative period (preoperative examination and the information (p<0.001)), the surgical period (attention (p<0.001), information (p=0.001), attitude (p=0.006), privacy (p=0.007) and speed of work (p<0.001)) and postoperative period (pain intensity during movements (p<0.05), itching (p=0.012) and discomfort (p<0.001)) factors. Conclusion: 1. 70.6% of patients were completely satisfied with the provided anesthesia and analgesia with a mean score above 8.5. 2. Preoperative factors such as preoperative examination and provided information affect patient satisfaction. 3. The attention, information, attitude, privacy, and speed of work of the anesthesia team during the operation period affect patient satisfaction. 4. In the postoperative period pain intensity during movements and side effects (itching and discomfort) affect patient satisfaction.
