Virtuālās realitātes tehnoloģiju pielietošana multiplās sklerozes pacientu diagnostikā

Ievads. Multiplās sklerozes diagnostikai ir pieejamas vairākas metodes, tajā skaitā funkcionālie testi, ar kuru palīdzību var izvērtēt pacienta stāvokli, slimības progresiju. Testus ir iespējams integrēt virtuālajā realitātē, radot aizraujošāku un atvieglotāku izvērtēšanas procesu pacientiem un personālam un ieviešot inovatīvus risinājumus neiroloģijas nozarē. Darba mērķis. Izpētīt virtuālās realitātes tehnoloģiju precizitāti multiplās sklerozes diagnostikā, izmantojot multiplās sklerozes funkcionālos testus un izvērtēt grūtības to izmantošanā. Metodes un materiāli. Pētījumā piedalījās 60 dalībnieki, vecuma grupā no 24 līdz 50 gadu vecumam. 30 multiplās sklerozes pacienti, kas ārstējas Latvijas Jūras Medicīnas centrā un 30 kontroles grupa, kas bija veseli cilvēki, bez kustību ierobežojumiem. (Ētikas komitejas atzinums Nr. 19-25/179). Pētījuma ietvaros tika veikta neiroloģiskā izvērtēšana, izmantojot tādus testus kā: SDMT, 9HPT, testus, kas izvērtē kognitīvo funkciju, atmiņu. Testi tika integrēti virtuālajā realitātē ar programmas Unity 3D palīdzību. Pēc diagnostikas veikšanas pacienti aizpildīja anketu par viņu pašsajūtu testu izpildes laikā. Rezultāti. Pirmo testu kontroles grupa veica ātrāk, salīdzinot ar MS grupu (p<0.004). Otrajā testā- SDMT MS grupā tika pieļautas vairāk kļūdas. Kontroles grupa 45 sekunžu laikā spēja savākt vairāk pariezu atbilžu, nekā MS grupa (p<0.001). Trešais un ceturtais tests neuzrādīja statistiski nozīmīgu atšķirību starp abām grupām (attiecīgi p=0.277 un p=0.174). Vecuma ietekme uz rezultātiem tika novērota tikai otrajā testā, pārējos statistiski nozīmīgas korelācijas netika konstatētas. Secinājumi. Pirmā un otrā testa rezultāti var liecināt par to, ka pacientiem skarta īslaicīgā atmiņa, kā arī ģeogrāfiskā orientēšanās spēja, ir samazināts smadzeņu informācijas apstrādes laiks. Trešais un ceturtais tests neuzrādīja statistiski nozīmīgu atšķirību starp divām grupām. Ir nepieciešama izvērtēšana ilgtermiņā, lai novērtētu slimības progresu. Kopējā cilvēku attieksme pret šāda veida tehnoloģijām ir pozitīva. Pētījumā izvirzītā hipotēze apstiprinājās. Introduction. Several methods are available for the diagnosis of multiple sclerosis, including functional tests, which can be used to evaluate the patient's condition and disease pro-gression. It is possible to integrate the tests into virtual reality, creating an exciting and easy evaluation process for patients and staff and introducing innovative solutions in the field of neurology. Aim. To investigate the accuracy of virtual reality technology in the diagnosis of multiple sclerosis using multiple sclerosis functional tests. Methods and materials. 60 participants in the age group from 24 to 50 take part in the study. 30 multiple sclerosis patients treated at the Latvian Maritime Medical Center and 30 in the control group, who were healthy people without movement restrictions. (Ethical Com-mittee Opionion Nr. 19-25/179). As part of the study, a neurological evaluation was performed using tests such as: SDMT, 9HPT, as well as tests that evaluate cognitive function, memory. The tests were integrated into virtual reality with the help of the Unity 3D pro-gram. After diagnosis, patients filled out a questionnaire about how they felt during the tests. Results. The control group completed the first test faster than the MS group each time (p<0.004). In the second test - SDMT MS group only made more mistakes. The control gro-up was able to collect more correct answers within 45 seconds than the MS group (p<0.001). The third and fourth tests showed no statistical difference between the two groups (p=0.277 and p=0.174, respectively). The effect of age on the observed was observed only in the se-cond test, statistically significant correlations were found in the others. Conclusions. The results of the first and second tests may indicate that the patients' short-term memory is affected, as well as the geographical orientation ability, the brain's infor-mation processing time is possible. The third and fourth tests showed no statistically significant difference between the two groups. Long-term evaluation is needed to assess disease progression. The general attitude of people towards this type of technology is positive. The hypothesis proposed in the study was confirmed.
