Miega traucējumi un tos ietekmējošie faktori pacientiem ar epilepsiju

Ievads. Epilepsija ir viena no biežāk sastopamajām neiroloģiskajām slimībām un saistīta ar daudzām komorbiditātēm, kas būtiski pasliktina epilepsijas pacientu dzīves kvalitāti. Miega traucējumi ir viena no šādām komorbiditātēm, tomēr miega un epilepsijas savstarpējā saistība ir vēl līdz galam neskaidra, un nepieciešami turpmāki pētījumi, lai to labāk izprastu. Mērķis. Noskaidrot miega traucējumu prevalenci epilepsijas pacientu vidū un miegu ietekmējošo faktoru nozīmi miega traucējumu izplatībā. Materiāli un metodes. Pētījums no 2022. līdz 2023. gadam norisinājās Rīgas Austrumu klīniskās universitātes slimnīcā, pētījumā tika iekļauti 46 epilepsijas pacienti. Pētījuma dalībnieki tika anketēti, izmantojot četras anketas – pētījuma anketu, Epvorta miegainības skalu (ESS), Atēnu bezmiega skalu (AIS) un Ģeneralizētas trauksmes izvērtēšanas skalu (GAD-7 ). Iegūtie dati tika apkopoti Microsoft Excel programmā un analizēti, izmantojot statistiskās analīzes programmu IBM SPSS v 29.0. Rezultāti. 58.7% (n=27) pētījuma dalībnieku bija sievietes, bet 41.3% (n=19) bija vīrieši. Sūdzības par miega traucējumiem bija 72.0% (n=33) pētījuma dalībnieku, biežāk sastopams epilepsijas pacientu vidū bija bezmiegs (AIS ≥ 10) (45.7%, n=21), nedaudz retāk tika novērota pastiprināta miegainība dienas laikā (ESS ≥ 10) (32.6%, n=15). Vidējais iegūtais rezultāts ESS bija 8.4 punkti (SD = 4.1, mediāna – 9.0 punkti). AIS iegūtais vidējais punktu skaits bija 8.5 punkti (SD = 5.0, mediāna – 8.5 punkti). Tika atrasta statistiski nozīmīga sakarība starp bezmiegu un epilepsijas veidu (p=0.038), kā arī starp sūdzībām par miega traucējumiem un lēkmju biežumu (p=0.034). Tika noteikta statistiski nozīmīga korelācija, salīdzinot EES un GAD-7 skalā iegūtos rezultātus (r2 = 0.478, p < 0.001). Secinājumi. Miega traucējumi ir plaši izplatīta problēma epilepsijas pacientu vidū. Biežāk epilepsijas pacienti cieš no bezmiega, bet retāk no pastiprinātas miegainības dienas laikā. Epilepsijas lēkmes ir viens no miegu ietekmējošiem faktoriem, un biežas lēkmes ir riska faktors miega traucējumu attīstībai. Pacientiem ar ģeneralizētu un neprecizētu epilepsiju ir lielāks risks ciest no miega traucējumiem, salīdzinot ar pacientiem, kuriem ir fokāla epilepsija. Dati par pretepilepsijas medikamentu ietekmi uz miegu ir pretrunīgi, un būtu nepieciešami turpmāki pētījumi. Epilepsy is among the most common neurological diseases and is associated with many comorbidities that significantly impair the overall life quality in patients with this condition. Although sleep disorders is one of such comorbidities, the relationship between sleep and epilepsy is still unclear and further studies are required to better understand it. Aim. To determine the prevalence of sleep disorders among patients with epilepsy and the impact sleep factors have on the prevalence of sleep disorders. Materials and Methods. The study was conducted at Riga East University Hospital from 2022 to 2023 and included 46 epilepsy patients. Participants were interviewed by means of four questionnaires: the study questionnaire, Epworth Sleepiness Scale (ESS), Athens Insomnia Scale (AIS) and Generalized Anxiety Disorder scale (GAD-7). The data were collected in Microsoft Excel and analysed using the statistical analysis software IBM SPSS v 29.0. Results. 58.7% (n=27) of the study participants were female, and 41.3% (n=19) were male. Complaints of sleep disorders were present in 72.0% (n=33) of all study participants; insomnia (AIS ≥ 10) was more common among epilepsy patients (45.7%, n=21); increased daytime sleepiness (ESS ≥ 10) was slightly less common (32.6%, n=15). The mean score on the ESS was 8.4 (SD = 4.1, median 9.0). The mean score on the AIS was 8.5 (SD = 5.0, median - 8.5). Statistically significant correlation was found between insomnia and type of epilepsy (p=0.038) and between complaints of sleep disorders and seizure frequency (p=0.034). Statistically significant correlation was found between EES and GAD-7 scores (r2 = 0.478, p < 0.001). Conclusion. Sleep disorders are a widespread problem among epilepsy patients. It is more common for epilepsy patients to suffer from insomnia, whereas increased daytime sleepiness is less common. Epileptic seizures are one of the factors that affect sleep, and high seizure frequency is a risk factor for the onset of sleep disorders. Patients with generalised and unspecified epilepsy are at higher risk of suffering from sleep disorders compared to patients with focal epilepsy. Data on the effects of antiepileptic drugs on sleep are conflicting and further research is needed.
