dc.description.abstract | Diplomdarba tēma – HPV kā prognotiskais marķieris mandeļu vēzim. Tēmas aktualitāti nosaka mandeļu vēža pieaugums, īpaši saistībā ar cilvēka papilomas vīrusu. Lai gan mandeļu vēzis ir visizplatītākais no mutes un kakla audzējiem, tas parasti ir ticis saistīts ar vecāka gada gājuma cilvēkiem, kuri ir hroniski smēķētāji un alkohola lietotāji, taču pēdējos gados arī HPV tiek saistīts kā viens no nopietnākajiem riska faktoriem, īpaši jaunākiem pacientiem. Pētījuma mērķis - salīdzināt HPV pozitīva un HPV negatīva mandeļu vēža saslimstības un mirstības rādītājus Latvijas Onkoloģijas centrā no 2020.gada līdz 2022.gadam. Kā arī izvērtēt, vai ir tieša saistība star pacienta prognozi, metastāžu biežumu, recidīvu esamību un cilvēka papilomas vīrusa esamību. Pētījumā izmantota kvantitatīva pētniecības metode, pētījuma instruments – pacienta slimības vēsture. Pētījuma bāze – Latvijas Onkoloģijas centrs, Galvas un kakla ķirurģijas nodaļa. Laika periodā no 2020.gada līdz 2022.gadam pētījumā iekļauti 74 pacienti un analizēti dati par pacientu vecumu, dzimumu, HPV infekcijas esamību, metastāžu esamību, slimības recidīvu, slimības stadiju diagnozes brīdī, komplikācijām, terapiju un slimības iznākumu. Pētījuma rezultāti - Kopā pētījumā tika iekļauti un analizēti 74 pacienti ar mandeļu vēzi Latvijas Onkoloģijas centra, Galvas un kakla ķirurģijas nodaļā. Pētījumā piedalījās 46 (62.2%) vīrieši un 28 (37.8%) sievietes. Vidējais pacienta vecums bija 58,2 ± 9.5 gadi, maksimālais pacienta vecums bija 81 gads, jaunākais pacients bija 35 gadus vecs. No visiem pētījumā iekļautajiem pacientiem 32 jeb 43.2% bija HPV pozitīvs audzējs un 42 jeb 56.8% ir bijis HPV negatīvs audzējs. | |
dc.description.abstract | Topic of the thesis – HPV as a prognostic marker for tonsil cancer. Topics significance is determined by the increase in tonsil cancer, especially with the human papillomavirus. Although tonsil cancer is the most common of the mouth and throat cancers, it is usually associated with older people who are chronic smokers and drinkers, but in recent years, HPV has also been linked as one of the most serious risk factors, especially in younger patients. The purpose of the study is to compare the morbidity and mortality rates of HPV-positive and HPV-negative tonsil cancer in the Latvian Oncology Center from 2020 to 2022. As well as to evaluate whether there is a direct relationship between the patient's prognosis, the frequency of metastases, the presence of recurrences and the presence of human papillomavirus. The research uses a quantitative research method; the research tool is the patient's medical history. Research base – Latvian Oncology Center, Department of Head and Neck Surgery. In the time period from 2020 to 2022, the study included 74 patients and analyzed data on patient age, gender, presence of HPV infection, presence of metastases, recurrence of the disease, stage of the disease at the time of diagnosis, complications, therapy and outcome of the disease. Results of the study - A total of 74 patients with tonsil cancer were included and analyzed in the Head and Neck Surgery Department of the Latvian Oncology Center. 46 (62.2%) men and 28 (37.8%) women participated in the study. The mean patient age was 58.2 ± 9.5 years, the maximum patient age was 81 years, and the youngest patient was 35 years old. Of all patients included in the study, 32 or 43.2% of patients had an HPV-positive tumor and 42 or 56.8% of patients had an HPV-negative tumor. | |
