Lipoproteīna (a) izvērtējums pacientiem ar smagu hiperholesterinēmiju

Lipoproteīns(a) (Lp(a)) ir ģenētiski noteikts riska faktors atersklerotisku kardiovaskulāru slimību attīstībai. Šobrīd nav pieejamu medikamentu Lp(a) līmeņa un tā radītā riska samazināšanai. Pētījuma mērķis: Raksturot Lp(a) līmeņus pacientiem ar smagu hiperholesterinēmiju, kas iekļauti Latvijas Ģimenes hiperholesterinēmijas reģistrā, un izvērtēt tā iespējamo asociāciju ar koronāro sirds slimību un holesterīnu pazeminošās terapijas efektivitāti. Metodes: Retrospektīvā pētījumā tika iekļauti 395 pacienti (47,5 ± 12 gadi; 37% vīriešu) ar smagu hiperholesterinēmiju un vismaz vienreiz noteiktu Lp(a) līmeni. Pacienti tika izvērtēti pēc Nīderlandes Lipīdu klīniku (NLK) tīkla kritērijiem ģimenes hiperholesterinēmijas diagnozes noteikšanai. Aterosklerotisku kardiovaskulāru slimību diagnozes tika noteiktas izmantojot pacientu anamnēzi un/vai pieejamos attēldiagnostikas izmeklējumus. Rezultāti: Mediānais Lp(a) līmenis pētījuma populācijā bija 23,9 mg/dL. Lielākajai daļai pacientu (n=214, 54,2%) Lp(a) līmenis bija zems <30 mg/dL, Lp(a) līmenis 30-50 mg/dL bija 27 (6,8%) pacientiem, bet augsts Lp(a) līmenis >50 mg/dL bija 154 pacientiem (39,0%). Netika atrasta Lp(a) līmeņa asociācija ar vecumu, dzimumu un NLK tīkla kritērijiem. Starp Lp(a) līmeni >180 mg/dL un koronāru sirds slimību (p<0,001), un agrīnu koronāru sirds slimību (p<0,001) tika atrasta pozitīva nozīmīga asociācija, bet starp Lp(a) > 180 mg/dL un miokarda infarktu tika atrasta nozīmīga inversa asociācija (p<0,001). Lp(a) saturošais holesterīns nozīmīgi ietekmēja ZBL holesterīna līmeni 48 (12,5%) pacientu, pārliekot tos kategorijā ar ZBLH <5 mmol/L. Pēc hipolipidemizējošās terapijas lietošanas mediānais ZBLH samazinājums ir nozīmīgi lielāks, ja Lp(a) <30 mg/dL, salīdzinot ar grupu, kurai Lp(a) >50 mg/dL, attiecīgi mediānais ZBLHsam 4,01 mmol/L un mediānais ZBLHsam 3,46 mmol/L (p=0,021). Secinājumi: Pacientiem ar izteiktu hiperholesterinēmiju salīdzinoši bieži ir sastopams augsts Lp(a) līmenis. Šajā pētījuma populācijā tika noteikta nozīmīga pozitīva asociācija starp Lp(a) >180 mgdL un koronāru sirds slimību, kā arī agrīnu koronāru sirds slimību. Pacientiem ar Lp(a) <30 mg/dL hipolipidemizējošās terapijas efektivitāte ir lielāka nekā pacientiem ar Lp(a) līmeni >50 mg/dL. Background: Lipoprotein(a) (Lp(a)) is a genetically determined risk factor for atherosclerotic cardiovascular disease. There are no available treatments for minimizing Lp(a) levels and associated risks for disease caused by elevated Lp(a). Objective: To evaluate Lp(a) levels in patients with severe hypercholesterolemia included in Latvian Registry of Familial Hypercholesterolemia and to determine probable association between Lp(a) and coronary heart disease and efficiency of lipid lowering therapy. Methods: In this retrospective study 395 patients were included (47,5 ± 12 years; 37% male) who had severe hypercholesterolemia and Lp(a) measurement at least once. Patients were evaluated according to Dutch Lipid Clinic (DLC) network criteria for diagnosis of familial hypercholesterolemia. Atherosclerotic cardiovascular disease was determined according to patient history and/or using imaging studies. Results: Median Lp(a) level was 23,9 mg/dL. Most patients (n=214, 54,2%) had Lp(a) level <30 mg/dL, 27 (6,8%) patients had Lp(a) level 30-50 mg/dL. High Lp(a) level was found in 154 (39,0%) patients. There was no association between Lp(a) and age, sex and DLC network criteria. There was positive significant association between Lp(a) >180 mg/dL and coronary heart disease (p<0,001) and premature coronary heart disease (p<0,001), but Lp(a) >180 mg/dL was inversely associated with myocardial infarction. Cholesterol found in Lp(a) changed LDL cholesterol (LDL-C) levels for 48 (12,5%) patients, reassigning them to category with LDL-C <5 mmol/L. After using lipid lowering therapy median LDL-C reduction was significantly bigger in group with Lp(a) < 30 mg/dL comparing to group with Lp(a) level >50 mg/dL. Conclusions: In patients with severe hypercholesterolemia elevated Lp(a) can often be found. In this study there was an association between Lp(a) level >180 mg/dL and coronary heart disease and premature coronary heart disease. In patients with Lp(a) <30 mg/dL (median LDL-C lowered by 4,01 mmol/L) lipid lowering therapy is more effective compared to patients with Lp(a) level >50 mg/dL (median LDL-C lowered by 3,46 mmol/L) (p=0,021)
